Changes in blood pressure, medication adherence, and cardiovascular-related health care use associated with the 2018 angiotensin receptor blocker recalls and drug shortages among patients with hypertension.

BACKGROUND: One of the largest-ever retail drug shortages began in 2018 when several angiotensin II receptor blockers (ARBs) for treating hypertension, heart failure, and chronic kidney disease-valsartan, losartan, and irbesartan-were recalled for carcinogenic impurities. The long-term consequences of the ARB shortages and whether certain groups experienced more adverse outcomes is unknown. OBJECTIVE: To evaluate changes in adherence and health outcomes after ARB recalls and to identify patients who experienced greater changes in access and adverse clinical outcomes. METHODS: Using an integr

Effects of different negative pressure wound therapy modes on wound healing: a systematic review.

INTRODUCTION: Negative pressure wound therapy (NPWT) has been widely used in wound care, yet the optimal application mode-continuous, intermittent, or dynamic-remains unclear. This systematic review aimed to compare the effects of different NPWT modes on wound-healing-related outcomes and to provide evidence for mode selection in clinical practice. METHODS: Following PRISMA guidelines, a systematic search was conducted across 11 databases-PubMed, MEDLINE, EMBASE, ProQuest, CINAHL, Web of Science, CENTRAL, CNKI, CBM, VIP, and WANFANG-from 1997 to 2024. RESULTS: Ten clinical studies and 15 ani

Renal function and cardiovascular events in predialysis care.

Cardiovascular events (CVE) are highly prevalent in chronic kidney disease (CKD), with risk increasing as CKD progresses. Whether renal function remains associated with CVE in advanced CKD is uncertain. We evaluated the association between CVE and renal function in a prospective observational cohort of predialysis patients (GFR < 30 ml/min). Renal function was measured using plasma-clearance of iohexol (mGFR) and estimated (eGFR) by four equations (CKD-EPI creatinine, MDRD, CKD-EPI cystatin, CKD-EPI-Creatinine-cystatin). Associations were analysed with logistic regression models adjusted for t

Accelerated Muscle Strength Decline and Reduced Lean Mass in Adults with Treated Inflammatory Bowel Disease: A NHANES Study.

BACKGROUND: Muscle loss is associated with chronic systemic inflammation and metabolic stress in the context of inflammatory bowel disease (IBD). However, studies that assessed the population-level effects of treated IBD on muscle health are limited. METHODS: In this study, pooled National Health and Nutrition Examination Survey 2011-2014 data of adults aged 20-59 years were analyzed through dual-energy X-ray absorptiometry and grip strength measurements. The treated IBD status was determined using prescription medication records, prioritizing specificity to capture individuals with clinicall

Development of a Modern Standard Arabic version of the pain disability index: translation, cross-cultural adaptation, psychometric, and validity data.

The assessment and treatment of chronic pain rely heavily on patient self-report, making linguistically and culturally appropriate tools essential. However, no well-validated Arabic language measures of pain-related disability are widely available. The objective of this study was to create and validate a Modern Standard Arabic (MSA) version of the Pain Disability Index (PDI). This prospective cross-sectional study was conducted in a pain management clinic in a tertiary care center in the United Arab Emirates (UAE). The MSA PDI was developed using a forward-backward translation protocol by a te

From breath to brain: influenza vaccination as a pragmatic strategy for dementia prevention.

Aging populations require scalable strategies to delay or prevent dementia. Beyond the prevention of neurological injury associated with seasonal influenza, vaccination may help mitigate vascular and neuroinflammatory injury underlying cognitive impairment. Influenza infection can cause a marked short‑term increase in myocardial infarction risk, and acute infections have also been associated with transient increases in stroke risk. Experimental models show prolonged microglial activation and synaptic loss even from non-neurotropic strains - processes likely modulated by vaccination. Epidemiolo

Evaluating working memory functioning in individuals with myalgic encephalomyelitis/chronic fatigue syndrome: a systematic review and meta-analysis.

Individuals with myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) frequently report pronounced cognitive difficulties, yet the empirical literature has not fully characterised how discrete components of working memory are affected. Given that working memory serves as a foundational system supporting complex cognitive processes, differentiating performance across verbal and visual modalities provides critical insight into which higher-order functions may be most vulnerable. This systematic review/meta-analysis aimed to synthesise current research to investigate how ME/CFS impacts wor

Review: Systemic inflammation after stroke. Therapy and perspective.

Systemic inflammation following ischemic stroke is driven by a complex interplay among pro-inflammatory cytokines, immune cell activation, and neurovascular dysfunction. Both aging and obesity significantly amplify this inflammatory response, exacerbating stroke severity and impeding recovery. Aging induces a chronic low-grade inflammatory state-referred to as inflammaging-that heightens vulnerability to stroke-induced brain injury. Similarly, obesity promotes a persistent pro-inflammatory milieu that disrupts metabolic and immune homeostasis, further worsening neurological outcomes. The combi

Brain imaging as a prognostic biomarker in urea cycle disorders.

The Urea cycle disorders (UCDs) represent a group of rare inborn errors of metabolism that have impaired nitrogen handling from the breakdown of protein, with ensuing acute episodic or chronic hyperammonemia and neurotoxicity. Although advances in newborn screening, molecular diagnosis, and therapeutic interventions have improved survival, neurocognitive, behavioral, and psychiatric sequelae remain prevalent in those with both neonatal onset as well as later onset partial deficiencies. The mechanisms of brain injury in UCDs are complex and multifactorial, and in some cases are not fully known.

A device-invariant multi-modal learning framework for respiratory disease classification.

Recent advances in cough sound analysis using deep learning techniques enable smartphone-based respiratory disease screening suitable for self-management care in a home setting, yet their utility is limited by device heterogeneity, population diversity, and challenges in multimodal integration. We propose a device-invariant, multimodal deep learning framework that jointly models cough acoustics, demographic data, and symptom descriptions for multi-label classification of adult respiratory diseases. To address the issues of device effect, an adversarial branch is embedded in the audio encoder t

Health-Related Social Needs in Children With Sickle Cell Disease Are Associated With Worse Health-Related Quality of Life.

BACKGROUND: Children with sickle cell disease (SCD) face multiple acute and chronic medical complications that may impact their quality of life as reported by patients themselves. Health-related social needs (HRSNs), such as food and housing insecurity, are common in people with SCD, but the association between HRSNs and patient-reported quality-of-life outcomes in pediatric SCD patients has not been examined. METHODS: In this cross-sectional study, parents/guardians of children 5-18 completed a demographic questionnaire, an HRSN screener, and the PROMIS Pediatric Parent-Proxy 25 Profile v2.0

Hydroxyurea treatment for sickle cell disease in Nairobi informal settlements: a retrospective cohort study.

BACKGROUND: Sickle Cell Disease (SCD) causes acute and chronic complications. Hydroxyurea (HU) reduces vaso-occlusive crises (VOCs) and improves patients' outcomes. However, evidence on the real-world implementation and effectiveness of HU in resource-limited primary care settings, particularly those serving informal settlements, remains scarce. This study evaluates HU use and its effects in two Nairobi clinics serving informal settlements. METHODS: We retrospectively analyzed 2,206 clinical visits from 328 SCD (HbSS) patients (March 2019-March 2021), comparing complication rates in HU users

Up to date comprehensive review of atraumatic shoulder instability: anatomy, causes, management, and psychosocial considerations.

Atraumatic shoulder instability (ASI) poses a significant challenge in orthopedic practice, characterized by functional and anatomical deficits in the glenohumeral joint (GHJ) resulting from chronic overuse rather than acute trauma. This review aims to provide a comprehensive summary of the current understanding of ASI, covering its anatomy, etiology, the influence of psychosocial factors and primarily focusing on management strategies. The management of ASI is a central focus for restoring normal biomechanical and anatomical function of the injured upper extremity. A literature review was con

Autoimmune disease-associated lymphomas: research progress and review.

Autoimmune diseases (ADs) are strongly associated with a significantly increased risk of lymphoma, with the standardised incidence ratio (SIR) markedly elevated in certain conditions, most notably in Sjögren's disease (SjD) where an SIR as high as 18.8 has been reported. The risk is particularly prominent for diffuse large B-cell lymphoma (DLBCL) and mucosa-associated lymphoid tissue (MALT) lymphoma. This review systematically elucidates the epidemiological features, pathological mechanisms, risk factors, and therapeutic strategies of ADs-associated lymphomas. Epidemiological studies have conf
