Laparoscopic robotic-assisted "keel" bladder neck construction.

INTRODUCTION: Robotic technology has gained popularity allowing performance of several complex and difficult reconstructive procedures. This video demonstrates the advantages of the robotic approach for a "keel" bladder neck construction in an obese patient. MATERIAL AND METHODS: A 13 year-old-girl (BMI = 28) already treated elsewhere for a uro-genital sinus (Total Urogenital Mobilization) has been evaluated for disturbing persistent total urinary incontinence. Cisto-colposcopy revealed an extremely short and tortuous urethra. Bladder neck was wide open and incompetent. Video-Urodynamic evalu

Outcome of continence procedures after failed endoscopic treatment with dextranomer-based implants (DEFLUX®).

PURPOSE: To evaluate outcome of further continence procedures after failure of endoscopic injections of dextranomer-based bulking agent. MATERIALS AND METHODS: From 1997, 89 children (3-18 years) and one young adult were treated for incontinence with 145 endoscopic injections of dextranomer. On evaluation, each patient was classified as: dry, significantly improved, or treatment failure. Eighty-five patients had at least 12 months of follow up. Out of 34 (40%) treatment failures, 24 patients had a subsequent bladder neck procedure: artificial urinary sphincter (7), bladder neck plasty (9), bl

[Surgical management of entero-vesical fistulas in Crohn's disease].

BACKGROUND: Crohn's disease (CD) is a chronic inflammatory disease of the intestine that can cause an attack by contiguity of the urinary tract. Although the shape is common and fistulizing 35% of all patients with CD, entero-urinary fistulas are rare and only seen in 2-8% of patients. aim: To report the frequency of occurrence of this complication among the group of surgical forms of CD. Describe the different pathophysiological mechanisms of occurrence of entero-vesical fistula (EVF) during the CD. methods: We report, retrospectively, seven observations of EVF complicating MC made during the

Limited value of the Fairley test in urologic infections in patients with neuropathic bladders.

In an attempt to localize sites of urinary tract infection, a combined Fairley washout test and prostatic localization was performed on 110 spinal cord injured (SCI) patients with neurogenic bladder dysfunction and a history of recurrent bacteriuria. Analysis of 100 patients demonstrated both single and double site of localization. Single sites of localization involved: upper tract 21 of 92 (23%), bladder 15 of 92 (16%), prostate eight of 92 (9%), and 48 of 92 (52%) undetermined. In eight cases, double site of localization was documented: prostate and upper tract six of eight (75%), prostate a

Bladder exstrophy: current management and postoperative imaging.

Bladder exstrophy is a rare malformation characterized by an infra-umbilical abdominal wall defect, incomplete closure of the bladder with mucosa continuous with the abdominal wall, epispadias, and alterations in the pelvic bones and muscles. It is part of the exstrophy-epispadias complex, with cloacal exstrophy on the severe and epispadias on the mild ends of the spectrum. Bladder exstrophy is the most common of these entities and is more common in boys. The goal of this paper is to describe common methods of repair and to provide an imaging review of the postoperative appearances.

Effect of alpha1 adrenoceptor antagonist on the urodynamics of the upper and lower urinary tract of the male rat.

In this investigation, we examined the impact of the alpha1 adrenoceptor (alpha1-ADR) antagonist prazosin on the urodynamic characteristics of upper urinary tract function and associated micturition characteristics of the adult male rat. The focus of the study was to evaluate the extent to which prazosin affects urine production and ureteral transport relative to its effect on micturition. Control micturition studies were first performed using 28 awake Sprague-Dawley rats that were placed in metabolic cages for characterization of the frequency and mean and total volume voided over a 4-hr peri

Delayed presentation of intraperitoneal bladder perforation after blunt abdominal injury mimicking an acute abdomen: A case report and literature review.

INTRODUCTION: Urinary tract injuries occur in 10 % to 15 % of cases of blunt or penetrating abdominal trauma. Urinary bladder perforation happens in 15 % of pelvic fractures and can be intraperitoneal, extraperitoneal, or mixed. Delayed diagnosis of an intraperitoneal bladder perforation is uncommon in the post-computer tomography era. Failure to identify and treat it promptly could lead to a potentially fatal complication. CASE PRESENTATION: We report a case of a 38-year-old man with a delayed diagnosis of traumatic intraperitoneal bladder perforation, occurring two weeks after a blunt assau…

Interactions between vulvovaginal disorders and urinary disorders: The case for an integrated view of the pelvis.

Lower urinary tract symptomatology is often difficult to categorize if history and investigation focus only on the urinary tract. Disease and dysfunction in organs more posteriorly can often cause or influence such bladder and urethral symptoms. Vulvovaginal skin diseases are an important but often missed influence on lower urinary tract symptomatology.

LRIG2 mutations cause urofacial syndrome.

Urofacial syndrome (UFS) (or Ochoa syndrome) is an autosomal-recessive disease characterized by congenital urinary bladder dysfunction, associated with a significant risk of kidney failure, and an abnormal facial expression upon smiling, laughing, and crying. We report that a subset of UFS-affected individuals have biallelic mutations in LRIG2, encoding leucine-rich repeats and immunoglobulin-like domains 2, a protein implicated in neural cell signaling and tumorigenesis. Importantly, we have demonstrated that rare variants in LRIG2 might be relevant to nonsyndromic bladder disease. We have pr…

A rare case of emphysematous cystitis and intravesical fungus balls caused by Candida tropicalis.

BACKGROUND: The concurrent fungal emphysematous cystitis (EC) and bladder fungus balls in patients with urinary tract infection is rare. Diagnosis and management remain challenging because of nonspecific clinical manifestations, limited imaging features, and the lack of standardized treatment strategies. CASE PRESENTATION: A 62-year-old woman with poorly controlled type 2 diabetes mellitus (DM) presented with EC and bladder fungus balls caused by Candida tropicalis. Imaging revealed an intravesical mass surrounded by a large amount of gas. Conservative management, including antifungal therapy

Quick note on tissue engineering-based surgical measures to treat patients with neurogenic bladder-due detrusor/sphincter dyssynergia.

To treat the neurogenic bladder-due detrusor/urethral rhabdosphincter dyssynergia, early combined clean intermittent catheterization/ pharmacotherapy (anticholinergic-, β3-adrenoceptor agonist drugs) management may be at times crowned with success of preserving an adequate bladder compliance and renal safe conditions. The persistence, instead, of elevated bladder filling pressure levels with high voiding pressure/uroflow values, together with aberrant urethral rhabdosphincter electromyographic findings, make necessary the resort to surgery strategies, among which - a part from rhabdosphincterot

Neural Mechanisms of Poststroke Urinary Incontinence: Results From an fMRI Study.

BACKGROUND: Urinary incontinence after a stroke significantly affects patient outcomes and quality of life. It is commonly associated with uninhibited detrusor contractions, but the underlying neural mechanisms remain poorly understood. This study aimed to explore the brain activity patterns associated with volitional and involuntary bladder contractions in stroke survivors. METHODS: This cohort study enrolled 15 stroke survivors with documented urinary incontinence and 9 healthy controls. Participants underwent simultaneous blood oxygen level-dependent functional magnetic resonance imaging o

Resection of Bilateral Endometriosis of the Uterosacral Ligaments Resolves Voiding Dysfunction: A Case Report.

Patients with deep endometriosis of the posterior pelvic compartment might present various lower urinary tract symptoms, including urinary retention, without any prior pelvic surgery in their medical history. These symptoms, including urinary retention, pollakiuria or urge incontinence, urinary urgency, and decreased bladder sensitivity, imply an infiltration of the pelvic autonomic nerves by endometriosis lesions, in combination with inflammatory phenomena. Although there are numerous publications dealing with de novo urinary dysfunction after endometriosis surgery, significantly less scienti
