Solitary bone cyst of the cervical spine--case report.

A 63-year-old female presented with an extremely rare solitary bone cyst in the spine involving the C-5 vertebral body. Computed tomography showed an air-density area inside the cyst, without postcontrast enhancement. Magnetic resonance imaging demonstrated a low-intensity lesion on both T1- and T2-weighted images. At surgery, the cyst cavity contained no fluid. Histological examination of the surgical specimen revealed that the cyst wall was lined by a thin fibrous membrane. This rare bone cyst may have originated from a defect in the epiphyseal plate of the vertebral body.

[A case of diffuse idiopathic skeletal hyperostosis (DISH) causing dysphagia].

A case of diffuse idiopathic skeletal hyperostosis causing dysphasia was reported. A 62 year-old man was admitted to our hospital complaining of nuchal pain and dysphagia. Roentgenograms of the cervical spine demonstrated a "flowing" ossification anterior to three contiguous vertebral bodies, C 4-6. Esophagraphic examination demonstrated esophageal compression caused by the enormous ossification. Roentgenograms of the thoracic and lumbar spine revealed an anterior bony bridge and prediscal ossicles at multiple levels of the vertebral column. These radiographic features were characteristic of D

[Dysphagia as a symptom of a ventral cervical prolapsed disk].

Ventral osteophytes, which occur frequently, usually cause no symptoms. They are sometimes identified as the cause of dysphagia. Cervical disc prolapses which cause symptoms of dysphagia are far less common. This paper describes a patient with dysphagia in whom ventral cervical disc prolapse was finally diagnosed by MR tomography, after she had been through a "diagnostic odyssey". The result was subsequently confirmed at surgery. The relevant literature is also reviewed.

[The value of magnetic resonance tomography in diagnosis and follow-up of cervical spine injuries].

We report upon 12 cases of magnetic resonance imaging of cervical spinal cord injuries, in 8 cases done within 24 h after the injury. We found haemorrhages in the spinal cord in 3 cases, compression of the spinal cord in 3, swelling of the spinal cord in 2, and transsection of the spinal cord and hematoma in the epidural space in 1 case each. A normal MR image was seen in a patient with a complete transverse lesion of the spinal cord. The other 4 MR examinations were carried out between 2 weeks and 8 months after injury or operation. In 2 of these trauma victims interbody fusion of the cervica

[Ossification of the posterior longitudinal ligament].

The authors described 21 patients aged 37 to 83 with ossification of the posterior longitudinal ligament (10 with ischemic myelopathy, 6 with aftermaths of a trauma of the cervical spine, and 5 with the radicular syndrome on the basis of vertebral osteochondrosis. X-ray signs of three types of ossification of the posterior longitudinal ligament were defined and specified: linear, curved and mixed. The linear type was more frequently observed in the cervical spine whereas the curved type was more frequent in the lumbar spine.

Traumatic spondylolisthesis of the axis associated with a spinal cord rupture in T1.

Traumatic spondylolisthesis of the axis is rarely associated with neurological deficit. The authors describe a case of complete tetraplegia due to spinal cord rupture at T1 without any other osteoligamentous lesion at the level. The mechanism of this lesion is discussed; it seems to be due to hyperflexion, with preceding hyperextension responsible for the C2 pedicular fracture.

Exclusion of cervical spine injury: a prospective study.

Trauma room lateral cervical spine radiographs (LCSR) may improve the safety of intubation and transportation of multiply injured patients by providing earlier recognition of spinal vertebral injuries. We prospectively evaluated 60 consecutive trauma admissions to determine the impact of clearance of cervical spine radiographs on patient care. Fifty-three patients had no cervical spine injury (CSI). Intubations, emergency head CT scan, aortography, or urgent operation (less than 6 hours after admission) were required in the majority of patients and preceded complete cervical spine clearance in

[Radiographic screening of asymmetry and postural changes in orthodontic patients].

The role of OPM and of the teleradiography in the diagnosis of the asymmetries and of the postural problems of the orthodontic patients, is pointed out from this analysis on the group of 204 patients from mixed dentition to the adult age with different types of malocclusion. The evidence of a significant amount of patients with asymmetries and incorrected posture shows the utility of this radiological screening and gives advises for a therapeutic approach more corrected.

Esophageal perforation following anterior cervical spine surgery.

The authors surveyed the Cervical Spine Research Society to compile a series of esophageal perforations following anterior cervical spine surgery. Twenty-two cases were assembled. Six occurred at the time of surgery, 6 in the postoperative period, and 10 weeks to months later. Eight surgeries were because of fracture. Hardware was implicated in 25% of cases occurring after surgery. Diagnosis was confirmed most often by direct vision at reexploration or esophography. Treatment usually consisted of drainage, repair, and parenteral antibiotics; 2 cases were successfully treated by enteral feeding

Impairment of vertebral artery flow caused by extrinsic lesions.

In a consecutive series of 71 cases of extrinsic lesions involving the vertebral artery (VA), 51 patients presented with external compression of this vessel. The compressive agents included 34 tumors, 4 osteophytes, 5 fibrous bands, 4 traumatic lesions, 2 neural elements, and 2 infectious processes. The main site was the second portion of the VA (C2-C6) (30 of 51 patients). Compression always induced at least significant stenosis, and in 8 patients caused complete occlusion. The compression was either permanent (44 patients) or intermittent (7 patients). Symptoms were observed in 11 patients,

[Traumatic rupture of the vertebral artery associated with fracture of the cervical spine: a case report].

We report here a case of traumatic rupture of the vertebral artery associated with fracture of the cervical spine. A 72-year-old woman was found at the bottom of the stairs by her family. She was immediately transferred to our hospital. She was, in spite of being in a state of shock, alert and oriented with normal respiration. A curvilinear laceration at the right suboccipital region was noticed. On neurological examination, she was diagnosed as having tetraparesis. Roentgenograms confirmed fractures of a vertebral body at C 6 and a transverse process at C 7. Head CT scan demonstrated a small

Airport induced "cervical traction" radiculopathy: the OJ syndrome.

The escalation of airline travel compounded by federal deregulation, fostering crowded airports, delayed flights, and inconvenient gate scheduling, has forced the frequent flyer to run from one gate to another, carrying heavy under-the-seat bags. Eleven busy executive, nine men and two women, developed cervical radicular pain complaints associated with weakness and painful dysesthesias in the involved extremity. Four had right C6 root symptoms and seven had left C7 complaints. All had evidence, by x-ray, of cervical degenerative disc disease and three had abnormal electromyograms. Treatment, c

Bilateral internuclear ophthalmoplegia due to acute cervical hyperextension without head trauma.

A 58-year-old woman developed bilateral internuclear ophthalmoplegia, probable right oculosympathetic paresis (Horner's syndrome), and right facial dysesthesias with acute cervical hyperextension upon sustaining a rear-end automobile collision. There was no head trauma. A nuclear magnetic resonance scan revealed a discrete area of increased signal in the tegmentum of the pons to the left. Extensive recovery was noted 1 year later. The acute cervical hyperextension suggested acute shearing and stretching of axons from brain stem deceleration rather than transient vertebral artery ischemia. Inte

[Injuries of the cervical spine. Review of 70 cases treated over a 5-year period at the "Fondation Jeanne Ebori" of Libreville, Gabon (Central Africa)].

The authors report their experience of 70 cases of cervical spine injuries treated locally, over a 5 years period, 1981 to 1986, at the Fondation Jeanne Ebori Hospital by a multidisciplinary approach. An etiologic survey shows a predominance of male (82.85%) with a peak in incidence between 21 and 30 years (41.43%). Traffic accidents represented the principale cause (74.3%). 29.4% of the lesions involved the upper cervical spine (C1 + C2). 70.6% involved the lower cervical spine (C3 to C7). Clinically, 53% had neurological problems based on entrance examinations of which 49% were complete tetr

Interpreting the spinal X-ray: 1.

It has been assumed that all patients with injury, deformity or pain in the spine should be X-rayed and the films interpreted in the same way regardless of the condition suspected. This is not an effective approach and in part 1 of this article I will discuss the interpretation of the spinal X-ray of the injured patient. The interpretation of spinal X-rays of the patient who complains of pain or deformity will be covered in part 2, to be published in a subsequent issue of the Journal.
