Parathyroid carcinoma with intracranial metastasis at diagnosis in a patient with uncontrolled hypercalcemia.

Parathyroid carcinoma is an uncommon malignancy and a rare cause of primary hyperparathyroidism. Although this tumor is capable of metastasi … eport a case of metastatic parathyroid carcinoma with two intracranial metastatic foci (in the left frontal lobe and left cerebellar hemisphere) identified at the time of the primary tumor diagnosis in a patient who presented with symptomatic hypercalcemia.

Nicotinic acetylcholine receptors in human genetic disease.

Nicotinic acetylcholine receptors represent a family of ligand-gated ion channels that are widely expressed in the central and peripheral nervous systems. To date, 16 genes encoding subunits of mammalian nicotinic acetylcholine receptors have been identified. The various subunits form homomeric or heteromeric receptor proteins, allowing for a complex and adaptable system of nicotinic neurotransmission. Mutations of nicotinic receptor genes can cause Mendelian disorders, most importantly congenital myasthenic syndromes, multiple pterygium syndromes, and nocturnal frontal lobe epilepsies. Haploi…

Transient involvement of the cerebral cortex in a flare of Behçet's syndrome.

We describe a flare of Behçet's syndrome in a 42-year-old man who presented with florid mucocutaneous manifestations, headache and vomiting, but without meningeal or neurologic signs. A single, non-enhancing cortical lesion was found in the frontal lobe by computed tomography and magnetic resonance (MR) imaging studies. Diffusion-weighted analysis and MR spectroscopy were consistent with focal inflammation. The patient improved with colchicine, and after 5 months, a repeat MR scan demonstrated resolution of the inflammatory changes suggesting that involvement of the cerebral cortex may be a se…

Extraventricular neurocytoma in a child: case report and review of the literature.

Neurocytomas are relatively rare central nervous system tumors first described in intraventricular sites but increasingly recognized in the brain and spinal cord parenchyma. Herewith, we report a case of a 3-year-old girl with generalized seizures. Computed tomography and magnetic resonance imaging revealed a lesion in the left frontal lobe. Angiography showed minimal vascularity. The tumor was totally excised. Histopathologic examination demonstrated an extraventricular neurocytoma. On follow-up magnetic resonance imaging after 1 year, no evidence of tumor recurrence was noted. Cerebral neuro…

Progressive Multifocal Leukoencephalopathy in HIV-Infected Children: A Case Report and Literature Review.

We report a case of a perinatally HIV-infected patient aged 9 years, who presented with right-sided hemiplegia. His initial CD4 T-cell was of 0.21% (4 cells/muL) and plasma HIV RNA virus of 185 976 copies/mL (log 5.27). Plasma and CSF samples were subsequently positive for JCV. Twelve days after the initiation of highly active antiretroviral therapy (HAART), the MRI showed progressive white matter lesions with asymmetrical deep and subcortical white matter lesions over the left frontotemporoparietal region and the right frontal lobe. Immune Reconstitution Inflammatory Syndrome (IRIS) was suspe…

Unusual case of 2 renal cell carcinoma metastases ipsilateral to an occluded internal carotid artery.

Hemodynamic factors may play a role in the seeding and subsequent growth of cerebral metastasis. The authors present a case with 2 foci of cerebral metastasis in the same vascular territory ipsilateral to an occluded internal carotid artery. A 65-year-old man with chronic left frontal lobe infarct and left internal carotid artery occlusion was diagnosed with 2 large hemorrhagic metastases in the left middle cerebral artery territory. Diminished flow signals were seen in the trunk and branches of the left middle cerebral artery on magnetic resonance angiography. This case illustrates preferenti…

Anaplastic secretory meningioma: a case report.

Secretory meningiomas are rare histological subtypes of meningiomas with benign biological behaviour. In this study, the authors describe the first case of secretory meningioma with many mitotic figures and brain invasion, and discuss the clinicopathologic features including immunohistochemical staining profile and ultrastructural appearance of this tumour. A case of a 54-year-old man diagnosed with an intracranial tumour located in the left frontal lobe is presented. On pre-contrast CT scans, the tumour was hypodense and the contrast enhancement was marked in the pseudo membrane. The tumour w…

[Functional neuroanatomy of emotions].

OBJECTIVE: In the present study, functional neuroanatomy of the frontal lobe and limbic system could be uncovered as emotion processing neural substrates. fMRI was used to test the influence of induced emotions on a non-affective, attention-demanding, cognitive task. METHOD: 6 Psychopaths versus 7 controls were analyzed by fMRI with a simon-paradigm after emotion induction. RESULTS: Emotion induction differentially influenced cortical activation patterns in emotion related brain areas like the amygdala and the medial prefrontal cortex. CONCLUSION: Structural-morphological neural areas of em…

Intraventricular ganglioglioma: case report.

We report a case of ganglioglioma located in the body and frontal horn of the right lateral ventricle. The lesion was found in a 71-year-old man who suffered from recurrent episodes of right hemicranial headache. CT and MRI showed a heterogeneous intraventricular tumour with multiple microcysts that suggested a diagnosis of subependymoma or central neurocytoma, but the pathological findings were consistent with ganglioglioma. Although the most frequent intracranial location of ganglioglioma is the temporal lobe, any location in the central nervous system may be seen. Nevertheless an exclusivel

Large diffuse arteriovenous malformation affecting the right cerebral and cerebellar hemispheres: case report.

A cerebral vascular malformation was triggered in a 24-year-old man by left hemiparesis resembling transient ischemic attack. Cerebral angiography revealed slight dilation of the right anterior cerebral artery, middle cerebral artery, superior cerebellar artery, and posterior inferior cerebellar artery, as well as marked capillary blush and dilation of medullary veins involving the right cerebral hemisphere, especially the frontal lobe and cerebellar hemispheres. This vascular malformation, a clinical rarity, seemed to be in the same family as diffuse arteriovenous malformation or "medullary v…

Neurologic complications following bone marrow transplantation for sickle cell disease.

A boy with sickle cell anemia underwent bone marrow transplantation (BMT). He was normal on neurological examination, but had radiologic evidence of an old left frontal lobe infarct, multiple cerebral vascular stenoses and moyamoya collaterals. After BMT he developed seizures with extension of the infarct and subarachnoid hemorrhage. One year later angiography revealed worsening stenosis of the M1 segments of both middle cerebral arteries. At that time an increase in von Willebrand's factor with decreased large molecular weight multimers (LvWF) was observed. We speculate that LvWF dependent, s…

Septation and focal dilatation of ventricles associated with cryptococcal meningoencephalitis.

A 35-year-old woman developed temporal lobe seizures. Isolated dilatation of the right temporal horn was demonstrated by computed tomography. She was asymptomatic for the next 10 months while on anticonvulsants before severe headaches, vomiting, and mental confusion prompted hospitalization. Both temporal horns were now dilated, there was marked periventricular edema, and cryptococci were cultured from the ventricular fluid. She succumbed after prolonged systemic and intrathecal antifungal therapy, having developed isolation and dilatation of both frontal horns and third and fourth ventricles.

Behaviour outcomes in children with epilepsy 1 year after surgical resection of the ventromedial prefrontal cortex.

Early damage to the ventromedial prefrontal cortex (VM) has been associated with impaired behavioural functioning in children without epilepsy, yet behaviour in children with epilepsy and VM lesions has not been investigated. The primary aim of this study was to examine behavioural outcomes in children with epilepsy emanating from the VM preoperatively and one year after epilepsy surgery compared to the general population and matched epilepsy controls. Behavioural outcomes were defined as comprising both problems and competencies (i.e. social, school and co-curricular performance). A secondary

Brain regions and functional interactions supporting early word recognition in the face of input variability.

Perception and cognition in infants have been traditionally investigated using habituation paradigms, assuming that babies' memories in laboratory contexts are best constructed after numerous repetitions of the very same stimulus in the absence of interference. A crucial, yet open, question regards how babies deal with stimuli experienced in a fashion similar to everyday learning situations-namely, in the presence of interfering stimuli. To address this question, we used functional near-infrared spectroscopy to test 40 healthy newborns on their ability to encode words presented in concomitance

Evaluation of the new consensus criteria for the diagnosis of primary progressive aphasia using fluorodeoxyglucose positron emission tomography.

BACKGROUND: New consensus criteria have been proposed to classify primary progressive aphasia (PPA) into three variants: agrammatic, semantic, and logopenic. Some studies have subsequently addressed the usefulness of these criteria, with controversial results. We aimed to determine the correlation between the clinical diagnosis according to the new criteria and brain topography in (18)F-fluorodeoxyglucose positron emission tomography (FDG-PET). METHODS: Patients meeting the PPA criteria were prospectively recruited in a single center during a period of 18 months. They were clinically classifi

Brain activity in predominantly-inattentive subtype attention-deficit/hyperactivity disorder during an auditory oddball attention task.

Previous functional neuroimaging studies have found brain activity abnormalities in attention-deficit/hyperactivity disorder (ADHD) on numerous cognitive tasks. However, little is known about brain dysfunction unique to the predominantly-inattentive subtype of ADHD (ADHD-I), despite debate as to whether DSM-IV-defined ADHD subtypes differ in etiology. This study compared brain activity of 18 ADHD-I adolescents (ages 12-18) and 20 non-psychiatric age-matched control participants on a functional magnetic resonance image (fMRI) auditory oddball attention task. ADHD-I participants had significant
