Finite element analysis of the three different posterior malleolus fixation strategies in relation to different fracture sizes.

PURPOSE: Appropriate fixation method for the posterior malleolar fractures (PMF) according to the fracture size is still not clear. Aim of this study was to evaluate the outcomes of the different fixation methods used for fixation of PMF by finite element analysis (FEA) and to compare the effect of fixation constructs on the size of the fracture computationally. MATERIALS AND METHODS: Three dimensional model of the tibia was reconstructed from computed tomography (CT) images. PMF of 30%, 40% and 50% fragment sizes were simulated through computational processing. Two antero-posterior (AP) lag

Surgical treatment for fibrous dysplasia of femoral neck with mild but prolonged symptoms: a case series.

BACKGROUND: The proximal femur is one of the most common sites involved by fibrous dysplasia. In cases with mild deformity that does not require corrective surgery, occasional patients suffer sustained pain because of repeated microfractures. This study aimed to clarify the outcomes of surgery with autogenous fibular cortical strut grafting and compression hip screw fixation. METHODS: Since 2002, eight consecutive patients (nine hips) with femoral neck fibrous dysplasia without severe deformity were prospectively treated with autogenous fibular strut grafting and compression hip screw fixatio

Development of a Radiographic Union Score for Determining Osteotomy Union Rates in Long Bones of the Foot.

Determining the status of bone healing after osteotomy can be challenging and has implications ranging from clinical decision-making to standardization of research outcomes without the use of computed tomography. To date, no method has been validated for determining osseous healing of an osteotomy site of the long bones of the foot. The purpose of the present study was to develop a radiographic union scoring system that would enhance the diagnostic healing assessment. We adapted existing orthopedic scales that had been validated for healing in the leg for application in the long bones of the f

Effect of Calcar Screw Use in Surgical Neck Fractures of the Proximal Humerus With Unstable Medial Support: A Biomechanical Study.

OBJECTIVES: To evaluate the effect of calcar screw use in proximal humeral fractures with unstable medial support treated with locked plates. METHODS: Standard osteotomies were performed in 36 cadaveric humeri to create a surgical neck fracture proximal humeral model. For static testing, 12 pairs of humeri were divided into 4 groups: normal alignment and varus deformity groups with and without 5-mm medial deficiencies. Calcar screw function was measured in each group by axial, shear, and torsion stiffness tests. Another 6 pairs of humeri with 5-mm medial deficiencies were subjected to cyclic

Medial locking plate versus screw fixation for fixation of the Ludloff osteotomy.

A prospective cohort study of the Ludloff osteotomy, stabilized with either lag screws or a locking plate, was undertaken from May 2001 to November 2010, involving patients treated for hallux valgus with a first intermetatarsal angle greater than 15°. All patients were assessed using the American Orthopaedic Foot and Ankle Society (AOFAS) and Roles and Maudsley scores before and after surgery. The cohort included 119 procedures in 112 patients, 91 (76.47%) of which were fixated with lag screws and 28 (23.53%) with locking plates. The participants were categorized as active (n = 69 procedures)

Minimal invasive screw fixation and early mobilization of acute scaphoid fractures in the middle third: operative technique and early functional outcome.

Fractures of the scaphoid are relatively common injuries. Differentiation between stable and unstable fractures (Herbert classification) cannot always be made with conventional radiographs and should be additionally evaluated by computed tomographic scan. Under most circumstances, minimal invasive surgery with cannulated screws is currently the treatment of choice. Cast immobilization is not necessary. This article describes the technique of fracture fixation in the middle third of the scaphoid from a palmar approach and early functional outcomes. The outcome assessment included measurement of

The use of resorbable hardware for fixation of pediatric mandible fracture. Case report.

The diagnosis and management of mandible fractures in the pediatric patient population can pose multiple challenges to the oral and maxillofacial surgeon. Resorbable plates and screws for fixation in this population are both well tolerated and effective. They enable realignment and stable positioning of rapidly healing fracture segments, while obviating any potential impediments to long-term metal retention.

Intraoral autogenous block onlay bone grafting for extensive reconstruction of atrophic maxillary alveolar ridges.

BACKGROUND: Endosseous implants require sufficient bone volume for complete bone coverage. Alveolar deficiency can prevent ideal implant placement. Local bone grafts are a convenient source of autogenous bone in alveolar reconstruction. The aim of this study was to describe a technique, and to evaluate the success of extensive bone reconstruction of atrophic maxillary alveolar ridges using only intraoral block bone grafts prior to dental implantation. METHODS: Files of 10 healthy patients with extensive bone reconstruction of the maxillary alveolar ridge using intraoral block bone graft opera

Modern surgical treatment of complex facial fractures: a 6-year review.

Open reduction and internal fixation (ORIF) with refined titanium plates and screws have revolutionized the surgical treatment of complex facial fractures, but its outcome remains relatively unknown. The purpose of this study is to review the results of modern surgical treatment of complex facial fractures involving at least two of the three zones of the face. From 1996 to 2002, 54 consecutive patients (41 males, 13 females; age 5-74 years) with complex facial fractures were treated by the plastic surgery service in our institution. All patients had their fractures repaired within 10 days afte

Results after preoperative traction and pinning in slipped capital femoral epiphysis: K wires versus cannulated screws.

We have retrospectively reviewed 53 cases (62 hips) with a diagnosis of slipped capital wedge epiphysis. After admission to our Hospital, a skin longitudinal traction was applied for 2 weeks, All patients were treated afterward with pinning in situ without manipulation in the operating room. Group A (31 hips) consisted of patients treated with smooth K wires and group B (31 hips) of patients treated with cannulated screws. We found a high incidence of pin penetration in group A (27 hips), whereas there was just one case in group B. Physical closure was considered when 75% of proximal growth pl

Treatment of fractures about hip prostheses with compression plating.

Between 1984 and 1986, 10 patients who sustained a femur fracture about a hip prosthesis were treated with open reduction and internal fixation using compression plating. Nine of the 10 fractures healed in an average time of 5 months. Other than one nonunion, no significant complications were noted. Follow-up study, ranging from 13 to 44 months (average, 26 months), was obtained on all 10 patients. One had a loose femoral component, which was attributed to inadequate stem size. None of the other patients had either clinical or radiographic evidence of loosening. Based on Harris hip scores, the

Results of operative treatment of displaced external rotation-abduction fractures of the ankle.

One hundred and fifty patients with a displaced fracture of the ankle caused by external rotation-abduction forces were treated by open reduction and rigid internal fixation. After an average follow-up of three and one-half years, the results were satisfactory in 90 per cent. Less satisfactory results were noted in the more severely injured ankles. We found that the ruptured deltoid ligament did not need to be repaired if the lateral side was anatomically and rigidly fixed; in the Maisonneuve fracture, restoration of the fibular length was as important as stabilization of the fracture; with th

Outcomes and complications of bipolar fusionless constructs for early-onset scoliosis: a systematic review.

PURPOSE: Early-onset scoliosis (EOS) is a spinal deformity manifesting before age ten, often linked to neuromuscular conditions. Traditional Growing Rod (TGR) techniques, while effective, are associated with high complication rates and require frequent surgeries. Bipolar fixation techniques, which utilize ilio-sacral screws and sliding rods, have emerged as a less invasive alternative, aiming to preserve spinal growth. This systematic review evaluates the safety and efficacy of bipolar fixation in predominantly neuromuscular EOS and mixed pediatric cohorts reported in the current literature, e
