An Observational Analysis of a Large Cohort of Pneumocystis jirovecii Pneumonia.

BACKGROUND: Pneumocystis jirovecii pneumonia (PCP) is increasingly seen in patients who are non-HIV immunocompromised hosts, yet modern risk factors and outcomes are poorly recognized and defined. METHODS: A retrospective review was conducted of adult patients who met criteria for proven or probable PCP from 2017 to 2024 across a large academic multicenter health system. The primary outcome was mortality among cohorts, defined by etiology of immunosuppression through univariate and age-adjusted logistic regression. Secondary analyses included trends in annual diagnoses and testing patterns. …

Disseminated nocardiosis caused by Nocardia farcinica in a patient with colon cancer: A case report and literature review.

RATIONALE: Nocardiosis is an uncommon and potentially life-threatening infection that usually affects immunocompromised hosts. No clinical guidelines have been established for managing this rare disease, and the optimal treatment modality remains unclear. Nocardia farcinica, a relatively infrequent pathogen of nocardiosis, causes a clinically aggressive infection. In addition to our patient data, our search of the literature for patients who presented with empyema caused by N. farcinica will provide fundamental information for optimal treatment modalities. PATIENT CONCERNS: A 64-year-old man …

Impact of a silver layer on the membrane of tap water filters on the microbiological quality of filtered water.

BACKGROUND: Bacteria in the hospital's drinking water system represent a risk for the acquisition of a nosocomial infection in the severely immunocompromised host. Terminal tap water filters may be used to prevent nosocomial Legionnaires' disease. We present data from water samples using an improved kind of tap water filters. METHODS: In a blinded study on an intermediate care unit of the thoracic surgery department, a modified type of the Germlyser water filter (Aqua-Free Membrane Technology) with a newly-introduced silver layer on the filtration membrane was compared to its preceding type w…

Cytomegalovirus in upper gastrointestinal ulcers.

Cytomegalovirus (CMV), an important cause of severe infections in immunocompromised patients, can cause ulcerations anywhere in the gastrointestinal tract, most commonly stomach and colon. Only a few cases of CMV inclusions in gastrointestinal ulcers have been reported in normal hosts. We undertook a prospective study of the incidence of CMV in gastroduodenal ulcerations in immunocompetent patients. Thirty-eight patients who were referred for evaluation of dyspepsia, hematemesis, melena, guaiac-positive stools, or iron deficiency anemia and who had gastric or duodenal ulcerations without stigm

JC virus DNA is present in many human brain samples from patients without progressive multifocal leukoencephalopathy.

Sections of normal and diseased brain and kidney tissues were screened for the presence of JC virus (JCV) DNA by using the polymerase chain reaction. As expected, all samples obtained from patients with progressive multifocal leukoencephalopathy (PML) tested positive when multiple JCV-specific primer and probe combinations were used. Unexpectedly, more than 50% of non-PML-affected brains were also found to harbor low levels of JCV DNA. To confirm that the positive signals seen in the tissue sections were not the result of contamination, amplified DNA was cloned and sequenced and in some cases

Mouse models of acute and chronic hepatitis E virus infection.

BACKGROUND & AIMS: Hepatitis E virus (HEV) causes approximately 19.47 million symptomatic cases annually and is an emerging cause of chronic hepatitis in immunocompromised individuals. No definite evidence demonstrates that laboratory mice are susceptible to all known HEV genotypes. The factors that define the host species range of HEV remain unclear. Given the numerous genetic variants and toolkits available for mice, which permit deep mechanistic studies, we aimed to develop an inbred mouse model of HEV infection. METHODS: AG129 and C57BL/6J mice were infected with HEV. Key infection parame

Filamentation Is Associated with Reduced Pathogenicity of Multiple Non-albicans Candida Species.

Candidiasis affects a wide variety of immunocompromised and medically compromised patients. Candida albicans, a major human fungal pathogen, accounts for about 50% of all cases, while the remainder are caused by the less pathogenic non-albicans Candida species (NACS). These species are believed to be less pathogenic, in part, because they do not filament as readily or robustly as C. albicans, although definitive evidence is lacking. To address this question, we used strains for two NACS, Candida tropicalis and Candida parapsilosis, which were genetically engineered to constitutively express th

Bacillus Calmette-Guerin (BCG) Vaccine-associated Complications in Immunodeficient Patients Following Stem Cell Transplantation.

PURPOSE: Bacillus Calmette-Guerin (BCG) is a live attenuated vaccine with the potential of causing severe iatrogenic complications in patients with primary immunodeficiency diseases (PID) before and after hematopoietic stem cell transplantation (HSCT). We aim to investigate risk factors of post-HSCT BCG-related complications in PID patients. METHODS: A retrospective analysis of pediatric PID patients who had received the BCG vaccine and underwent HSCT at Hadassah-Hebrew University Medical Center, between 2007 and 2019. RESULTS: We found 15/36 (41.67%) patients who developed post-HSCT BCG-rel

Life Course Immunization in Japan: A Practice-Oriented Framework for Internal Medicine.

Life-course immunization extends vaccination beyond childhood by creating repeated opportunities according to age, disease, immune status, pregnancy, occupation, travel, and social context. This approach is particularly relevant to internal medicine because older adults, patients with chronic diseases, immunocompromised hosts, and adults with incomplete records experience substantial morbidity due to vaccine-preventable diseases. In Japan, adult vaccination remains fragmented due to incomplete records, distinctions between routine and voluntary vaccination, variable municipal subsidies, and li…

Out of Season and Out of Pattern: Winter Ehrlichiosis in a Heart Transplant Recipient With Possible Reactivation or Reinfection.

Human monocytic ehrlichiosis (HME) is a tick-borne infection caused by Ehrlichia chaffeensis, an obligate intracellular bacterium that infects monocytes and can lead to systemic inflammatory illness. Clinical manifestations typically include fever, malaise, cytopenias, transaminitis, and, in severe cases, neurologic or multiorgan involvement. Although HME is increasingly recognized in immunocompromised hosts, it remains uncommon in solid organ transplant recipients and is rarely reported outside peak seasonal exposure periods. In transplant patients, atypical presentations, baseline hematologi…

Neuroinvasive West Nile Encephalitis Presenting With Seizures and Acute Flaccid Paralysis: A Case Report.

West Nile encephalitis (WNE) is a serious neurological disorder caused by the West Nile virus (WNV), a mosquito-borne flavivirus. The virus is mainly transmitted to humans through bites from infected Culex mosquitoes, with birds acting as the primary reservoir hosts. While most WNV infections are asymptomatic or cause a mild febrile illness (West Nile fever), a small subset of cases progresses to severe neuroinvasive disease, such as encephalitis, meningitis, or acute flaccid paralysis. WNE is marked by brain inflammation, resulting in high fever, headache, neck stiffness, disorientation, trem

Pathogens transmitted by Ixodes ricinus.

Ixodes ricinus is the most important tick vector in central and western Europe and one of the most researched parasites. However, in the published literature on the tick and the pathogens it transmits, conjecture about specific transmission cycles and the clinical significance of certain microbes is not always clearly separated from confirmed facts. This article aims to present up-to-date, evidence-based information about the well-researched human pathogens tick-borne encephalitis virus, louping-ill virus, Anaplasma phagocytophilum, Borrelia burgdorferi sensu lato and several Babesia species,

A Possible New Diagnostic Method for Early Diagnosis of Cryptococcus Infection in Lymphoma Patient Co-Infected with SARS-CoV-2.

A Cryptococcus subspecies, neoformans, represents the most pathogenic infection for humans, particularly in immunocompromised hosts (e.g., cancer patients, drug users). In the present study, we described a 67-year old woman with non-Hodgkin lymphoma who developed an infectious disease sustained by Cryptococcus neoformans. Biochemical data documented a decrease in lymphocytes count while clinical evaluation was suggestive on meningeal infection. The microbiological analysis of the serum, using a dilution pattern through the CrAg lateral flow assay (Immy, Norman, OK 73069, USA) detected the anti…
