Integration of Substance Use Disorder Treatment Into a Traditional Community Mental Health Treatment System.

In response to the opioid crisis, Catholic Charities of Baltimore sought to integrate substance use disorder treatment into their outpatient community mental health clinics. The agency developed a systematic practice improvement strategy that included a competency-based curriculum and supervision plan for psychiatric providers and therapists. Psychiatric providers developed competency with medications used to treat substance use disorders, and therapists developed competency in therapeutic modalities to treat substance use disorders, all of which were gradually integrated into outpatient clini

Developing protocols for obstetric emergencies.

There is potential for important steps to be missed in emergency situations, even in the presence of many health care team members. Developing a clear plan of response for common emergencies can ensure that no tasks are redundant or omitted, and can create a more controlled environment that promotes positive health outcomes. A multidisciplinary team was assembled in a large community hospital to create protocols that would help ensure optimum care and continuity of practice in cases of postpartum hemorrhage, shoulder dystocia, emergency cesarean surgical birth, eclamptic seizure and maternal c

Intimacy and multiple sclerosis.

Persons with multiple sclerosis require extensive management of the physiologic and psychologic sequelae of their chronic disease process. Sexual intimacy is affected by many of these disease effects, which impacts them in diverse ways. Many persons with multiple sclerosis do not discuss sexual intimacy with their health care provider because they assume it is an expected part of the disease and nothing can help, or they are too embarrassed to admit to problems at a very young age. Since this is a disease that usually occurs between 20 and 40 years of age, sexual intimacy is affected from many

Progress toward poliomyelitis eradication--African Region, 1998-April 1999.

In 1988, the World Health Assembly resolved to eradicate poliomyelitis globally by 2000. To achieve this goal, the African Region (AFRO) of the World Health Organization (WHO) has accelerated polio eradication strategies, but the region remains one of the two major reservoirs for wild poliovirus transmission. This report summarizes progress toward polio eradication from 1998 through April 1999 in AFRO, highlights supplementary vaccination activities (National Immunization Days [NIDs]) and acute flaccid paralysis (AFP) surveillance conducted in the region, and describes plans for program accele

Value of mammographic screening: assessment of studies and opinion.

The purpose of mammographic screening is to improve patient survival by detecting cancer at an earlier, more curable stage than is possible with physical examination. Three major studies have been used to assess the effect of mass screening on the survival rates and mortality for patients with breast cancer: the Breast Cancer Detection Demonstration Project, the Health Insurance Plan study, and the study conducted by the Swedish National Board of Health and Welfare. The author reviews these studies, describes statistical biases (lead time, length time, patient self-selection, and overdiagnosis

Diverging trends in health at older ages in England, 2004-2024: evidence from the English Longitudinal Study of Ageing.

BACKGROUND: Given uncertainty about whether later-life health at similar ages is improving over time, we examined trends across multiple health domains. METHODS: We analysed data from community-dwelling adults aged 50 and older in the English Longitudinal Study of Ageing in 2004/05, 2012/13, and 2023/24 (main survey: N=8389, 8549, and 6090, respectively). Outcomes included self-rated health, limiting long-standing illness, pain, mobility limitations, cardiometabolic and chronic conditions, obesity, inflammation, mental health, quality of life, and memory. Weighted pooled modified Poisson and

The Red River Cart Model: a Métis conceptualization of health and well-being in the context of HIV and other STBBI.

OBJECTIVES: Métis people experience health inequities and often face discrimination when accessing health services. Métis-specific services are limited, and pan-Indigenous approaches to health services fail to acknowledge heterogenous identities and distinct health needs of the Métis. This study explored a Métis response to HIV and other sexually transmitted and blood borne infections to inform public health services development for Métis people. METHODS: As part of the DRUM & SASH Project, this study used a community-based research approach which privileged Métis knowledges and processes. Th

Health professionals' responses to multiple sclerosis and motor neurone disease.

This study compares the responses of health professionals to multiple sclerosis (MS) and motor neurone disease (MND) in order to determine similarities and variations in responses to the two disorders and the issues critical to caring for patients with these conditions. Health professionals were more negative about MND compared with MS in terms of the amount they felt able to offer patients, their confidence in managing patients and their ability to convey hope. For a number of issues concerning the management of patients, the level of difficulty experienced by health professionals was similar

Determinants of bacterial survival and proliferation in blood.

Bloodstream infection is a major public health concern associated with high mortality and high healthcare costs worldwide. Bacteremia can trigger fatal sepsis whose prevention, diagnosis, and management have been recognized as a global health priority by the World Health Organization. Additionally, infection control is increasingly threatened by antimicrobial resistance, which is the focus of global action plans in the framework of a One Health response. In-depth knowledge of the infection process is needed to develop efficient preventive and therapeutic measures. The pathogenesis of bloodstre

Recently released with HIV/AIDS: primary care treatment needs and experiences.

The research objectives of this study are to describe the re-entry experiences of people recently released from jail who were living with HIV/AIDS, and to identify factors that influence their access to primary care and adherence to a treatment regimen. The research used a mixed-method, qualitative and quantitative research design. The findings indicate that the overall instability in the lives of many of the former inmates studied hampered their ability to attend to their HIV/AIDS-related health care needs. Most former inmates returned to the community with co-occurring problems related to ho

What leads to good health in midlife women physicians? Some clues from a longitudinal study.

Data from a longitudinal study of women physicians is used to study psychosocial and current implications of good health in midlife. The women comprise the classes 1964 to 1967 of the University of California School of Medicine at San Francisco. The subjects were interviewed and tested using the California Psychological Inventory while they were still in medical school (Time 1, average age 24 years) and at midlife (Time 2, average age 46 years). The women in good health at midlife, in contrast to their peers, were characterized by certain personality traits, including a higher degree of intell

HMOs don't have to fail.

One of the bright spots in the sometimes black picture that is painted of our national health care system is HMOs. Health maintenance organizations provide health services to an enrolled group of patients for a fixed, prepaid fee. This arrangement induces HMOs to try to keep patients healthy for a competitive rate. Because of its special nature, however, an HMO cannot be managed like a fee-for-service hospital or group practice. And because few managers fully understand them, more and more HMOs are failing. The authors of this article say, however, that some failures can be avoided. Good manag

The dialogue dilemma: the role of patient-clinician communication for low-income people who smoke and manage multiple conditions.

INTRODUCTION: Adults from low-income backgrounds who smoke face significant health disparities related to tobacco use, often at disproportionately high rates. These individuals are more likely to endure multiple mental and physical (MP) health conditions, which can negatively influence their self-rated health (SRH). The quality and effectiveness of patient-clinician communication (PCC) can influence how patients perceive their own health. Understanding how PCC influences SRH among low-income adults who smoke and suffer from multiple MP conditions is essential for clinical care as multimorbidit
