Experience with the surgical treatment of 265 patients with a compressive stenosis of the celiac trunk has been analyzed. Stenosis of the celiac trunk is known to be a cause of ischemic abnormalities of the stomach, duodenum, pancreas. Decompression of the celiac trunk is an effective method of treatment of the ischemic disease of digestion organs resulting in better organic blood circulation and involution of the appearing pathologic processes. In a number of patients there appeared symptoms for associated interventions due to complications of ischemic gastroduodenal ulcers or concomitant dis

Seventy-three patients with paralytic ileus hospitalized at the Surgical Clinic of the Hospital in Herford during the years 1971-1975 are reviewed. All the patients were treated with a small bowel fistula according to a modified technique of Witzel-Heidenhain. The special indication, the technical details, and the results are discussed. The overall mortality of a total of 73 patients with paralytic ileus undergoing a small bowel fistula operation was 19%. The temporary small bowel fistula according to the modified technique of Witzel-Heidenhain is proposed as a simple, safe, and with accurate

Exploration of the middle ear was performed on 15 patients with sudden deafness that had failed to improve after intravenous infusion of Eupaverin and Rheomacrodex. Three patients had abnormalities of the round window membrane, in 2 there was a perilymphatic fistula and in 1 an abnormal bulging of the membrane with no transmission of perilymphatic fluid movement. These 3 patients presented with a history of exertion or stress prior to the onset of symptoms. Surgical closure ofthe 2 fistulae led to improvement of hearing, and perilymphatic decompression in the third patient improved the tinnitu

OBJECTIVE: Meralgia paresthetica (MP) is a neuropathy of the lateral femoral cutaneous nerve (LFCN) manifested by burning pain, allodynia, and sensory loss in the anterolateral thigh. While simple decompression is commonly performed in patients who do not respond to nonoperative measures, LFCN transposition is an alternative technique for which rates of symptomatic relief and surgical complications are presently poorly understood. The authors report the largest prospective case series of outcomes from a series of consecutive patients who underwent LFCN transposition for refractory MP.

Background Chiari malformation type II (CM-II) may not always present as an asymptomatic disorder but prove to be difficult in managing. This is especially true for neonates who show the worst prognosis. There is confounding data over whether shunting or craniocervical junction (CVJ) decompression should be employed. This retrospective analysis summarizes the results of 100 patients diagnosed and treated for CM-II along with hydrocephalus and myelomeningocele. Methods We reviewed all the children who were diagnosed and surgically treated for CM-II at the Moscow Regional Hospital. Surgical tim

BACKGROUND: Due to their location, large and giant intracavernous and paraclinoid aneurysms remain a challenge for vascular neurosurgeons. We identified characteristics, surgical indications and treatment strategies of large and giant intracavernous and paraclinoid aneurysms in 36 patients.
METHODS: The pterional approach was routinely used. The cervical internal carotid artery was exposed for proximal control of parent vessel and retrograde suction decompression. Paraclinoid aneurysms were directly clipped, intracavernous pseudoaneurysm was repaired and the intracavernous aneurysms were trap

STUDY DESIGN: A retrospective investigation of the results of operative treatment of patients with symptomatic thoracic spinal stenosis.
OBJECTIVES: To establish the effectiveness and define the limitations of surgical treatment for stenosis of the thoracic spinal canal.
SUMMARY OF BACKGROUND DATA: In contrast to cervical and lumbar stenosis, symptomatic narrowing of the thoracic spinal canal is rarely encountered. Although the treatment of thoracic stenosis has been described in multiple case reports and in several small series with minimal follow-up evaluation, there are few studies of pat

INTRODUCTION: Gluteal compartment syndrome (GCS) is a rare, often overlooked emergency caused by rising compartment pressures leading to muscle and nerve ischemia, frequently after substance-related immobilization. Nonspecific symptoms such as diffuse pain and reduced mobility contribute to delayed diagnosis, increasing risk of irreversible neuromuscular injury and death.
CASE SUMMARY: Two cases of GCS following polysubstance overdose are presented.•Case 1: A 44-year-old male developed right-sided GCS with extensive gluteal muscle necrosis following delayed diagnosis, initially misattributed

OBJECTIVE: Complex cranial wound conditions (CCWC), particularly when associated with hydrocephalus and/or implant-related infections, pose a major challenge in neurosurgical complication management. In such cases, moving the shunt system from the cranial to the lumbar compartment appears to be a valuable salvage strategy.
RESEARCH QUESTION: Can lumboperitoneal shunting (LPS) including differential pressure and gravitational valves serve as an effective rescue strategy in patients with different types of CCWC?
METHODS: We conducted a single-center retrospective study of 15 patients treated w

OBJECTIVE: Chiari malformation type I (CMI) is a common neurologic condition and surgery is the standard treatment. We aim to establish the cerebrospinal fluid (CSF) flow change as a potential objective indicator of surgical efficacy to improve symptoms.
METHODS: We performed a retrospective 2-center analysis of surgically treated patients with CMI. Qualitative CSF flow studies obtained preoperatively and postoperatively were analyzed by the neuroradiologist, seeking improvement in CSF flow. Symptom status, including headache and neck pain, were recorded.
RESULTS: Forty-nine patients were id

OBJECTIVEThe authors report their current experience with their previously published novel form of treatment in 70 cases of lumbar canal stenosis. The treatment consisted of only fixation of the spinal segments by the transarticular screw fixation technique. No bone, ligament, osteophyte, or disc resection was done for spinal canal and neural foraminal decompression. The proposed treatment is based on the concept that vertical instability that results in telescoping of the facets on physical activity forms the nodal point of pathogenesis of lumbar canal stenosis.METHODSDuring the period June 2

BACKGROUND: Secondary trigeminal neuralgia (TN) is uncommon. When a space-occupying lesion with mass effect is identified, the associated TN is often exclusively attributed to the tumor. This report illustrates the importance of considering coexistent actionable pathology when surgically treating secondary TN.
CASE DESCRIPTION: A 51-year-old woman presented with abrupt-onset TN of the V2 and V3 nerve divisions with hypesthesia. She denied changes in hearing, balance, or facial nerve dysfunction. Magnetic resonance imaging revealed a 1.6-cm contrast-enhancing cerebellopontine angle tumor that

Surgical treatment of moderate hallux rigidus remains controversial and the optimal surgical technique has yet to be defined. Decompressive metatarsal osteotomy is one of the procedures available; however, one of the potential drawbacks is the effect of the metatarsal shortening. We evaluated the global effect of the decompressive metatarsal osteotomy, accounting for the metatarsal index. We retrospectively evaluated 78 patients with stage II and III hallux rigidus who had undergone Youngswick osteotomy and analyzed their outcomes according to the metatarsal index. The candidates for inclusion

OBJECTIVE: For patients with medically unresponsive trigeminal neuralgia (TIC) and hemifacial spasm (HS), surgical microvascular decompression (MVD) is the procedure of choice. The authors of this report sought to review their outcomes with MVD in patients with TIC and HS, and the success of preoperative magnetic resonance imaging (MRI) in identifying the offending vascular compression.
METHODS: Since 2004, there were a total of 51 patients with TIC and 12 with HS with available MRI scans. All patients underwent preoperative MRI to rule out non-surgical etiologies for facial pain and facial s

BACKGROUND: Subperiosteal inflammatory disease, subperiosteal abscess and orbital cellulitis are the most common sequelae of both acute and chronic sinusitis. Early, appropriate evaluation and management observing signs and symptoms of orbital inflammation are required to prevent blindness. Computerised tomography (CT) is the investigation of choice in diagnosing the discussed pathological conditions. Cellulitis does not require drainage of the orbit on a routine basis. Conventionally, the orbital abscess is drained via an external incision and sinus surgery is performed at the same time to re
