Basal Crux Ventricular Tachycardia in Ischemic Heart Disease.

BACKGROUND: Basal crux ventricular tachycardia (VT) is traditionally considered an idiopathic arrhythmia. However, its occurrence in patients with structural heart disease, especially ischemic heart disease (IHD), is often under recognised. CASE SUMMARY: We report two patients with a history of myocardial infarction who presented with basal crux VT. In both cases, VT was rendered non-inducible, and both patients experienced recovery of their left ventricular ejection fraction (LVEF), indicating a significant burden of arrhythmia-induced cardiomyopathy. DISCUSSION: It is unclear whether IHD i…

Lifelong Care of Females With Congenital Heart Disease.

Medical and surgical advancements of the past 70 years have resulted in a remarkable shift in the natural history of congenital heart disease (CHD) such that survival to adulthood is expected for >90% of children born with congenital heart defects, including those with complex CHD. There are now more adults than children living with CHD, a majority of them are female. As significant strides have been made in the management of adult patients with CHD, there has been an evolving understanding of the important role inherent sex-specific differences play in impacting long-term outcomes for females…

Association between sleep disorders and subsequent heart failure.

BACKGROUND: Sleep disorders are prevalent conditions that may influence the progression of various heart diseases. However, the relationship between sleep disorders and the onset of heart failure (HF) remains unclear. OBJECTIVE: The purpose of this study was to investigate whether sleep disorders are associated with an increased risk of developing HF. METHODS: We conducted a retrospective cohort study using data from 1,293 general practices across Germany sourced from the IQVIA Disease Analyzer database. The study included patients with an initial diagnosis of HF (ICD-10 code: I50) between J…

Unmasking High-Output Heart Failure: Beyond Conventional Paradigms.

Heart failure (HF) poses a significant medical challenge, affecting millions of adults in the United States. High-output heart failure (HOHF) is a distinct subtype characterized by elevated cardiac output exceeding 8 L/min or a cardiac index >4 L/min/m². Patients with HOHF often present similarly to those with heart failure with reduced ejection fraction and heart failure with preserved ejection fraction. However, the underlying pathophysiology differs; triggered by a progressive decline in systemic vascular resistance, the ensuing elevation of cardiac output eventually leads to left ventricul…

Cardiogenic shock: Who benefits from heart replacement therapy?

BACKGROUND: The characteristics of patients requiring heart replacement therapy (HRT) in cardiogenic shock (CS) are poorly described. This study aims to characterize the clinical profile of CS patients undergoing HRT. METHODS: FRENSHOCK is a prospective registry including 772 CS patients from 49 centres. HRT included heart transplantation (HTx) and ventricular assist devices (VADs). The primary endpoint was the rate of in-hospital HRT. The secondary endpoint was the rate of HRT performed between discharge and 1-year follow-up. RESULTS: Among 772 patients, 49 (6.3 %) underwent in-hospital HRT…

Arrhythmias in Congenital Heart Disease: Atrial Switch Operations.

Arrhythmias remain a frequent contributor to morbidity and mortality in patients following atrial switch operations for surgical management of dextro-transposition of the great arteries. Effective management of arrhythmias in this patient population requires thorough understanding of the arrhythmia types, substrates, and anatomic contributors and considerations. Prudent application of pharmacologic, ablative, and cardiac implantable electronic device treatment strategies is imperative. Despite treatment advancements, arrhythmia recurrence is unfortunately common and requires lifelong surveilla

[Fetal Heart Failure: Exploring Diagnosis and Treatment Strategies].

It is difficult to appropriately diagnose the severity of fetal heart failure using only ultrasonography. Biomarkers of fetal heart failure in the fetal blood, amniotic fluid, and maternal blood have not been established. Therefore, we investigated natriuretic peptides (NPs) such as Atrial natriuretic peptide (ANP), brain natriuretic peptide (BNP), and N-terminal proBNP (NT-proBNP) in umbilical cord blood and amniotic fluid in cases of fetuses with congenital heart disease, and investigated whether maternal serum biomarkers could diagnose fetal heart failure. The features of NPs in the umbilic…

Heart-Liver Interplay in Patients with Fontan Circulation.

Background: The Fontan procedure has provided pediatric patients suffering from severe congenital heart disease the opportunity to reach adulthood. Increasingly, we encounter the liver repercussions of Fontan circulation, alongside a decline in heart function and exercise performance. This study aims to identify the univentricular heart malformations that are most susceptible to liver dysfunction; assess which markers of liver injury are essential for multidisciplinary clinical follow-up of Fontan patients; determine the optimal approach for evaluating liver function in Fontan patients; and ex…

Disease modifying therapies in heart failure: an update.

There is currently limited evidence-based disease-modifying pharmacotherapy in patients with heart failure and mildly reduced/preserved ejection fraction (HFmrEF/HFpEF), forming an unmet clinical need in contemporary heart failure care. However, according to the findings from recent clinical trials with some medications such as finerenone and glucagon-like peptide-1 receptor agonist, a clinical concept of pharmacological therapy in patients with HFmrEF/HFpEF will be updated and upgraded. Based on recent evidence, we herein aim to discuss the possible roles of those medications in the HFmrEF/HF…

Ciliary biology intersects autism and congenital heart disease.

Autism spectrum disorder (ASD) and congenital heart disease (CHD) frequently co-occur, yet the underlying molecular mechanisms of this comorbidity remain unknown. Given that children with CHD are identified as newborns, understanding which CHD variants are associated with autism could help select individuals for early intervention. Autism gene perturbations commonly dysregulate neural progenitor cell (NPC) biology, so we hypothesized that CHD genes disrupting neurogenesis are more likely to increase ASD risk. Therefore, we performed an in vitro pooled CRISPR interference screen to identify CHD…

Image-based computational hemodynamics in the right heart.

Characterizing flow within the right heart (RH) is particularly challenging due to its complex geometries. However, gaining insight into RH fluid dynamics is of extreme diagnostic importance, given the high prevalence of acquired and congenital heart diseases with impaired RH function. In this proof-of-concept study, we propose a pipeline for patient-specific simulations of RH hemodynamics. We reconstruct the geometry and motion of the patient's right atrium, ventricle, and pulmonary and tricuspid valves, from multi-series cine MRI. For this purpose, we develop a novel and flexible reconstruct…

Subcutaneous furosemide in heart failure: a systematic review.

BACKGROUND AND AIM: Intravenous loop diuretics are the primary treatment for congestion in patients with decompensated heart failure (HF). Furosemide is the most commonly used loop diuretic and is licensed for administration either orally, intramuscularly or intravenously but not subcutaneously. Recently developed, pH-neutral, concentrated, and 'skin-friendly' preparations of furosemide have been developed which allow subcutaneous administration. In this systematic review, we summarize and critically appraise the current evidence for subcutaneous furosemide in patients with HF. METHODS AND RE…
