Inhibition of Na(+)/H(+) exchanger type 3 reduces duration of apnea induced by laryngeal stimulation in piglets.

Reflexes from the larynx induce cessation of breathing in newborn animals. The magnitude of respiratory inhibition is inversely related to the level of central chemical input. Recent studies indicate that selective inhibition of Na(+)/H(+) exchanger type 3 (NHE3) activates CO(2)/H(+)-sensitive neurons, resembling the responses evoked by hypercapnic stimuli. Hence, the use of NHE3 inhibitors may reduce reflexly mediated respiratory depression and duration of apnea in the neonatal period. This possibility was examined in decerebrate, vagotomized, ventilated, and paralyzed piglets by testing the …

Cardiac rhythm disturbances in the obstructive sleep apnea syndrome: effects of nasal continuous positive airway pressure therapy.

STUDY OBJECTIVES: A high incidence of nocturnal cardiac rhythm disturbances among patients with obstructive sleep apnea (OSA) syndrome has been described in some reports, but not in others. We wished to examine the prevalence of significant cardiac rhythm disturbance in patients with established moderate to severe OSA syndrome and, in particular, to assess the impact of nasal continuous positive airway pressure (nCPAP) therapy. DESIGN AND SETTING: A prospective study of consecutive eligible patients in a dedicated sleep disorders unit of a university teaching hospital. MEASUREMENTS AND RESUL…

Nasal intermittent positive pressure ventilation (NIPPV) versus nasal continuous positive airway pressure (NCPAP) for apnea of prematurity.

BACKGROUND: Apnea of prematurity is almost universal in infants who are born before 34 weeks gestation. Previous randomised trials and systematic reviews have found methylxanthines to be effective in preventing apnea of prematurity. However, recent concerns about potential long term side effects of methylxanthines on the neurodevelopment of low birth weight infants have led to an increased interest in alternate methods of treating apnea of prematurity. Nasal continuous positive airway pressure (NCPAP) is a useful method of respiratory support which reduces the incidence of obstructive or mixed…

Magnetic resonance imaging of the upper airway in obstructive sleep apnea before and after oral appliance therapy.

OBJECTIVE: To determine the mechanism by which an oral appliance may be used to treat obstructive sleep apnea syndrome (OSAS). METHODS: Eleven OSAS patients (8 males and 3 females) were involved in the study. Mean age was 52.2 +/- 10.6 years; height was 166.6 +/- 7.2 cm, and weight was 75.6 +/- 9.3 kg; body mass index (BMI) was 27.2 +/- 2.9 kg/m2. Each patient underwent magnetic resonance imaging (MRI) and polysomnography before and after oral appliance therapy. Pharyngeal changes were measured and compared with the variation in sleep parameters. Also, Pearson's correlation and multiple linea…

Paraoperative change of sleep-disordered breathing in healthy snorers and sleep apnea patients compared to preoperative values.

BACKGROUND: Patients with sleep-disordered breathing often show upper airway narrowing due to anatomical factors, which can be treated surgically. The paraoperative risk for sleep apnea patients is discussed from different points of view. We studied the effects of general anesthesia which is mainly used for surgery on the degree of disordered breathing. METHODS: After two nights of polysomnographie 20 healthy subjects with snoring, light or severe sleep apnea were scheduled for surgery of the nose, the paranasal sinuses, the tonsils and/or the soft palate. A common screening device for disord…

Sleep-related myocardial ischemia and sleep structure in patients with obstructive sleep apnea and coronary heart disease.

STUDY OBJECTIVES: Patients with coronary heart disease (CHD) and obstructive sleep apnea may have an increased cardiac risk due to nocturnal myocardial ischemia triggered by apnea-associated oxygen desaturation. Sleep structure in patients with obstructive sleep apnea is fragmented by activation of the central nervous system (CNS) (arousal) due to obstructive apneas. Nocturnal myocardial ischemia may lead to activation of the CNS as well. PATIENTS: Fourteen patients with obstructive sleep apnea and CHD disease and seven patients suffering from obstructive sleep apnea without CHD were studied.…

[Central sleep apnea syndrome successfully treated with nasal bi-level positive airway pressure and sleep position adjustment].

A 55-year-old obese man was admitted to our hospital because of a severe morning headache. He snored and had recurrent episodes of sleep apnea that began 10 years earlier and had since become much worse. An overnight polysomnographic recording confirmed that he had sleep apnea syndrome, predominantly of the central type. The apneas were more frequent when he lay on his back (apnea index 54.5) than on his side (apnea index 1.2). He was treated with sleep position adjustment and nasal bi-level positive airway pressure, inspiratory positive airway pressure at 5 cmH2O and expiratory positive airwa…

[Apnea characteristics of children who later died: comparison of sudden infant death with other causes of death].

Apnea characteristics of infants, who were destined to die, were compared with control infants, matched for gestational age, sex, birth weight and age at sleep study. From 1989 to 1994 polygraphic recordings were performed in 4268 infants. In this population 7 cases of sudden infant death (SID) and 13 deaths from other causes (non-SID) occurred. Concerning all forms of apneas a higher frequency and maximal duration was found in the SID group. The number of infants with obstructive apneas and the number of obstructive apneas per infant were higher in future SID victims, as was the frequency, me…

Predictive factors of long-term compliance with nasal continuous positive airway pressure treatment in sleep apnea syndrome.

The long-term acceptability of treatment with nasal continuous positive airway pressure (CPAP) was studied prospectively in 44 patients with obstructive sleep apnea syndrome. At 14 months on the average after starting treatment with CPAP, 30 patients (68 percent) were found to be compliant (characterized by use of the apparatus every night throughout the night, for more than 5 h per night). The daily use of nasal CPAP was significantly correlated to the initial apnea/hypopnea index (p = 0.013; r = 0.37), as well as to the percentage of light sleep (p = 0.045; r = 0.30) and slow-wave sleep (p =…

[Functional nasopharyngeal fiberoptic endoscopy for pre-therapeutic diagnosis of sleep apnea syndrome in infants. A case report].

We report a 2-year-old infant with severe obstructive sleep apnoea. The symptoms had deteriorated for several months, and indicated complete obstruction shortly after the child fell asleep, with reduction of the oxygen saturation to under 30%. Since the obstruction could only be interrupted by waking the child, a tracheostomy was proposed. Endoscopy under general anaesthesia revealed no pathological findings. The stenosis could only be seen using transnasal fibre-optic endoscopy when the obstruction occurred during sleep: the oropharyngeal wall collapsed at the level of the velopharyngeal sphi

Periodic sleep apnea: chronic sleep deprivation related to intermittent upper airway obstruction and central nervous system disturbance.

Periodic sleep apnea may be due to repeated episodes of upper airway obstruction in patients who have a short thick neck and/or large jowls. Apnea due to complete cessation of breathing may occur to a lesser extent. Anaylsis of the sleep electroencephalogram shows that these patients rarely achieve deep sleep and have less stage 1-REM sleep than normal subjects of comparable age. They are chronically sleep-deprived, a manifestation expressed by daytime somnolence, chronic fatigue and often by personality disturbances marked by paranoia, agitated depression and hostility. The definitive diagnos…

Mandibular Advancement Devices With Remote Monitoring Technologies: Expanding the Dentist's Role in the Management of Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is a prevalent disorder with significant systemic and quality-of-life consequences. Mandibular advancement devices (MADs) are an established treatment option, but conventional follow-up often relies on intermittent visits and self-reported adherence. Sensor-integrated MADs and remote monitoring may enable objective adherence assessment and personalized longitudinal care, although their additional clinical value remains uncertain. This narrative review critically appraises current evidence on sensor-integrated MADs with remote monitoring for OSA, focusing on treatm…

A Fine-Grained Diagnostic Method for Sleep Apnea Based on Target Detection Network With SpO2 and Nasal Airflow.

The extant automatic diagnostic technologies for sleep apnea primarily focus on the accuracy of apnea-hypopnea index (AHI) estimation and severity classification, ignoring the accurate annotation for abnormal respiratory events. Motivated by target detection principle, we proposed a fine-grained diagnostic method called apnea-hypopnea event detector (AHE-Detector) in this paper. Through fusing time-frequency features of pulse oxygen saturation and nasal airflow signal, our model can accurately identify abnormal respiratory events on the continuous time axis. The model specifically contains thr…
