FPSIR predicts clinical therapeutic responses and survival outcomes in patients with metastatic colorectal cancer undergoing first-line bevacizumab-containing chemotherapy.

BACKGROUND: Identifying patients who are most likely to benefit from the combination of bevacizumab and chemotherapy (Bev/CT) is essential for the optimal management of metastatic colorectal cancer (mCRC). The aim of this study is to investigate the utility of chronic inflammatory biomarkers in predicting clinical response to Bev/CT and outcomes in patients with mCRC. MATERIALS AND METHODS: This study enrolled 364 patients with mCRC undergoing first-line Bev/CT therapy. The patients were randomly assigned to discovery (n=249) and validation (n=115) cohorts, maintaining an approximate 2:1 rati

Resveratrol Modulates Phosphatidylcholine Metabolism-Related Enzymes and Enhances Chemosensitivity in Colorectal Cancer Models.

Colorectal cancer (CRC) remains a major cause of cancer-related mortality worldwide, largely due to the emergence of chemoresistance driven by metabolic adaptations. Among these alterations, lipid metabolic reprogramming has emerged as a critical determinant of tumor progression and therapeutic response. In particular, dysregulation of enzymes involved in phosphatidylcholine (PC) synthesis and remodeling, through the Kennedy pathway and the Lands cycle, has been associated with therapeutic resistance. Lysophosphatidylcholine acyltransferase 2 (LPCAT2), a key enzyme involved in PC remodeling, n

Gut microbiome-mediated primary and acquired resistance to immune checkpoint inhibitors in MSI-H/dMMR colorectal cancer: mechanisms, biomarkers, and therapeutic implications-a narrative review.

INTRODUCTION: Immune checkpoint inhibitors (ICIs) have transformed the management of microsatellite instability-high/deficient mismatch repair (MSI-H/dMMR) colorectal cancer (CRC). However, a substantial proportion of patients exhibit primary resistance or eventually develop acquired resistance, highlighting the need for a better understanding of the biological mechanisms influencing therapeutic response. Increasing evidence suggests that the gut microbiome-immune axis is an important regulator of antitumor immunity through complex interactions among microbial communities, microbial metabolite

A first-in-human, phase I/IIa trial of the T cell immunotherapy personalised tumour trained lymphocytes in patients with advanced colorectal cancer: Study protocol.

BACKGROUND: Adoptive T cell therapy is a promising alternative to conventional therapies for certain solid tumours. Personalized tumour-trained lymphocytes (pTTL) is an autologous T cell therapy derived from tumour-draining regional lymph nodes (RLNs), trained to target patient-specific neoantigens arising from tumour-specific mutations. The protocol for an ongoing phase I/IIa first-in-human trial of pTTL in Stage IV Colorectal Cancer (CRC), NEOGAP-CRC-01, is presented here. METHODS: pTTL is produced by in vitro expansion of T cells from RLNs using EpiTCer® technology. Tumour-specific neoanti

Systemic-local immune remodeling in colorectal cancer: CD14+ cell-derived cytokine responsiveness, mucosal immune-cell organization, and exploratory metastasis-associated patterns.

BACKGROUND: Immune regulatory networks contribute to colorectal cancer (CRC) progression, but most clinical studies focus on tissue immune-cell abundance or static inflammatory mediators. We investigated cytokine-response patterns in cultures derived from peripheral-blood CD14+ cells and local immune-cell composition in tumor-adjacent and resection-margin mucosa. METHODS: This single-center prospective observational study included 106 patients with colorectal adenocarcinoma and 20 comparison subjects with non-malignant surgical conditions. Peripheral-blood CD14+ cells were isolated, different

Comparison of Predictive Performance of Three Lymph Node Staging Systems in Colorectal Signet Ring Cell Carcinoma Based on Machine Learning Model.

Lymph node status is a critical prognostic predictor for patients; however, the prognosis of colorectal signet-ring cell carcinoma (SRCC) has garnered limited attention. This study investigates the prognostic predictive capacity of the log odds of positive lymph nodes (LODDS), lymph node ratio (LNR), and pN staging in SRCC patients using machine learning models (Random Forest, XGBoost, and Neural Network) alongside competing risk models. Relevant data were extracted from the Surveillance, Epidemiology, and End Results (SEER) database. For the machine learning models, prognostic factors for can

Microplastics' Impact on the Development of AOM/DSS-Induced Colitis-Associated Colorectal Cancer in Mice.

Recently, evidence indicating that microplastics (MPs) have a hazardous effect on human health is accumulating. The potential of MPs having a role in carcinogenesis was suggested. Therefore, the aim of this study was to evaluate MPs' effect on colitis-associated colorectal cancer (CAC) development. The AOM/DSS-induced CAC model was reproduced in two groups of adult male C56BL/6 mice. One of these groups received MPs (5 μm polystyrene microbeads) with drinking water at a dose of 1.48 mg/kg/day throughout the experiment (12 weeks), and the other received water untreated with MPs. Histopathologic regression in lymph nodes predicts occult metastasis in ypT0 colorectal cancers after neoadjuvant therapy: a retrospective cohort study.

In locally advanced colorectal cancer (LACRC), neoadjuvant therapy (NT) followed by total mesorectal or complete mesocolic excision is standard. Although a pathological complete response at the primary site (ypT0) predicts excellent survival, this benefit is offset when residual nodal metastasis (ypT0N+) is present. Reliable predictors of ypT0N+ are lacking. We retrospectively reviewed LACRC patients (2009-2024) who received NT and achieved ypT0 after curative surgery. Clinicopathologic variables were analyzed by uni- and multivariable logistic regression. A lymph node (LN) showing fibrosis, n

Evaluation of prognostic factors in patients undergoing surgery for colorectal cancer lung metastases.

OBJECTIVES: Lungs are the second most frequent site of colorectal cancer metastases. Complete resection of isolated lung metastases is recommended in the current guidelines because of the high long-term survival rate and low morbidity-mortality. The aim of this study is to determine prognostic factors for lung metastasectomy in order to better select patients who will benefit from surgical resection. METHODS: This single-center retrospective study is based on the medical data of patients operated for resection of colorectal lung metastases between 2004 and 2023. RESULTS: A total of 109 patie

Dual-Nanocomplex Delivery of Neoantigen Vaccines and Doxorubicin for Synergistic Chemo-Immunotherapy against Colorectal Cancer.

Colorectal cancer (CRC) is still an important global health challenge, with limited treatment efficacy. Neoantigen-based cancer vaccines offer tumor-specific immune activation but are limited by poor immunogenicity and rapid degradation, while chemotherapeutics like doxorubicin (DOX) suffer from non-selective toxicity and resistance. To overcome these challenges, a dual-nanocomplex delivery platform is developed that integrates immunotherapy and chemotherapy. The first nanocomplex, HCNPs, is functionalized with the neoantigen peptide Adpgk, CpG oligodeoxynucleotides, and hyaluronic acid (HA) v

Multi-pathway therapeutics in colorectal cancer: targeting EMT, CSCs, and non-apoptotic cell death for drug resistance reversal.

Colorectal cancer (CRC) is a leading global malignancy, with therapeutic outcomes hampered by chemoresistance from epithelial-mesenchymal transition (EMT), colorectal cancer stem cells (CR-CSCs) and apoptosis evasion. Evidence highlights the therapeutic value of non-apoptotic cell death pathways such as paraptosis, ferroptosis, cuproptosis and autophagy as strategies to overcome resistance. This review evaluates natural and synthetic scaffolds targeting EMT-driven chemoresistance in CRC by inducing regulated cell death pathways and disrupting stemness-associated survival mechanisms. Rational Design of Bioinspired Lipoprotein System to Improve Penetration in Colorectal Peritoneal Metastases.

Chemotherapy of lethal colorectal peritoneal metastases (PM) is notoriously challenged by poor drug delivery efficiency in PM tumors. Inspired by the histopathological examinations of PM tumors from colon cancer patients, a C[RGDfK] peptide-modified bioinspired lipoprotein (R-BLP) system was optimized from 8 formulations with profound penetrating ability in PM tumors of colorectal cancers. Then, a chemotherapeutic 7-ethyl-10-hydroxy-camptothecin (SN38)-loaded R-BLP (termed SR-BLP) was designed to promote their penetration in PM tumors and improve the chemotherapeutic efficacy. In CT26-induced

No Effect of Computer-Aided Diagnosis on Colonoscopic Adenoma Detection in a Large Pragmatic Multicenter Randomized Study.

INTRODUCTION: Multiple randomized studies have demonstrated that computer-assisted detection (CADe) improves the colonoscopic adenoma detection rate (ADR); however, these improvements have not been consistently reproduced in some more recent studies. METHODS: Patients aged 50 years and older undergoing outpatient colonoscopy in 12 German institutions were randomized to either the CADe group or the control group. The primary outcome parameter was the ADR, while secondary outcomes included the adenomas per colonoscopy rate, detection rates of different histologic subgroups, and adverse events.

Machine learning in colorectal polyp surveillance: A paradigm shift in post-endoscopic mucosal resection follow-up.

Colorectal cancer remains a major health concern, with colorectal polyps as key precursors. Endoscopic mucosal resection (EMR) is a common treatment, but recurrence rates remain high. Traditional surveillance strategies rely on polyp characteristics and completeness of the resection potentially missing key risk factors. Machine learning (ML) offers a transformative approach by integrating patient-specific data to refine risk stratification. Recent studies highlight ML models, such as Extreme Gradient Boosting, which outperform conventional methods in predicting polyp recurrence within one-year
