Double Appendix: Implications for the Emergency Department.

BACKGROUND: A double appendix, also known as vermiform appendix duplex, is a rare anomaly in humans, with a reported incidence of 1 in 25,000. The rarity of vermiform appendix duplication makes it a noteworthy medical occurrence. Approximately 100 reported cases have been documented since the first observed case in 1892. There are multiple types of appendiceal duplications, some of which include duplications of other organs. CASE REPORT: A 10-year-old boy was diagnosed with acute appendicitis by clinical examination and ultrasound. He underwent a laparoscopic appendectomy and had an unremarka

Modification of a children's hospital pediatric early warning score (EWS): An evaluation of inter-rater reliability, nurses' critical thinking and perceptions of the tool.

Pediatric early warning scores (EWS) have been utilized to assist the identification of children at risk for clinically decompensating, experiencing a cardiac or respiratory arrest, or requiring a transfer to a higher level of care. Although their use is widespread, little consistency exists between tools and research evaluating the effectiveness of these tools is lacking. This quasi-experimental project evaluated twenty-five medical-surgical staff nurses' use and perceptions as well as the inter-rater reliability of a newly modified pediatric EWS tool at a free standing, academic Midwestern p

Serious diagnoses at revisits in children discharged from the emergency department with back pain.

BACKGROUND: Back pain is often benign but can be a harbinger for serious pathology. Little is known about the outcome in children with back pain but no serious diagnoses detected at the initial visit. We sought to estimate the rate of serious diagnoses at revisits among children initially discharged from the emergency department (ED) with back pain. METHODS: We performed a multicenter retrospective cohort study of patients from 45 pediatric hospitals in the Pediatric Health Information System database from October 1, 2015, to March 31, 2019. We included patients discharged from the ED with a

Decreasing tracheostomy placement in hospitalized pediatric patients with subglottic stenosis - Kids' Inpatient Database (KID) analysis.

OBJECTIVES: We aimed to assess procedural trends for tracheostomy placement in the management of children with subglottic stenosis. We hypothesize that with increased use of less invasive airway interventions for subglottic stenosis, tracheostomy may be performed less frequently over time. METHODS: Data were collected from the Kids' Inpatient Database (KID) for pediatric discharges with an admission diagnosis of subglottic stenosis (Stenosis of larynx: ICD-9 code 478.74) from across the United States for years 2000, 2003, 2006, 2009, and 2012. The number of overall procedures and specific air

Investigation of myocardial protection during pediatric CPB: Practical experience in 100 Chinese hospitals.

Many measures have been proposed for myocardial protection in pediatric congenital heart surgeries, but little data is available for China. This study investigates myocardial protection strategies in pediatric cardiopulmonary bypass (CPB) throughout China. Online questionnaires were delivered to 100 hospitals in 27 provinces. The number of yearly on-pump pediatric cardiovascular surgeries in these hospitals varied greatly. About 91.0% of respondents believe that each surgery should have at least two perfusionists, while only 64.0% of hospitals actually met this requirement. For pediatric patie

Conditions requiring hospitalisations, more than general anaesthesia itself, are associated with diagnosis of learning disorders in children.

BACKGROUND: Anaesthesia is neurotoxic in developing primates. Retrospective clinical studies show a correlation between exposure to anaesthesia during infancy and the occurrence of learning disorders (LD). Prospective studies failed to detect any influence of a single exposure to anaesthesia on neurodevelopment. We hypothesised that some specific populations of children were electively sensitive to anaesthesia-related neurotoxicity. METHODS: Using a case-control design, we analysed the medical histories of children with LD, compared to those of their normally reading siblings. Interviews were

Use of an enterocolitis triage and treatment protocol in children with Hirschsprung disease reduces hospital admissions.

BACKGROUND: While a consensus for the definition of Hirschsprung associated enterocolitis (HAEC) is lacking, the mainstay of treatment includes rectal irrigations with or without antibiotics. This treatment is often effective when initiated as an outpatient. Our institution implemented a triage algorithm in an effort to standardize care thus providing more timely treatment and preventing unnecessary hospital admissions. We sought to review our short-term experience. METHODS: A retrospective review was performed of all Hirschsprung (HD) patients <6 years old over two distinct time periods from

The start-up phase of a non-specialist paediatric surgical service by outreach focused on capacity building at the Consolata Hospital Ikonda, Makete District, Tanzania.

UNLABELLED: age) are far lower than the recommended workforce size. Complex conditions require a significant increase in the number of paediatric surgeons. In contrast, children with minor diseases, living in rural areas, could be managed even at the district level by trained general surgeons. AIM: The aim of this study was to develop capacity for general paediatric surgical services in a district hospital by outreach with a focus on mentorship. METHODS: Capacity building priorities for non-specialist paediatric surgery were identified and addressed using evidence-based guidelines. Local gen

Success and Complications of the Ketamine-Only Intubation Method in the Emergency Department.

BACKGROUND: Rapid sequence intubation (RSI), defined as near-simultaneous administration of a sedative and neuromuscular blocking agent, is the most common and successful method of tracheal intubation in the emergency department. However, RSI is sometimes avoided when the physician believes there is a risk of a can't intubate/can't oxygenate scenario or critical hypoxemia because of distorted anatomy or apnea intolerance. Traditionally, topical anesthesia alone or in combination with low-dose sedation are used when physicians deem RSI too risky. Recently, a ketamine-only strategy has been sugg

Pain management in children has significantly improved in the Italian emergency departments.

AIM: Pain in children is often poorly assessed and treated in Italian emergency departments (EDs) as found in a survey conducted among the centres of the "Pain in Pediatric Emergency Room (PIPER)" Study Group in 2010. Our aim was to evaluate the changes in pain management in Italian EDs in the last years. METHOD: A structured questionnaire about pain assessment, protocols, use of local anaesthetics before venipuncture, opioids and adjuvants was mailed to 46 Italian EDs between November 2017 and April 2018. RESULTS: Pain was always assessed at triage in 34 centres (73.9%). Algometric scales w

Intracranial abscesses in children at Ile-Ife, Nigeria: a case series and review of literature.

PURPOSE: Central nervous system (CNS) infections are significant causes of morbidities and mortalities in children with some being prone to the development of abscesses which can either be within the brain parenchyma or located in extracranial structures. We aimed to describe the clinical profile and outcome of children with cranial abscesses at the Obafemi Awolowo University Teaching Hospital Complex (OAUTHC). METHODS: Consecutive cases presenting at the Children Emergency Ward of OAUTHC Ile-Ife were recruited. The pattern of presentation, predisposing factors and outcome of these children w

The Cost of Treating Caries-Related Complaints at a Children's Hospital Emergency Department.

OBJECTIVES: To determine the number and diagnoses of caries-related complaints presenting at Canada's largest children's hospital emergency department (ED) and the costs associated with treatment over 5 years. METHODS: We carried out a retrospective review of the health records of all children who presented to The Hospital for Sick Children, Toronto, with caries-related emergency complaints from 1 January 2008 to 31 December 2012. A caries-related complaint was defined as a chief complaint of pain or swelling resulting from the sequelae of dental decay (reversible pulpitis, irreversible pulpi

Laundry detergent pod ingestions: is there a need for endoscopy?

INTRODUCTION: Laundry detergent pod (LDP) exposures in children have resulted in several referrals to the emergency department. Signs and symptoms can include gastrointestinal symptoms (vomiting, drooling), neurological symptoms (depressed sensorium), or metabolic changes (lactic acidosis). There is limited literature on esophageal injury following LDP ingestions. CASE SERIES: We reviewed three cases of pediatric LDP ingestions that underwent an upper endoscopy in a tertiary care pediatric hospital. All of our patients were younger than 3 years old. The upper endoscopies revealed superficial
