[CME: Fear of Flying and Flying Phobia].

CME: Fear of Flying and Flying Phobia Abstract. Fear of flying and flying phobia are very common in the general population and can lead to substantial distress. Symptoms and diagnosis of fear of flying and flying phobia will be described. Severe fear of flying can be diagnosed as specific phobia or alternatively as agoraphobia. Cognitive behavioral therapy with exposure in vivo has been shown to be the most effective treatment. The treatment with benzodiazepines is the main priority for the general practitioner and will be subsequently discussed. The treatment of flying phobia and agoraphobia

Emergency Department Medical Clearance of Patients with Psychiatric or Behavioral Emergencies, Part 2: Special Psychiatric Populations and Considerations.

Patients who present to the emergency department (ED) with mental illness or behavioral complaints merit workup for underlying physical conditions that can trigger, mimic, or worsen psychiatric symptoms. However, there are wide variations in quality of care for these individuals. Psychiatry and emergency medicine specialty guidelines support a tailored, customized approach to patients. Our group has long advocated a dynamic comanagement approach for medical clearance in the ED, and this article summarizes best-practice approaches to the medical clearance of patients with psychiatric illness, t

Cognitive factors in the development, maintenance, and treatment of post-traumatic stress disorder.

Cognitive content and processes are central to theories of post-traumatic stress disorder (PTSD). In this paper, we highlight recent findings on cognitive factors in PTSD. Evidence for the role of negative post-traumatic cognitions in PTSD has continued to mount, with cognitions mediating PTSD symptoms and predicting PTSD over and above other key variables. Researchers have also continued to implicate cognitive processes like rumination in the development and maintenance of PTSD. Finally, we review how cognitive-behavioral therapies interrupt dysfunctional cognitive processes in PTSD and highl

Anxiety Disorders in Primary Care.

Investigation for a possible anxiety disorder should be considered in patients with multiple or persistent anxiety symptoms or multiple somatic complaints without a clear somatic etiology. The ideal treatment for anxiety disorders is a combination of pharmacologic and behavioral strategies. As primary care health care evolves, it is expected that the management of mental health disorders (including anxiety disorders) will largely occur in the context of collaborative care models in which patients and primary care clinicians are assisted by trained case managers who help facilitate a more compr

[Pick's disease].

Pick's disease is a type of frontotemporal lobar degeneration(FTLD) with circumscribed atrophy in the frontotemporal lobe. The terminology for Pick's disease has evolved over time. Pick's disease was a term formerly used to define a disorder with symptoms caused by frontal and temporal lobe dysfunction. Therefore, the diagnosis was previously based on clinical features and the distribution of brain atrophy. Pick's disease is currently defined by the presence of tau-positive Pick bodies, and thus can be diagnosed only pathologically. The clinical phenotypes of Pick's disease include behavioral

[Rumination in a young woman].

Rumination refers to bringing up undigested or partially digested food mass from the stomach into the mouth to be rechewed and ingested. This manner of digestion is characteristic of e.g. cattle, but exceptional and likely to cause symptoms for humans. This rare malfunction can be traced on the basis of accurate patient history and confirmed at a test meal by manometry and impedance measurement. Regurgitation may in some patients be triggered by gastroesophageal reflux or air suction into the esophagus caused by an unconscious movement of the pancreas. After the condition has been recognized,

Inflammatory Vulvar Dermatoses.

Inflammatory vulvar dermatoses affect many women, but are likely underdiagnosed due to embarrassment and reluctance to visit a health care provider. Although itch and pain are common presenting symptoms, the physical examination can help distinguish between different disease entities. Because many women's health providers have minimal training in the categorization and management of dermatologic disease, definitive diagnosis and management can be difficult. Herein, strategies for diagnosing vulvar lichen sclerosus, lichen planus, contact dermatitis, lichen simplex chronicus, and psoriasis are

Behavioral and neuroplastic effects of low-frequency rTMS of the unaffected hemisphere in a chronic stroke patient: a concomitant TMS and fMRI study.

Repetitive Transcranial Magnetic Stimulation (rTMS) ameliorates motor and neuropsychological deficits following stroke, but little is known about the underlying neuroplasticity. We investigated neuroplastic changes following 5 days of low-frequency rTMS on the intact motor cortex to promote motor recovery in a chronic patient with subcortical stroke. The feasibility of administering multiple treatments was also assessed 6 months later by applying the same protocol over the patient's parietal cortex to improve visuospatial disorders. Behavioral improvements and no adverse events were observed.

Orofacial pain and temporomandibular disorders: the impact on oral health and quality of life.

Many conditions may cause painful symptoms in orofacial structures. Among the chronic conditions that affect this area, temporomandibular disorders are the most common. Temporomandibular Disorder is a collective term that includes a number of clinical complaints involving the masticatory muscles, the Temporomandibular Joint and associated structures. In some cases, these complaints can be associated with depression, catastrophizing behavior and impact on quality of life. The present study aims to explain the relationship between Temporomandibular Disorders and pain chronification and their rel

Valproate as a treatment for dopamine dysregulation syndrome (DDS) in Parkinson's disease.

It has been previously well established that the use of dopaminergic agents in Parkinson's disease may contribute to behavioral disturbances such as dopamine dysregulation syndrome (DDS), impulse control disorders (ICD), and punding. ICD and punding have been most commonly addressed by reducing dose or by discontinuing the use of a dopamine agonist. Treatment of DDS has proven more challenging, and to date there has been no standard approach. In this paper, we review a series of four patients who met criteria for DDS, who were all refractory to medication adjustments. The DDS symptoms responde

Gaining a comprehensive understanding of pruritus.

Pruritus is a common symptom associated with many dermatoses, systemic abnormalities, and psychiatric / psychosomatic diseases. Additionally, pruritus is one of the most intractable symptoms due to its complex pathogenesis involving an increasing number of mediators and receptors, undefined neurophysiologic pathways, unclear cerebral processing, and psychophysiology interaction. Clinically, the first challenge of dermatologists is how to get general and interdisciplinary vision of pruritus and to preliminarily figure it out whether there might be underlying systemic or psychosocial disorders.

Sickle cell disease and plumbism in children.

Childhood lead poisoning is common, affects multiple organs and is preventable. Co-existence of plumbism and sickle cell disease (SCD) is uncommon, with major overlap in signs and symptoms. We describe a patient with SCD and plumbism. Dysfunctional eating patterns may cause ingestion and increased absorption of lead in these patients. Routine screening for pica and other dysfunctional eating habits with appropriate behavioral interventions is emphasized. Annual screening for lead in patients with SCD of all ages (1-21years) and supplementation of micronutrients like zinc and iron in deficient

Expect psychiatric side effects from corticosteroid use in the elderly.

Psychiatric side effects including mania, depression, psychosis, and delirium, are extremely common in patients treated with corticosteroids. The elderly and those with previous psychiatric diagnoses are not at increased risk for these side effects, but females and those with prior corticosteroid-induced psychiatric side effects are. Depression and mania are the most frequent behavioral side effects, followed by psychosis and delirium. In the geriatric population, there is a long list of medical etiologies of psychosis, delirium, mania, and depression. These must be distinguished from corticos

Overactive bladder: recognition requires vigilance for symptoms.

Overactive bladder (OAB) is a prevalent condition in both men and women that imposes significant burdens on the patient and his or her quality of life. Nevertheless, only a small percentage of patients with OAB receive diagnosis and treatment. The identification of OAB is well within the scope of the primary care provider, as it is symptom-based and does not generally require specialized testing. The treatment of OAB relies on behavioral modification and/or pharmacologic options, primarily antimuscarinic therapy. Better identification of OAB symptoms in the primary care setting should reduce t

Anxiety sensitivity and panic disorder.

Anxiety sensitivity refers to fears of anxiety-related sensations. Most often measured by the Anxiety Sensitivity Index (ASI), anxiety sensitivity is a dispositional variable especially elevated in people with panic disorder. Regardless of diagnosis, ASI scores often predict panic symptoms in response to biological challenges (e.g., carbon dioxide inhalation) that provoke feared bodily sensations. Prospective longitudinal studies indicate that scores on the ASI predict subsequent spontaneous attacks, indicating that elevated anxiety sensitivity is a risk factor for panic and perhaps panic diso

[Behavior therapy and antidepressive drugs. What helps with anxiety?].

Up to 25% of patients seen in the doctor's office are suffering from some sort of anxiety disorder. In accordance with the ICD-10 classification, such disorders include panic disorders with or without agoraphobia, generalized anxiety disorders, social phobia, specific phobia and mixed forms comprising anxiety and depression. Comorbidity with other anxiety disorders and depression is common. Symptoms reported by affected patients may suggest organic disease, which first must be excluded. All patients with severe persistent anxiety states require psychotherapeutic measures (cognitive behavioral

Extraneurologic symptoms as presenting signs of Sanfilippo disease.

Sanfilippo disease, or mucopolysaccharidosis type III, results from the deficiency of lysosomal hydrolases, which impairs heparan sulfate metabolism. Clinically, the disease is characterized by a mild somatic phenotype combined with early severe neurodegenerative illness with prominent behavioral disturbance. We report clinical and molecular findings of a child with Sanfilippo disease type B (alpha-N>-acetylglucosaminidase deficiency) who presented at age 18 months with marked systemic involvement and normal initial psychomotor development. These findings suggest that atypical mucopolysacchari

Bedside urodynamic studies: simple testing for urinary incontinence.

Through proper evaluation and assessment, a large number of patients suffering from urinary incontinence may be effectively treated. Urodynamic studies, which are tests used to assess lower urinary tract function, may be performed using elaborate machinery or simple equipment found in most office settings. Bedside urodynamic studies may be performed without machinery. Patients with symptoms of stress incontinence or urge incontinence are candidates for bedside urodynamic techniques. Using the assessment tool results, pharmacologic therapy and behavioral treatment may be instituted. This articl

Dimensions of response to antidepressant agents in bulimia nervosa: a review.

Many patients suffering from bulimia nervosa benefit from treatment with antidepressant medication. Nurses, like other mental health clinicians, tend to assess the effectiveness of a pharmacologic agent in patients with bulimia nervosa by the decline in the frequency of binge eating episodes. Clinical descriptions and research reports suggest the possibility of additional behavioral dimensions of treatment responsiveness, although the extent to which associated behaviors or symptoms improve is not well documented. This paper examines 16 placebo-controlled pharmacological trials in patients wit

Observations and ratings of tics in school settings.

This paper describes the findings of a school-based tic assessment procedure (direct observations, teacher rating scales) for hyperactive children with comorbid tic disorder. Rates of motor tic frequency were found to be moderately stable across both days and school settings. Correlations between direct observations of tics and clinician rating scales were generally in the low to moderate range as were correlations between teacher and clinician rating scales. Overall rates of hyperactive/disruptive behaviors were not associated with rates of motor tic occurrence, and the behavioral symptoms of…
