Report of an HIV clinic-based pain management program and utilization of health status and health service by HIV patients.

CONTEXT: Current reports on human immunodeficiency virus (HIV) pain are limited to epidemiological data on neuropathic pain in HIV and most studies were conducted before the availability of highly active antiretroviral therapy. Complex pain was reported to be highly prevalent and associated with advanced disease. OBJECTIVES: The authors conducted a retrospective review of the medical records of 81 patients from the Center for Positive Living (CPL) at Montefiore Medical Center who were newly referred to a pain management program in 2006 to identify the potential benefits of integrating a pain

Strategies to Manage Refractory Hypoxemia in the Intubated Patient.

Persistent hypoxemia after intubation is associated with high mortality and demands a systematic approach focused on the underlying pathophysiology. This article emphasizes developing a focused differential diagnosis that includes both mechanical and patient factors to guide management. The authors review key therapeutic strategies, including careful use of positive end expiratory pressure, positioning techniques, medications such as inhaled pulmonary vasodilators, sedation, and neuromuscular blockade, and, when conventional therapies fail, consideration of extracorporeal membrane oxygenation.

New Strategies for Locally Advanced Breast Cancer: A Review of Inflammatory Breast Cancer and Nonresponders.

This review explores the new strategies around the management of locally advanced breast cancer (LABC), particularly for nonresponsive tumors and/or initially unresectable tumors at diagnosis, inclusive of inflammatory breast cancer. Nonresponders to neoadjuvant systemic therapy present a unique clinical challenge. Emerging medical therapeutics as well as considerations for use of radiotherapy and/or surgery in this setting are discussed. Specifically, the use of neoadjuvant radiotherapy for LABC and lymphedema prevention with lymphatic reconstruction following axillary lymph node dissection a

Botulinum Toxin for the Treatment of Intractable Raynaud Phenomenon.

Raynaud phenomenon (RP) is a condition causing vasospasm in the fingers and toes of patients that can have a significant negative impact on quality of life. This can lead to pain, ulceration, and possible loss of digits. Several pharmacological options are available for treatment. However, RP can often be refractory to traditional modalities, leaving surgery or injections as the next available options. This article provides a review and update on the use of botulinum toxin as an effective therapy for the treatment of RP refractory to medical management.

Mechanical Mitral Valve Replacement during the 2nd Trimester of Pregnancy.

We report the case of a 44-year-old pregnant woman who was diagnosed with symptomatic severe mitral stenosis that did not respond to optimal medical therapy and balloon valvuloplasty. After a multidisciplinary team discussion on the timing and risks of interventions and postoperative optimization of peripartum anticoagulation, the patient underwent mechanical mitral valve replacement during the 2nd trimester of pregnancy. The outcome was excellent for the mother and the infant. This case emphasizes the importance of a multidisciplinary approach in managing unusual cases.

[Peripheral arterial disease: Treatment].

Medical management of peripheral arterial disease (PAD) patients is aimed at limb symptom relief and reducing systemic major adverse events risk. For the first purpose: exercise therapy is recommended in case of claudication; multidisciplinary evaluation for surgical options is mandatory in case of critical limb ischaemia. Reducing cardiac and stroke risk can be achieved through: statin prescription in most of the cases; antiplatelet agents in symptomatic PAD patients; cardio-vascular risk factors control.

The Brugada syndrome.

Brugada syndrome is a recognized cause of sudden cardiac death worldwide. An inherited ion channel abnormality produces abnormal repolarization leading to characteristic ST-segment elevation in precordial leads V1 to V3 and a pseudo right bundle branch block on electrocardiogram. Only recently has medical therapy and management been defined to allow for athletes to lead healthy lifestyles. This article provides a concise review of the clinical manifestations, pathophysiology, and therapeutic options for the sports medicine team.

Medical management of chronic pelvic pain.

Chronic pelvic pain is a symptom of poorly understood aetiology. Pelvic congestion and psychological factors have been identified as important aetiological factors but surprisingly few studies of therapies directed at these aetiologies have been performed. A limited number of trials suggest that the progestagen medroxyprogesterone acetate may be useful in relieving symptoms, but benefit may be restricted to duration of treatment only. Further research is required into both the basis of chronic pelvic pain and its medical management.

[Use of echocardiographic examination for infective endocarditis].

Echocardiography, both transthoracic and transesophageal techniques, is the procedure of choice in the diagnosis and management of patients with endocarditis. First of all, vegetative lesions can be detected. Echocardiography also plays a major role in recognition of such complications of endocarditis as: perivalvular abscesses, aneurysms, fistulas, rupture of valvular leaflets, chordae and papillary muscles or the interventricular septum. It provides useful information on valve destruction, severity of insufficiency, prosthetic valve dysfunction and, finally, it provides a chance for optimal

Management of orthotopic heart transplant recipients at Brooke Army Medical Center.

Advances in heart transplantation and post-operative rejection therapy have given new hope to patients with end-stage congestive heart failure and cardiomyopathies. The growing numbers of pre- and post-transplant patients and the complexity of their care underscore the need for a multispecialty approach to patient management. This study examines the success of a multi-service program at Brooke Army Medical Center which offers pre- and post-transplant care to active duty, dependent, and retired military personnel.

Hemispheric supratentorial low-grade astrocytoma.

The records of 57 patients who were operated on at the Beilinson Medical Center from 1970 through 1982 for supratentorial low-grade astrocytomas were reviewed. The management of these patients was correlated with survival. Our study demonstrates that a younger age at onset and a negative radionuclide uptake on isotope scan, were the most important factors associated with a better survival rate. Extensive tumor resection combined with radiation therapy also exercised a beneficial effect in prolonging survival.

Rectal prolapse in infants and children.

Rectal prolapse that is intractable to the usual medical therapy was successfully managed without significant complications in 10 patients by simple subcutaneous encirclement of the anus with a heavy nonabsorbable suture, which was in place until the suture was removed or broke after 4 to 6 months. Four patients required two sutures and one needed a third insertion. Since this procedure is simple, has no serious complications, and controls rectal prolapse, it is recommended as the preferred initial surgical treatment of this condition.

Short-term intubation in children with acute epiglottitis.

Management of the airway in acute epiglottitis is still controversial. One argument against intubation as opposed to tracheostomy is the increased complications from prolonged intubation. We describe a study of 26 children treated for acute epiglottitis from 1971 to 1979. Included in this series are 16 consecutive patients who were treated by nasotracheal intubation and simultaneous medical therapy. Fifteen of the 16 patients extubated safely in less than 48 hours, significantly decreasing the possibility for complications.

Cut or Treat: Surgery or Medical Management of Patients with Crohn's Disease and Evidence of Acute Appendicitis.

The clinical presentation of Crohn's disease (CD) is heterogeneous, reflecting its involvement of any segment of the gastrointestinal tract. Acute right-lower-quadrant pain in CD can closely mimic appendicitis, creating diagnostic and therapeutic challenges. We report a 33-year-old man with established inflammatory ileocolonic CD in clinical remission on infliximab who presented with recurrent right-lower-quadrant pain and imaging findings consistent with acute appendicitis. Colonoscopy demonstrated only mild inflammatory changes without appendiceal orifice involvement. Though initial medical

Malignancy-Associated Hypercalcemia and Kidney Disease: Facts, Controversies, and Management.

Hypercalcemia can affect multiple organ systems and, depending on its severity, presents with a wide range of clinical manifestations. Severe hypercalcemia may lead to serious complications such as confusion and encephalopathy. Malignancy-associated hypercalcemia (MAH) occurs in up to 10%-30% of patients with cancer, with a higher incidence in those with advanced disease, and is associated with a poor prognosis. Several mechanisms have been proposed to explain MAH, the most prominent being humoral hypercalcemia of malignancy and osteolytic hypercalcemia. Although less common, recent advances h

Resolution of Distal Tracheal Obstruction Using Endoscopic-Guided Holmium:Yttrium-Aluminium-Garnet Laser Ablation in a Blue and Gold Macaw (Ara ararauna).

A 2-year-old, female, blue and gold macaw (Ara ararauna) presented with respiratory distress following a 1-week history of altered vocalization. The bird was anesthetized for a computed tomography scan, air sac cannulation, and tracheoscopy. The bird was diagnosed with a distal tracheal obstruction, occupying more than 80% of the tracheal lumen. Initial attempts at biopsy and resection were unsuccessful because of limited access to the obstruction. The bird was managed medically for 1 week with a combination of an oral antifungal medication, antibiotic drugs, a nonsteroidal anti-inflammatory a

Coccidioidomycosis as a Potential Trigger for Sarcoidosis: A Case Study.

This study explores the complex interplay between coccidioidomycosis (valley fever) and sarcoidosis through a detailed case study of a 54-year-old male patient. The patient presented with elevated calcium levels, chronic kidney disease (CKD), and unintended weight loss. Interdisciplinary collaboration between nephrologists and pulmonologists played a crucial role in navigating the intricate medical challenges, including hypercalcemia, renal dysfunction, and pulmonary anomalies. The diagnostic journey involved extensive laboratory findings uncovering the involvement of both infectious agents an

Current Concepts of the Uveitis-Glaucoma-Hyphema (UGH) Syndrome.

PURPOSE: To discuss the pathophysiology, etiology, and current management strategies of uveitis-glaucoma-hyphema (UGH) syndrome. METHODS: Literature review. RESULTS: The classic UGH syndrome associated with anterior chamber intraocular lenses (ACIOL) have decreased in incidence with the modernization of IOL design and surgical techniques. The current UGH syndrome is increasing in prevalence largely related to a parallel increase in late onset dislocations of intraocular lenses (IOLs) and the developing techniques to remedy that condition. The modern features of UGH can present as cystoid mac
