CD71+ erythroid cells as a potential early biomarker for hemodynamic significant patent ductus arteriosus in preterm infants.

INTRODUCTION: Hemodynamically significant patent ductus arteriosus (hsPDA) in preterm infants is associated with increased mortality and morbidity. Early and accurate identification of infants at risk for PDA is critical for improving outcome. Perinatal inflammation is a known risk factor of PDA. CD71+ erythroid cells (CECs) are immature red blood cells with immunomodulatory roles and show variable abundance in preterm infants. We aimed to examine the proportion of CECs in preterm infants and investigate association with PDA. METHODS: In this prospective study, we evaluated the association be

En caul cesarean section for extremely low birth weight infants: a single-center, retrospective study.

BACKGROUND: Extremely low birth weight (ELBW) infants, which weighing less than 1000 g, are at high risk of adverse neonatal outcomes, including intraventricular hemorrhage (IVH), respiratory distress syndrome, and long-term neurodevelopmental impairments. En caul cesarean section (ECCS) is often performed for delivery of ELBW infants to minimize potential damages to the skin, bones, and brain. Although a few studies reported that ECCS is a safe procedure for infants, there is limited research investigating the association between this procedure and neonatal IVH. This study aims to assess whet

Hydralazine in infants with persistent hypoxemic respiratory failure.

BACKGROUND: Most deaths of infants with chronic lung disease (CLD) are caused by respiratory failure, unremitting pulmonary artery hypertension (PAH) with cor pulmonale, or infection. Although the exact prevalence of PAH in infants with CLD is unknown, infants with CLD and severe PAH have a high mortality rate. Except for oxygen supplementation, no specific interventions have been established as effective in the treatment for PAH in premature infants with CLD. Little has been proven regarding the clinical efficacy of vasodilators and concerns remain regarding adverse effects. OBJECTIVES: To r

Antibody responses to vaccination among South African HIV-exposed and unexposed uninfected infants during the first 2 years of life.

HIV-exposed but uninfected (HEU) infants born to HIV-infected mothers from areas in the world with a high burden of infectious disease suffer higher infectious morbidity and mortality than their HIV unexposed uninfected (HUU) peers. Vaccination provides protection from infection. The possibility exists that altered response to vaccination contributes to the higher rate of infection in HEU than in HUU infants. While short-term, cross-sectional studies support this notion, it is unclear whether or not HEU infants develop long-term protective immune responses following the WHO extended program on

Post-neonatal mortality, morbidity, and developmental outcome after ultrasound-dated preterm birth in rural Malawi: a community-based cohort study.

BACKGROUND: Preterm birth is considered to be associated with an estimated 27% of neonatal deaths, the majority in resource-poor countries where rates of prematurity are high. There is no information on medium term outcomes after accurately determined preterm birth in such settings. METHODS AND FINDINGS: This community-based stratified cohort study conducted between May-December 2006 in Southern Malawi followed up 840 post-neonatal infants born to mothers who had received antenatal antibiotic prophylaxis/placebo in an attempt to reduce rates of preterm birth (APPLe trial ISRCTN84023116). Are outcomes and care processes for preterm neonates influenced by health insurance status?

OBJECTIVE: The purpose of this work was to compare the processes of care and to evaluate outcomes of premature neonates delivered to women with Medicaid managed care versus private insurance. DESIGN/METHODS: All of the infants born at <37 weeks' gestation between January 2001 and August 2005 in the ParadigmHealth database were included in these analyses (n = 24151). Infants were categorized by maternal health insurance status as private insurance or Medicaid managed care and analyzed for differences in demographic data and length of stay. For survivors, differences in respiratory care, nutrit

Liver transplantation in pediatric patients: twenty years of experience at the University of Wisconsin.

Developments in surgical technique, immunosuppression, organ procurement and preservation, and patient selection criteria have resulted in improved long-term patient and graft survival after pediatric liver transplantation. In this study, we examined the results of 196 liver transplants performed in 155 pediatric patients at University of Wisconsin Children's Hospital. Patients were divided into two groups according to age at the time of liver transplant. Infants under 12 months of age comprised Group 1 (n=74) and children from one to 18 yr comprised Group 2 (n=122). Outcomes for whole, reduce

Urinary S100B protein measurements: A tool for the early identification of hypoxic-ischemic encephalopathy in asphyxiated full-term infants.

OBJECTIVE: Hypoxic-ischemic encephalopathy (HIE) is one of the major causes of perinatal mortality and morbidity. To date, there are no reliable methods to detect which infants will develop brain damage after asphyxia insult. DESIGN AND SETTING: Prospective study conducted in three tertiary departments of neonatology from December 1999 to July 2002. PARTICIPANTS: A total of 44 infants with perinatal asphyxia and 68 control infants. INTERVENTION: Routine laboratory variables, neurologic patterns, ultrasound imaging, and urine concentrations of S100B protein were determined at nine time point

Systemic Candida infection in extremely low birth weight infants: short term morbidity and long term neurodevelopmental outcome.

OBJECTIVE: To describe mortality, morbidity at discharge and neurodevelopmental outcome at 2 years corrected age in extremely low birth weight infants with systemic Candida infection during intensive care stay. METHOD: We identified all extremely low birth weight (birth weight <1000 g) infants diagnosed with Candida sepsis and/or meningoencephalitis between 1988 and mid-1996 in the tertiary neonatal intensive care centers of Toronto. The outcome of the infected infants at discharge and at 2 years corrected age was compared with a cohort of 470 extremely low birth weight infants born between 1

Fetal thrombocytopenia and its relation to maternal thrombocytopenia.

BACKGROUND: Neonates with severe thrombocytopenia can have bleeding leading to death or lifelong residual defects. The predictors, frequency, and consequences of fetal thrombocytopenia are not known, nor is it known if there are maternal clinical features that could predict fetal thrombocytopenia. METHODS: We conducted a seven-year cross-sectional study in which platelet counts were determined in newborns' umbilical-cord blood and blood obtained from their mothers at consecutive deliveries in one obstetrical unit. The relations of the umbilical-cord platelet count to maternal risk factors wer

Right pulmonary artery originating from the ascending aorta with tetralogy of fallot and pulmonary atresia.

KEY CLINICAL MESSAGE: Anomalous origin of right pulmonary artery from the ascending aorta is a rare congenital heart malformation that results in early infant mortality. These patients are at risk for the early development of significant pulmonary hypertension. The surgical management during the early period of life is imperative. ABSTRACT: Anomalous pulmonary artery originating from the ascending aorta (often called hemitruncus) is a rare congenital cardiac defect requiring immediate management in the neonatal period. We report a case of a rare variant of anomalous right pulmonary artery ori…

A unique case of hepatopulmonary fusion in congenital diaphragmatic hernia causing pulmonary hypoplasia in a newborn: A case report study.

INTRODUCTION AND SIGNIFICANCE: Hepatopulmonary fusion is a rare congenital malformation associated with right-sided diaphragmatic hernia. Often diagnosed during surgery, it carries a high mortality rate. CASE PRESENTATION: We present a case of a full-term infant who exhibited shortness of breath from birth. Radiological examinations confirmed a right congenital diaphragmatic hernia, and subsequent surgery revealed hepatopulmonary fusion. CLINICAL DISCUSSION: While hepatopulmonary fusion is uncommon, it should be considered in the differential diagnosis of right-sided congenital diaphragmatic

Stenotrophomonas Necrotizing Tonsilitis in a Pediatric Patient: A Rare Presentation With Upper Airway Obstruction.

Stenotrophomonas maltophilia is notorious for its intrinsic resistance to many commonly used antibiotics, making it a particularly challenging pathogen to treat. It often causes severe opportunistic infections in immunocompromised and hospitalized patients. The potential for this infection to become fulminant with high mortality rates in both adults and children necessitates a multidisciplinary approach. Prompt initiation of appropriate antibiotic therapy can be lifesaving. In this case report, we present a rare instance involving an eight-month-old previously healthy infant diagnosed with nec

What's Old and New in Tuberculosis Vaccines for Children.

Tuberculosis (TB) is a leading cause of global child mortality. Until the turn of the 21st century, Mycobacterium bovis bacille Calmette-Guerin (BCG) was the only vaccine to prevent TB. The pediatric TB vaccine pipeline has advanced in the past decade to include the evaluation of novel whole cell vaccines to replace infant BCG and investigation of subunit and whole cell vaccines to boost TB immunity during adolescence. We describe the history of BCG, current TB vaccine candidates in clinical trials, and the challenges and opportunities for future TB vaccine research in children. Children are a

Considerations for COVID-19 vaccination in pregnancy.

Pregnant women are at greater risk of severe COVID-19 than non-pregnant women. Despite limited safety data on use of COVID-19 vaccines in pregnancy, many international societies have recommended their use when pregnant women are at particularly high risk of acquiring COVID-19, or have suggested that vaccines should not be withheld from pregnant women where no other contraindications to COVID-19 vaccination exist. A number of vaccines, including those against influenza, tetanus and pertussis, have been shown to reduce both maternal and infant morbidity and mortality when used antenatally. We ex

[Pediatric tuberculous peritonitis. Case report].

Tuberculous peritonitis is an uncommon entity in the infant population. It is an uncommon form of extrapulmonary tuberculosis and represents a very low percentage of all cases of tuberculosis. Its symptoms are nonspecific and usually manifesting with ascites, abdominal pain, fever and low weight. The delay in its diagnosis and treatment, originated by its form of presentation, can cause an increase in its morbidity and mortality. We report the case of a 14-year-old patient without concomitant disease or pulmonary tuberculosis, who presented with ascites and fever. Laparoscopy showed multiple n
