Renal excretory profiles of loop diuretics: consequences for therapeutic application.

In healthy subjects, 24-h natriuresis, kaliuresis, calciuresis, and magnesiuresis increase in response to the first oral dose of a standard … ied before standard (diuretic) formulations of diuretics are used in the treatment of uncomplicated essential hypertension. When loop diuretics are employed in the treatment of congestive heart failure, the minimal dose compatible with the attainment of clinical objectives should be used.

Reversal of diuretic-induced secondary hyperaldosteronism and hypokalemia by trilostane, an inhibitor of adrenal steroidogenesis.

Correction of diuretic-induced hypokalemia is usually accomplished by potassium supplementation or antagonism of aldosterone's renal action. This study sought to determine if inhibition of aldosterone biosynthesis could reverse diuretic-induced hypokalemia and whether trilostane would be clinically useful in this regard. Essential hypertensives (n = 22) were treated with hydrochlorothiazide (HCTZ) 50 mg/d, and patients who became hypokalemic were randomly assigned to receive in addition to HCTZ either a placebo (n = 7), trilostane 240 mg/d (Trilo 240) (n = 7), or trilostane 60 mg/d (Trilo 60)

Antihypertensive efficacy of ketanserin alone or in combination with a beta-blocker or a diuretic: the Swiss Ketanserin Study.

In the Swiss Ketanserin Study the antihypertensive efficacy and tolerability of ketanserin (given in 20 or 40 mg doses twice daily) was investigated, after a placebo run-in phase, as monotherapy (n = 68) as well as in combination with either atenolol (100 mg/day) (n = 30) or the potassium-sparing diuretic hydrochlorothiazide (50 mg/day) and amiloride (5 mg/day) (n = 26) in 124 patients with essential hypertension, aged 41 to 82 years. With the addition of ketanserin, diastolic blood pressure fell by 8 +/- 8, 8 +/- 8, and 7 +/- 9 (+/- SD) mm Hg, respectively (p less than 0.05 for all) in the th…

Pregnancy in renal allograft recipients: prognosis and management.

Renal transplantation is invariably accompanied by improvements in reproductive function. The possibility of conception in women of childbearing age emphasizes the need for compassionate and comprehensive counselling. Couples who want a child should be encouraged to discuss all the implications. Therapeutic abortion is undertaken in 22% of conceptions and the spontaneous abortion rate is about 16%, the same as for the normal population. Of the conceptions that continue beyond the first trimester, over 90% end successfully. In most women, renal function is augmented during pregnancy, but perman

Alpha-adrenoceptor blockade by labetalol during long-term dosing.

The hypotensive effect of short-term labetalol, the alpha- and beta-adrenoceptor blocker, is greater in subjects in the orthostatic position, possibly because of the alpha-adrenoceptor blockade. During prolonged use the orthostatic blood pressure fall disappears. To verify whether or not this is due to reduction in alpha-blocking activity, phenylephrine-induced increase in blood pressure was studied in six subjects with mild essential hypertension before and after 3 and 6 days and 1 and 6 mo of continuous treatment with 200 mg labetalol three times a day by mouth. At the same intervals, isopro…

Regulation of myosin light chain phosphatase and myofilament Ca2+ sensitivity in smooth muscle: a tale of research on smooth muscle contraction.

Contraction of both striated and smooth muscles is regulated by intracellular Ca2+ signals. The mechanisms of contraction following the Ca2+ signal are significantly different between striated and smooth muscles. The Ca2+-dependent reversible phosphorylation of the 20 kDa myosin light chain plays an essential role in regulating smooth muscle contraction. The phosphorylation theory of smooth muscle contraction was proposed in the mid-1970s. In the late 1980s, quantitative evaluation of cytosolic Ca2+ concentrations and contraction revealed that the contraction was not always associated with cha

The roles of folate, MTHFR genetics, vitamin B12 in pregnancy outcomes.

Folate is essential for fetal development, and periconceptional folic acid (FA) supplementation is well-established for preventing neural tube defects. However, evidence regarding its role in other pregnancy outcomes, such as gestational diabetes mellitus, hypertensive disorders of pregnancy, fetal growth, miscarriage, and preterm birth, remains inconsistent. Current knowledge indicates that the effects of FA are not uniform but significantly influenced by the timing, dose, and duration of supplementation, frequently exhibiting U-shaped or timing-dependent relationships. Furthermore, methylene

Glutathione/Selenite Interaction Products as Potent Vascular Smooth Muscle Tone Modulators with Antihypertensive Effects in Spontaneously Hypertensive Rats.

Selenium is an essential trace element important for human health and its deficiency is typically compensated through dietary supplementation, including selenite (SeO32-). In biological systems, SeO32- undergoes multistep reduction, generating selenium species with distinct oxidation states and redox properties. In this study, we investigated the cardiovascular effects of a glutathione/selenite mixture (GSH/SeO32-) in spontaneously hypertensive rats (SHRs). Arterial pulse waves were recorded using a pressure microcatheter and vascular tension in isolated arteries was assessed by isometric wire

BACKGROUND: The Sister Mary Joseph's nodule (SMJN) is an umbilical skin metastasis observed in the advanced stages of abdominopelvic malignancies. It is rare and is typically associated with a poor prognosis. The appearance of the skin lesions is non-specific, often leading to misdiagnosis. Early detection and diagnosis of the primary lesion are essential for prompt management and improved patient survival. CASE PRESENTATION: In this report, we present the case of a 68-year-old North African woman with a hypertensive disorder who was referred to our oncology center for an umbilical mass that

Rosiglitazone-Mediated Activation of PPARγ Induces PlGF Expression in Trophoblast Cells.

Preeclampsia (PE) is a hypertensive pregnancy disorder marked by impaired trophoblast invasion and placental vascular dysfunction, resulting in severe maternal and fetal complications. Placental growth factor (PlGF) is critical for proper placental angiogenesis and is transcriptionally regulated by glial cell missing-1 (GCM1), a downstream effector of peroxisome proliferator-activated receptor-gamma (PPARγ). Decreased PPARγ activity in PE may therefore contribute to diminished PlGF levels, worsening placental pathology. In this study, we investigated the mechanistic role of rosiglitazone, a PP

JOSD2 alleviates hypertensive renal disease through deubiquitinating AKT in renal tubular epithelial cells.

Hypertensive renal disease (HRD) is a significant driver of end-stage renal disease. Discovering novel therapeutic targets for HRD is essential for its prevention and treatment. Deubiquitinating enzymes (DUBs) have shown increasing significance in renal diseases. Here, we investigated the role and mechanism of the DUB, Josephin domain-containing protein 2 (JOSD2), in HRD. HRD was induced in wild-type or Josd2 knockout mice via a 4-week chronic infusion of angiotensin II (Ang II). We found that deficiency of JOSD2 aggravated renal injury, epithelial-mesenchymal transition (EMT), and fibrosis in

Microbial Metabolites and Cardiovascular Dysfunction: A New Era of Diagnostics and Therapy.

Cardiovascular diseases (CVDs) pose a significant threat to human life and mortality worldwide, encompassing a variety of conditions that affect the heart and blood vessels. These diseases are influenced by both genetic and environmental factors, which play a critical role in their development. Recent research has highlighted the importance of gut microbes-the diverse community of bacteria in the gastrointestinal tract-that function as a "super organ" within the human body. These microbes have a remarkable impact on metabolic pathways and are increasingly recognized for their role in serious c

An Optimized Aerogel-Based Apheresis Device for Targeted Lipid Clearance in Elderly Hyperlipidemia Patients.

Elderly patients with hyperlipidemia often exhibit resistance to conventional hypolipidemic treatments, underscoring the need for more effective strategies to address lipid imbalances in this high-risk group. This study introduces LipClean, an aerogel-based apheresis device specifically designed to remove harmful plasma lipids. LipClean is constructed using hydrophilic cellulose fibers, which serve as a supramolecular platform for synthesizing hydrophobic conjugated polymers through a Sonogashira-Hagihara reaction. These conjugated polymers are then cross-linked with the cellulose fibers via p

Obesity, insulin resistance and fertility: unresolved questions and emerging insights.

PURPOSE OF REVIEW: Obesity significantly impacts fertility in women, contributing to hormonal imbalances, ovulatory dysfunction, and poor reproductive outcomes. This is especially pronounced in polycystic ovary syndrome (PCOS), where obesity and insulin resistance exacerbate fertility challenges. Moreover, obesity is a risk factor for type 2 diabetes (T2D) and gestational diabetes (GDM), further complicating reproductive health. Effective weight loss interventions before conception are essential to improve fertility and reduce the risks of adverse perinatal outcomes, such as GDM, hypertensive

From Pregnancy to Postpartum: The Cardiovascular Risks Associated with Gestational Diabetes.

Cardiovascular disease is the leading cause of pregnancy-related mortality, with pregnancy-related cardiovascular issues extending into the postpartum period. Recent studies suggest hyperandrogenism alters sex hormone levels, contributing to gestational cardiovascular disease CVD. Most of the factors behind the onset of CVD in postpartum women remain unknown. Animal studies mimic adverse pregnancy outcomes to explore molecular causes of severe prenatal cardiac events and their role in postpartum cardiovascular disease development. This review will be focused on summarising human and animal res

An Adapted Hybrid of Open and Laparoscopic Techniques for Ventral Hernia Repair in a Resource-Constrained Setting.

Ventral hernias are common surgical conditions managed through various techniques, including open, laparoscopic, and hybrid approaches. Laparoscopic repairs offer advantages such as reduced postoperative pain and faster recovery but carry an increased risk of bowel injury. Of all the repairs, retrorectus repair has demonstrated superior long-term outcomes. In resource-limited settings, adapting existing techniques, with consideration of cost, is essential to enhance patient safety and surgical efficiency. This report presents the case of a 65-year-old female, known hypertensive with a BMI of 3
