A Challenging Case of Creutzfeldt-Jakob Disease Presenting as Stroke.

INTRODUCTION: Early recognition of Creutzfeldt-Jakob disease relies on awareness of its clinical variability, particularly in patients with complex medical comorbidities. CASE PRESENTATION: A 66-year-old woman presented with generalized weakness, tremor, double vision, and slurred speech. Initial imaging and laboratory studies were unremarkable, except for subtle magnetic resonance imaging findings suggestive of encephalitis. Despite treatment with methylprednisolone, her condition worsened. Repeat magnetic resonance imaging raised suspicion for Creutzfeldt-Jakob disease, which was confirmed

The Paucity of Typical Pathology: An Autopsy Series of Typhus Group Rickettsiosis-Associated Hemophagocytic Lymphohistiocytosis.

Murine typhus (also called flea-borne or endemic typhus) is an undifferentiated febrile illness caused by the bacterium Rickettsia typhi. The disease, transmitted by rat and cat fleas, is endemic to seaboard regions worldwide. Recently, murine typhus has reemerged as an increasingly recognized cause of febrile illness in the United States, especially in Texas and Southern California. In addition to fever, manifestations often include headache, malaise, myalgias, and a maculopapular rash in approximately half of cases. Although usually considered a mild illness, when untreated, symptoms can las

Clinical case of a patient with fatal development of gas gangrene while using totally implantable venous access devices.

Gas gangrene, or clostridial myonecrosis, is a rare and life-threatening infection caused primarily by Clostridiumperfringens. Despite its reduced incidence due to advancements in wound care and antibiotics, the disease remains associated with significant morbidity and mortality, particularly in immunocompromised individuals. This report presents a clinical case of a 37-year-old male with esophageal carcinoma and a totally implantable venous access device (Port-a-Cath), who developed fatal gas gangrene following chemotherapy.

Sinonasal Nocardia farcinica in a cat with comorbidities.

A 15-year-old spayed female domestic shorthair cat was referred because of a chronic history of swelling over the right eye that had progressed to an open, exudative wound. The lesion had not responded to prolonged antimicrobial treatments that were chosen based on superficial culture results. Before this development, the cat had been diagnosed with International Renal Interest Society (IRIS) stage 2 chronic kidney disease with no other significant health issues. Computed tomography revealed fluid to soft-tissue attenuating material in the sinonasal cavities and concurrent bony lysis. Cytology

Disseminated Mycobacterium tilburgii infection complicated by pulmonary non-tuberculosis mycobacteriosis in a patient with acquired immunodeficiency syndrome: A case report and literature review.

RATIONALE: Mycobacterium tilburgii is the causative agent of disseminated non-tuberculosis mycobacterial infections in individuals who are immunocompromised, including those with human immunodeficiency virus infection. Owing to its non-culturable nature, identification of M. tilburgii relies solely on genetic analysis, making reports of M. tilburgii infections rare. We report a case of disseminated M. tilburgii infection complicated by a mixed pulmonary non-tuberculosis mycobacterial infection in a patient with acquired immunodeficiency syndrome. PATIENT CONCERNS: A male patient in his 40s, d

Unexpected decline of pulmonary function in a patient with thymoma mimicking obstructive bronchiolitis.

Obstructive bronchiolitis (OB) is a rare disorder characterized by progressive airflow limitation and is most commonly associated with autoimmune diseases, inhalation of toxins, lung transplantation, and hematopoietic stem cell transplantation. Thymoma is a rare mediastinal tumor frequently accompanied by autoimmune diseases, such as myasthenia gravis. We report a male patient with mild chronic obstructive pulmonary disease who developed thymoma and severe respiratory dysfunction mimicking OB. Postmortem autopsy revealed histopathological findings resembling systemic graft-versus-host disease

Radiographic Aspects of Pentastomiasis in Southern American Bushmaster (Lachesis rhombeata).

Pentastomids are parasites of the respiratory system of reptiles, birds, and mammals, where they can cause lesions resulting in the death of their intermediate hosts. This report describes radiographic aspects of pulmonary pentastomid infection in the Southern American bushmaster (Lachesis rhombeata). A female juvenile snake rescued in an urban area of the Northeast region of Brazil presented with lethargic behavior. Radiographic examination of the coelom cavity showed long cylindrical structures in the respiratory system with soft tissue radiodensity and width ranging between 4.0 and 5.0 mm.

Campylobacter jejuni hepatitis in a horse: case report and literature review.

Campylobacter spp. can cause gastroenteritis, hepatitis, bacteremia, and abortions in domestic animals and humans. Some Campylobacter spp. are zoonotic. To our knowledge, hepatitis caused by Campylobacter jejuni has not been reported in horses. Here we present a case of acute necrosuppurative hepatitis caused by C. jejuni infection in a 3-y-old gelding, and we review the literature on C. jejuni infections in various animal species. The horse had a one-week history of weight loss and weakness before becoming recumbent and dying. Grossly, the liver had rounded edges and was mottled. There were e

Fatal acute respiratory distress syndrome caused by donor-derived human metapneumovirus after lung transplantation.

Respiratory viral infections, including human metapneumovirus (hMPV), are increasingly recognized as a significant cause of morbidity and mortality in lung transplant recipients (LTRs). We present a case of hMPV pneumonia in a bilateral LTR, likely transmitted by the donor, resulting in severe acute respiratory distress syndrome and leading to death. Next-generation sequencing of hPMV genomes was performed from both the recipient's bronchoalveolar lavage and the organ conservation liquid and strongly supported the presence of donor-derived infection. There is still limited reported evidence to

Mandibular Metastasis as the First Manifestation of an Intrahepatic Cholangiocarcinoma.

PURPOSE: Cholangiocarcinoma is a malignant neoplasm with biliary differentiation and its metastatic spread to the oral and maxillofacial region is a rare event that can pose diagnostic challenges, especially when it is the initial presentation of the disease. METHODS: A 67-year-old woman presented with a painful and hard swelling in the right posterior aspect of the mandible with ulceration of the overlying mucosa. Radiological examination revealed a destructive lesion involving the body and angle of the mandible with cortical bone destruction. An incisional biopsy was performed. RESULTS: The

Multiple brain lesions in a patient with relapsed hairy cell leukemia: a case report and review of the literature.

Cerebral lesions are rare in hairy cell leukemia (HCL), and its incidence remains to be determined. Identifying the cause can be challenging. In this report, we present a case of brain lesions occurring several years after diagnosis. A 76-year-old male patient presented to the Emergency Department with confusion. He had been diagnosed with HCL in 1999 and had received five lines of treatment. Cerebral imaging revealed multiple nodular lesions, with edema and a hemorrhagic appearance. Cerebrospinal fluid tests were negative. The tumor origin was retained due to concomitant relapse (blood, lymph

Clinical Management Strategies for Severe Fever with Thrombocytopenia Syndrome Induced by Tick Bites: A Case Report and Literature Review.

BACKGROUND Severe fever with thrombocytopenia syndrome (SFTS) has a high mortality rate, and diagnosis and treatment options for SFTS-associated encephalopathy/encephalitis(SFTSAE) are limited. CASE REPORT A 79-year-old male farmer developed progressive dyspnea and productive cough 72 hours after tick exposure, accompanied by persistent high-grade fever (38.9°C). Following failed empirical antimicrobial interventions, he was transferred to our institution. At the same time, the local disease control center diagnosed him with fever with thrombocytopenia syndrome (SFTS). Initial evaluations at p

Peritoneal dialysis in congestive heart failure: reflections from a patient with arrhythmogenic right ventricular cardiomyopathy.

After multiple hospitalisations for diuretic-resistant acute decompensated heart failure (ADHF), an early 60s man with isolated right-sided heart failure due to arrhythmogenic right ventricular cardiomyopathy (ARVC) underwent continuous venovenous haemofiltration without lasting success. His blood urea nitrogen/creatinine ratio was high and renal blood flow and filtration fraction were low, indicative of the underlying cardiorenal dysbalance. Peritoneal dialysis (PD) was initiated because this form of renal replacement therapy provides better haemodynamic stability and a flexible schedule. Thi

Enterovirus D68 pneumonia in two older patients: A case report and literature review.

Enterovirus D68 mainly causes respiratory tract infections, with children being the main infected population and most adults experiencing latent infections. This report presents the cases of two older patients with enterovirus D68 pneumonia. Both of them were diagnosed using next-generation sequencing, and one of them died of mediastinal emphysema despite undergoing various treatments. This report also reviewed the biological characteristics and epidemiological features of enterovirus D68 as well as the clinical diagnosis and treatment of the viral pneumonia. Currently, there are no specific a

Intentional Hydroxychloroquine Overdose Leading to Severe Hypotension and Multiorgan Failure.

INTRODUCTION: The use of hydroxychloroquine for treating malarial infections and certain autoimmune diseases is well established, but data on hydroxychloroquine overdose and management are relatively scarce. Given its increased use in recent years and because life-threatening symptoms can occur within hours of ingestion, it is important to understand its potential for toxicity and available treatment options. CASE PRESENTATION: We present a case of a 43-year-old male with intentional overdose of 6 grams hydroxychloroquine, along with 6 grams trazodone and 30 grams metformin. He presented in r

Group 4 pulmonary hypertension due to choriocarcinoma after molar pregnancy: case report and review.

A 46-year-old Mexican woman developed type 4 pulmonary hypertension (PH) due to tumor embolism from choriocarcinoma (CC), which is an uncommon presentation of this malignancy. She presented with precordial pain, dyspnea, and rapid functional decline. Imaging revealed obstructive thrombi in the pulmonary arteries, initially suggestive of operable chronic thromboembolic PH (CTEPH). A bilateral subsegmental pulmonary thromboendarterectomy was performed, the standard treatment for CTEPH, aimed at removing obstructive material and restoring hemodynamic stability. However, histopathological examinat

Clinical Analysis of Primary Prostatic Diffuse Large B-Cell Lymphoma.

OBJECTIVE: Given the extreme rarity of primary diffuse large B-cell lymphoma of the prostate (P-DLBCL-P), medical institutions face challenges in accumulating a sufficient number of cases. Thus, investigating the clinical manifestations of this disease and exploring potential therapeutic strategies holds significant importance. CASE REPORT: We present three cases of P-DLBCL-P, all admitted to our hospital with symptoms of lower urinary tract obstruction. Following pathological examination of the prostate, all three patients were definitively diagnosed with P-DLBCL-P. DISCUSSION: Case 1 disch

Spontaneous Tumor Lysis Syndrome as a Complication of Metastatic Breast Cancer: A Rare Case Report.

Tumor lysis syndrome (TLS) is a well-established potentially fatal condition which is considered an oncologic emergency. TLS typically develops from the rapid lysis of neoplastic cells after the commencement of cytotoxic treatment in patients with some kinds of lymphomas, especially the Burkitt variant, and acute lymphoblastic leukemia. Spontaneous TLS (STLS), a variant of TLS occurring in the absence of chemotherapy or radiation therapy, has been observed in non-Hodgkin lymphoma and acute leukemia, and rarely in solid neoplasms. In this paper, we present a 62-year-old woman with STLS, seconda
