Preconception care of women with diabetes.

Preconception care in women with diabetes reduces the risk of spontaneous abortion and congenital malformations as a result of improved glycemic control before and during organogenesis. Prepregnancy planning encompasses optimizing glycemic control and also affording the opportunity for genetic counseling, contraceptive selection, management of diabetes complications, and evaluation of psychosocial aspects of pregnancy, childbearing, and diabetes care. The purpose of this article is to provide nurses with current clinical assessment and management strategies of women with diabetes in order to i

Examining multilevel resilience as a moderator of associations between gendered racial microaggressions and postpartum mental health and functioning.

The United States is currently facing a maternal health crisis, and mental health issues are a leading preventable cause of maternal mortality. We assessed the influence of resilience on the relationship between gendered racial microaggressions (GRM) and postpartum depressive symptoms and functioning. We analyzed data among 265 postpartum Global Majority participants across four Philadelphia and New York hospitals. We administered the adapted Gendered Racial Microaggressions Scale (α = 0.97) to participants before discharge. Postpartum depressive symptoms, postpartum functioning, and resilienc…

Advances in understanding the crosstalk between mother and fetus on iron utilization.

A full-term pregnancy comes with significant demand for iron. Not meeting this demand has adverse effects on maternal health and on the intrauterine and postnatal development of the infant. In the infant, some of these adverse effects cannot be reversed by postnatal iron supplementation, highlighting the need to tackle iron deficiency in utero. Achieving this requires sound understanding of the pathways that govern iron transfer at the fetomaternal interface. Two pathways are emerging as key players in this context; the hepcidin/ferroportin axis pathway and the iron regulatory protein (IRPs) p…

Long-term mortality in mothers of infants with neonatal abstinence syndrome: A population-based parallel-cohort study in England and Ontario, Canada.

BACKGROUND: Opioid addiction is a major public health threat to healthy life expectancy; however, little is known of long-term mortality for mothers with opioid use in pregnancy. Pregnancy and delivery care are opportunities to improve access to addiction and supportive services. Treating neonatal abstinence syndrome (NAS) as a marker of opioid use during pregnancy, this study reports long-term maternal mortality among mothers with a birth affected by NAS in relation to that of mothers without a NAS-affected birth in 2 high-prevalence jurisdictions, England and Ontario, Canada. METHODS AND FI

Identifying Pregnant Women With Disabilities and Maternal and Newborn Outcomes.

IMPORTANCE: Pregnant women with disabilities are at higher risk of poor pregnancy and birth outcomes. Different methods for identifying disability may affect estimates of health disparities in this population. OBJECTIVE: To compare pregnancy and birth outcomes among pregnant women using different ways of identifying maternal disability. DESIGN, SETTING, AND PARTICIPANTS: Retrospective cohort study of linked vital records and maternal and newborn claims for Medicaid-insured live births from January 2010 to December 2014 in Texas. Data analysis was conducted from October 2023 to May 2024. EXP

DESIGN: A prospective cohort study was conducted. Participants/Materials: The study enrolled 62 pregnant women with singleton breech presentations at or beyond 37 weeks of gestation who consented to undergo ECV. Decision and economic evaluation of abortion availability in the United States military.

BACKGROUND: Active-duty service women rely on the civilian sector for most abortion care due to limits on federal funding for abortion. Abortion is now banned in many states with large military presences. The Department of Defense has implemented policies to assist active-duty service women in accessing abortion, but there is debate to reverse this support. OBJECTIVE: Our goal was to compare the cost-effectiveness and incidence of adverse maternal and neonatal outcomes of a hypothetical cohort of active-duty service women living in abortion-restricted states comparing restricted abortion acce

Short Maternal Stature Increases Risk of Small-for-Gestational-Age and Preterm Births in Low- and Middle-Income Countries: Individual Participant Data Meta-Analysis and Population Attributable Fraction.

BACKGROUND: Small-for-gestational-age (SGA) and preterm births are associated with adverse health consequences, including neonatal and infant mortality, childhood undernutrition, and adulthood chronic disease. OBJECTIVES: The specific aims of this study were to estimate the association between short maternal stature and outcomes of SGA alone, preterm birth alone, or both, and to calculate the population attributable fraction of SGA and preterm birth associated with short maternal stature. METHODS: We conducted an individual participant data meta-analysis with the use of data sets from 12 pop

Healthcare-Seeking Behaviour for Obstetric Complications in Ethiopia: A Multilevel Mixed-Effect Analysis.

BACKGROUND: Healthcare-seeking behaviour, and its associated factors, for obstetric complications are an important determinant of maternal deaths and adverse foetal outcomes. However, there is limited evidence on healthcare-seeking behaviours from health facilities in response to obstetric complications among Ethiopian women. OBJECTIVE: To investigate women's healthcare-seeking behaviour in response to obstetric complications, and its associated factors, in Ethiopia. METHODS: Data were sourced from the Performance Monitoring for Action (PMA-E) longitudinal survey national database. Andersen'

Screening of attention deficit hyperactivity disorder among preschool children Gharbia Governorate, Egypt.

BACKGROUND: Children who experience mental health issues, such as attention deficit hyperactivity disorder (ADHD), experience significant distress and impairment at home, at school, and in the community. Maternal Anxiety and Breastfeeding Self-Efficacy in the Neonatal Intensive Care Unit: A Narrative Review of Their Relationship with Breastfeeding Outcomes.

Mothers of infants cared for in the neonatal intensive care unit (NICU) experience substantial psychological burden related to preterm birth, mother-infant separation, and stressors inherent to the intensive care environment. In this context, maternal anxiety and breastfeeding self-efficacy have been identified as key psychosocial factors influencing the initiation and continuation of breastfeeding. This narrative review examines the relationship between maternal anxiety and breastfeeding self-efficacy during the NICU stay and evaluates how this interaction is reflected in breastfeeding outcom
