Bennett Fractures: A Biomechanical Model and Relevant Ligamentous Anatomy.

PURPOSE: This study examined a palmar beak fracture model to determine which thumb carpometacarpal (CMC) joint ligament is the primary ligament relevant to the pattern of injury. METHODS: Six fresh-frozen cadaveric wrists were used. The radius, ulna, and first metacarpal were secured and tested with a materials testing system, holding the wrist in 20° extension, 20° ulnar deviation, and 30° palmar abduction of the first metacarpal. Testing consisted of preconditioning cycles followed by compressive loading at 100 mm/s. We confirmed fractures with fluoroscopy and dissected the specimens to exa

The Posterior Bundle's Effect on Posteromedial Elbow Instability After a Transverse Coronoid Fracture: A Biomechanical Study.

PURPOSE: There has been increased interest in the role of the posterior bundle of the medial collateral ligament (pMUCL) in the elbow, particularly its effects on posteromedial rotatory stability. The ligament's effect in the context of an unfixable coronoid fracture has not been the focus of any study. The purposes of this biomechanical study were to evaluate the stabilizing effect of the pMUCL with a transverse coronoid fracture and to assess the effect of graft reconstruction of the ligament. METHODS: We simulated a varus and internal rotatory subluxation in 7 cadaveric elbows at 30°, 60°,

The traumatized vertebral spine reloaded: injury mechanisms and their radiologic patterns.

The ideal classification of spinal trauma does not yet exist, primarily because the implementation of morphological, biomechanical, and clinical parameters in a single nomenclature is a difficult task. For radiologists and surgeons, who are partners in trauma teams, only a few classifications of injury patterns have been shown to be useful enough to provide rapid and stable therapy decisions in daily practice. From a didactic point of view, however, simplifications of injury mechanisms are of help to become aware of the most important radiologic injury patterns of vertebral trauma. The members

[Clinical outcome of surgical treatment of terrible triad of elbow].

OBJECTIVE: To evaluate the clinical outcome of surgical treatment of the posterior dislocation of the elbow with coroniod and radial head fractures. METHODS: From January 2004 to March 2009, 9 patients with terrible triad of the elbow were reviewed. There were 7 males and 2 females (4 left elbows and 5 right elbows), with an average age of 41.2 years, ranged from 21 to 67 years. The radial head fractures were classified according to the Schatzker-Tile criteria: 4 patients had the fractures of type I, 3 patients had type II and 2 patients had type III. The ulnar coronoid fractures were classif

[Diagnosis and treatment of terrible triad of elbow].

OBJECTIVE: To study diagnosis and treatment methods of terrible triad of elbow. METHODS: From June 2001 to June 2009, 7 patients with terrible triad of elbow were reviewed. Among the patients, 6 patients were male and 1 patient was female, ranging in age from 20 to 68 years, averaged 36.5 years. All the fractures were fresh. All the patients were treated through a single lateral approach. The general approach was used to repair the damaged structures sequentially from deep to superficial, from coronoid to anterior capsule, to radial head, lateral ligament complex, and common extensor origin.

Percutaneous reduction and external fixation for foot and ankle fractures.

Percutaneous reduction with external fixation is an advanced technique for surgical management in foot and ankle trauma. In the high-risk patient, closed or percutaneous reduction followed by external fixation for reduction and stabilization is a unique alternative to achieve anatomic stabilization without increased physical strain to the patient. The significant advantages of this approach include minimizing damage to the vascular supply, maintaining and preserving a stable soft tissue envelope, and allowing for correction to be performed acutely or adjusted gradually over time. This article

A specific closed percutaneous technique for reduction of Jeffery type II lesion.

Open reduction is commonly recommended in Jeffery type II fractures. Attempts to reduce these fractures percutaneously were reported as unsafe and unreliable. We revisited this technique and used a specific percutaneous reduction that turned out to be successful in two cases. Instead of lifting the radial head as described in leverage maneuver, we use a pushing-back procedure to reduce the fracture. The maneuver aims at suppressing the capitellum interposition between the head fragment and the metaphysis by reproducing the reversed trajectory of trauma. This reduction is made possible because

Controversies in ankle fracture treatment. Indications for fixation of stable Weber type B fractures and indications for syndesmosis stabilization.

Although the treatment of ankle fractures is often straightforward, several controversies remain. This article reviews the need for open reduction and internal fixation of the displaced supination-external rotation fracture, and contrasts the studies that mandate anatomic reduction with clinical results. The many issues surrounding syndesmosis fixation are also reviewed, including the need for fixation of distal fractures and the timing of screw fixation.

Controversies regarding radial neck fractures in children.

Fractures of the radial neck in children are not uncommon, yet several aspects of their management remain controversial. Until a consensus is reached regarding the determination of displacement, acceptability of initial angulation, treatment, and outcome, the complication rate of these fractures will remain high. The authors recommend measuring the displacement as the angle between a line perpendicular to the articular surface of the radial head with a line down the shaft of the proximal radius. Fractures that are angulated less than 30 degrees require immobilization alone. Fractures angulated

Transtriquetral perihamate ulnar axial dislocation and palmar lunate dislocation.

Transtriquetral perihamate ulnar axial dislocation associated with palmar lunate dislocation is a rare condition. We could find no other similar cases reported. The mechanism of injury was a combined anteroposterior crushing force that caused axial disruption of the carpus and the metacarpals and wrist hyperextension that caused the transtriquetral lunate dislocation. Our patient was treated with open reduction and internal fixation with good result.

Repair of sixth lumbar vertebral fracture-luxations, using transilial pins and plastic spinous-process plates in six dogs.

Transilial pins and paired plastic spinous-process plates were used to repair fracture-luxations of the sixth lumbar vertebra in 6 dogs. All dogs had signs of lumbar pain and variable lower motor neuron deficits to the hindquarters (including hind limbs, tail, and pelvic region). Lumbar pain was decreased or resolved the day after surgery in all dogs, and 3 dogs were able to walk without assistance. One dog initially deteriorated neurologically after surgery and 2 dogs with multiple concurrent orthopedic injuries had no improvement in neurologic function and remained nonambulatory. Pin migrati

Essentiality of the lateral cervical spine radiograph.

Proper interpretation of the lateral cervical spine (C-spine) film is a prime emergency medicine responsibility. Patients with potential neck injury must be identified rapidly and handled carefully; their C-spine films must be interpreted quickly and accurately. We stress the urgency in obtaining a good, readable lateral C-spine film and present a systemic "ABCs" approach to reading this film. This approach includes specific, sequential attention to Alignment, Bony integrity, Cartilaginous structures, and the surrounding Soft-tissue spaces. In the cases of 50 consecutive patients with C-spine

A comparison of different surgical approaches to hemiarthroplasty for the femoral neck fractures: A meta-analysis.

There are three traditional surgical approaches to hemiarthroplasty (HA) for femoral neck fractures, respectively, the anterior approach (AA), the lateral approach (LA) and the posterior approach (PA). However, the optimum approach is still controversial, the purpose of this meta-analysis is to identify the merits and demerits of all three approaches. All clinical published studies in PubMed, Web of Science, Embase, and the Cochrane Library from January 2000 to April 2022 were searched which compared different surgical approaches and covered surgery-related outcomes and frequent complications.

Deformation and Strength Parameters of a Composite Structure with a Thin Multilayer Ribbon-like Inclusion.

Within the framework of the concept of deformable solid mechanics, an analytical-numerical method to the problem of determining the mechanical fields in the composite structures with interphase ribbon-like deformable multilayered inhomogeneities under combined force and dislocation loading has been proposed. Based on the general relations of linear elasticity theory, a mathematical model of thin multilayered inclusion of finite width is constructed. The possibility of nonperfect contact along a part of the interface between the inclusion and the matrix, and between the layers of inclusion wher

Nebulized Ketamine Used for Pain Management of Orthopedic Trauma.

BACKGROUND: Ketamine is a noncompetitive N-methyl-D-aspartate/glutamate receptor complex antagonist that decreases pain by diminishing central sensitization and hyperalgesia. When administered via i.v. (push-dose, short infusion, or continuous infusion) or intranasal routes, ketamine has shown to be effective in patients with acute traumatic pain. However, when i.v. access is not attainable or readily available, the inhalation route of ketamine administration via breath-actuated nebulizer (BAN) provides a noninvasive and titratable method of analgesic delivery. The use of nebulized ketamine ha

Local infiltration of liposome bupivacaine in foot and ankle surgery: case-based reviews.

Foot and ankle surgical procedures, ranging from simple procedures, such as bunionectomy and correction of hammer toe, to more complex surgery, such as ankle fusion and ankle replacement, are extremely painful. Moreover, there is increasing interest in performing these procedures in an outpatient setting. Nerve blocks are extensively used in foot and ankle surgery, and commonly used techniques include sciatic nerve block with saphenous nerve augmentation; ankle block; and local, digital, or field block. Whereas more extensive blocks are associated with increased medical risk, higher cost, and
