Autologous multiantigen-targeted T cell therapy for pancreatic cancer: a phase 1/2 trial.

T cell therapy has proven challenging for pancreatic ductal adenocarcinoma (PDAC), partly due to heterogeneous expression of tumor-associated antigens (TAAs). To address tumor heterogeneity and mitigate immune evasion, an ex vivo expanded, polyclonal, T helper 1 cell-polarized T cell product targeting five TAAs-PRAME, SSX2, MAGEA4, Survivin and NY-ESO-1-was developed. These antigens were chosen based on their tumor specificity, oncogenicity, immunogenicity and level of expression. In a phase 1/2 trial, this autologous nonengineered T cell product was administered (1 × 107 cells m-2 per infusio

Automated manufacturing of clinical-grade BDCA2 CAR NK cells in a closed system for the treatment of blastic plasmacytoid dendritic cell neoplasm.

Recent progress in chimeric antigen receptor (CAR) natural killer (NK) cell therapy has demonstrated their promising potential in cancer immunotherapy. However, most current CAR NK cell manufacturing processes utilize open systems with multiple manual steps, making it challenging to maintain consistent therapeutic quality and regulatory compliance for clinical applications. We specifically developed blood dendritic cell antigen 2 (BDCA2)-targeting CAR NK cells for treating blastic plasmacytoid dendritic cell neoplasm (BPDCN). Here, we present an automated, current good manufacturing practice (

Anti-CD19 CAR T-cell therapy for autoimmune diseases: from exploratory clinical trials to quantitative pharmacodynamic modeling development.

Anti-CD19 CAR T-cell therapy has produced major advances in treating hematologic malignancies. Recently, researchers have explored its use in autoimmune diseases. However, the complex pathogenesis and high heterogeneity of these disorders create substantial uncertainty about treatment efficacy and safety in clinical settings. Quantitative systems pharmacology (QSP) models integrate multiple pathophysiological mechanisms and map real patients onto virtual "digital twins" to simulate the in vivo behavior of a given CAR T-cell dose after infusion and to predict benefits and risks. Such models can

TP53 mutations and baseline lymphocytes indicate response to glofitamab in relapsed or refractory diffuse large B-cell lymphoma.

Although CAR-T cell therapy has emerged as a promising therapeutic option for relapsed/refractory (R/R) diffuse large B-cell lymphoma (DLBCL), long manufacturing time, high cost and toxicities limit its clinical use. T-cell engagers (TCEs) including glofitamab may address these unmet needs, but real-world data remain scarce. This real-world study (NCT06481826) enrolled 30 Chinese patients. The median prior treatment lines were 2 (2-5) and 90% had no prior CAR-T therapy. After a median follow-up of 15 months, the overall response rate (ORR) was 66.7%. The 1-year progression-free survival (PFS)

High-Content SRS Imaging Unveils Altered Cholesterol Metabolism in Ovarian Cancers Under CAR-T Treatment.

Ovarian cancer is one of the most lethal gynecological cancers worldwide and has one of the highest recurrence rates. Recently developed Chimeric Antigen Receptor (CAR) -T cell therapy has shown potent clinical efficacy against hematological malignancies. However, solid tumors, including ovarian cancer, possess several mechanisms that hinder T cell activity, and metabolic alteration of cancer cells has been shown to contribute to resistance to immune cell attack against solid tumors. Here, we explore the metabolic response of ovarian cancer cells to CAR-T cell attack using label-free high-cont

CAR T cell-derived TNF is an early determinant of cytokine release syndrome severity.

Cytokine release syndrome (CRS) is a common and potentially severe toxicity of chimeric antigen receptor (CAR) T cell therapy, characterized by activation of the host myeloid compartment and systemic inflammation. Although downstream effectors such as interleukin-6 (IL-6) and IL-1β are well-characterized, the upstream signals that initiate CRS remain incompletely understood. By selectively disrupting tumor necrosis factor (TNF) signaling in a mouse model of CRS, we show that CAR T cell-derived TNF promotes the accumulation of pro-inflammatory monocyte-derived macrophages at the tumor site and

Why CAR T cell therapy fails in renal cell carcinoma.

Chimeric antigen receptor (CAR) T cell therapy has transformed the treatment landscape of hematologic malignancies, yet its clinical efficacy in solid tumors remains limited. Renal cell carcinoma (RCC) presents a striking paradox: despite its established responsiveness to immune modulation and the expression of targetable tumor antigens, CAR-T therapies have failed to produce durable clinical benefit. This failure has often been attributed to antigen heterogeneity or lack of tumor specificity; however, accumulating clinical and experimental evidence suggests that antigen recognition alone does

Anti-CD19 CAR T cells for pediatric patients with treatment-refractory autoimmune diseases.

Chimeric antigen receptor (CAR) T cell therapy was recently proposed as a treatment for adults with B-cell-mediated autoimmune diseases (ADs) refractory to conventional immunomodulatory therapy. We present a case series of eight children with severe/refractory AD (four systemic lupus erythematosus, three dermatomyositis, one systemic sclerosis) treated at Ospedale Pediatrico Bambino Gesù, Rome, and University Hospital Erlangen with a single infusion of 1 × 106 kg-1 point-of-care manufactured autologous CD19 CAR T cells (zorpocabtagene autoleucel), in a hospital exemption (HE) program. Low-density neutrophils preferentially infiltrate the skin compared to conventional neutrophils in an experimental psoriasis model.

Neutrophils infiltrate lesional skin robustly in individuals with psoriasis. However, their role in chronically inflamed skin-particularly in terms of phenotypic and functional diversity-remains poorly understood. In this study, we investigated the functional potential of 2 distinct circulating neutrophil populations-conventional, polymorphonuclear neutrophils (cNeu) and low-density neutrophils (LDNeu)-to infiltrate lesional skin in an experimental model of psoriasis. We found that, similar to human psoriasis, imiquimod-induced psoriasis-like dermatitis in mice is associated with elevated leve

Association of baseline immune cell composition with CAR-T cell expansion and survival in Relapsed/Refractory large B-Cell lymphoma.

BACKGROUND: Chimeric antigen receptor T (CAR-T) cell therapy has demonstrated remarkable efficacy in relapsed/refractory large B-cell lymphoma (R/R LBCL), yet nearly 40-60% of patients fail to achieve durable responses. The mechanisms underlying interpatient variability in CAR-T expansion and persistence remain incompletely understood. In this exploratory study, we preliminarily investigated the associations between baseline peripheral blood immune subsets, CAR-T expansion kinetics, and clinical outcomes. METHODS: We retrospectively analyzed 33 patients with R/R LBCL who received CD19/CD22 bi

Myeloid-associated B-cell identity remodeling in CAR T-cell resistance: a hypothesis-generating framework for diffuse large B-cell lymphoma.

CD19-directed chimeric antigen receptor (CAR) T-cell therapy has transformed the treatment of relapsed or refractory large B-cell lymphoma (LBCL), including diffuse large B-cell lymphoma (DLBCL), yet primary resistance and relapse remain frequent. Clinical studies link pretreatment B-cell gene expression signatures, CD19 gene expression, malignant-cell CD19 protein expression, immune contexture and myeloid/inflammatory features with CAR T-cell outcomes, but they do not establish that myeloid cells directly induce loss of B-cell identity. In this Mini Review, we use B-cell identity remodeling a

Patient-Specific Determinants of Response to BCMA- and GPRC5D-Targeted CAR T-Cell Therapy in Multiple Myeloma: A QSP Analysis of Clinical Trial and Real-World Data.

Despite promising outcomes in CAR T-cell therapy for relapsed/refractory multiple myeloma (RRMM), nearly all patients eventually relapse. Resistance and relapse may be driven by CAR T-cell and tumor-intrinsic factors. Here, we developed a mechanistic quantitative systems pharmacology (QSP) model of multiple myeloma growth and CAR T-cell therapy using measurable biomarkers to predict and identify factors associated with response and relapse. The model incorporates key components to explore disease dynamics and CAR T-cell expansion. Our model reproduced published pharmacokinetics and biomarker r

PARP Inhibitors plus Anlotinib as Bridging Therapy for Armored CAR-T Cells in Ovarian Cancer Enhances Infiltration and Antitumor Efficacy.

Chimeric antigen receptor (CAR)-T cell therapy is largely ineffective in most solid tumors, partially because of inadequate intratumoral trafficking. Bridging therapy, administered between leukapheresis and CAR-T cell infusion, offers a distinct opportunity to control the disease and precondition the tumor microenvironment without directly exposing CAR-T cells to concomitant drugs. Here, a poly (ADP-ribose) polymerase (PARP) inhibitor combined with anlotinib is evaluated as bridging therapy for ovarian cancer. In immunocompetent mice bearing ovarian tumors, this regimen induces durable intratu

Engineering CAR-T cells for solid tumors: overcoming antigenic, trafficking, and microenvironmental barriers.

Chimeric antigen receptor T-cell (CAR-T) therapy has demonstrated remarkable clinical efficacy across a spectrum of hematological malignancies, with particularly profound responses observed in B-cell leukemias, lymphomas, and multiple myeloma. However, the clinical benefits of CAR-T cell therapy have not yet been effectively extended to most solid tumors. This limitation arises from multiple biological and structural barriers, including the paucity of truly tumor-specific antigens, variable antigen expression and loss, inefficient trafficking and infiltration, and the presence of a highly immu

Multi-Omics-Enabled Precision Strategies for Overcoming CAR-T Therapy Limitations in Gastrointestinal Malignancies.

Gastrointestinal malignancies, including gastric cancer, colorectal cancer, hepatocellular carcinoma, and pancreatic ductal adenocarcinoma, remain major causes of cancer-related morbidity and mortality worldwide. Although chimeric antigen receptor T-cell (CAR-T) therapy has revolutionized the treatment of hematologic malignancies, its efficacy in gastrointestinal solid tumors remains limited by antigen heterogeneity, insufficient trafficking and infiltration, immunosuppressive tumor microenvironments, on-target off-tumor toxicity, and adaptive resistance. In this review, we summarize the curre

In vivo CAR T-cell generation: delivery platforms, clinical progress, and translational barriers.

Chimeric antigen receptor (CAR) T-cell therapy has transformed the treatment of several hematological malignancies, but its broader application remains constrained by the complexity, cost, and time required for conventional ex vivo manufacturing. In vivo CAR T-cell therapy has emerged as a promising next-generation strategy that aims to generate CAR T cells directly within the patient through targeted delivery of CAR-encoding genetic information to endogenous T cells. This approach has the potential to simplify treatment workflows, shorten manufacturing timelines, reduce production costs, and
