BACKGROUND: Tympanometry and pneumatic otoscopy are recommended for diagnosis of otitis media, but are not frequently used by general practitioners (GPs). We examined how, after targeted short training, GP diagnosis and management of childhood ear disease was changed by the addition of these techniques to non-pneumatic otoscopy. For decades the commonly held belief that local anaesthesia (LA) with vasoconstrictor can cause necrosis in end-arteries has been sustained by medical teachers, text books and clinical practice. This literature review, identifying 24 articles investigating the use of LA with epinephrine in fingers, toes, noses and ears, including more than 15,000 patients, finds no evidence to support this belief. No case of necrosis has been reported since the introduction of commercial lidocaine with epinephrine in 1948. This review concludes that it is safe to use lidocaine with adrenaline in end-arteries.

PURPOSE: This tutorial demonstrates the effects of tubing length and coupling type (i.e., foam tip or personal earmold) on hearing threshold and real-ear-to-coupler difference (RECD) measures.
METHOD: Hearing thresholds from 0.25 kHz through 8 kHz are reported at various tubing lengths for 28 normal-hearing adults between the ages of 22 and 31 years. Osteoprotegerin (OPG) is a key regulator of bone remodeling. Mutations and variations in the OPG gene cause many human diseases that are characterized by not only skeletal abnormalities but also poorly understood hearing loss: Paget's disease, osteoporosis, and celiac disease. To gain insight into the mechanisms of hearing loss in OPG deficiency, we studied OPG knockout (Opg(-/-)) mice.
