Is the decremental pattern in Lambert-Eaton syndrome different from that in myasthenia gravis?

OBJECTIVE: We reviewed our experience to determine if the decremental pattern during low frequency repetitive nerve stimulation (LF-RNS) distinguishes between the Lambert-Eaton myasthenic syndrome (LEMS) and myasthenia gravis (MG). METHODS: LF-RNS studies were reviewed from 34 LEMS and 44 MG patients, 4 of whom had antibodies to muscle specific kinase (MuSK). In each train we calculated the ratio between the early and the later decrement. Receiver-operator characteristic curves were calculated to determine the ratio that best distinguished between LEMS and MG. RESULTS: The late decrement was…

Paraneoplastic syndromes of the neuromuscular junction: therapeutic options in myasthenia gravis, lambert-eaton myasthenic syndrome, and neuromyotonia.

Myasthenia gravis (MG), Lambert-Eaton myasthenic syndrome (LEMS) and neuromyotonia are neuromuscular transmission disorders occurring with or without associated malignancy. Due to the common antibody-mediated pathophysiology, immunosuppression has an important role in the treatment of each of these disorders. Symptomatic treatment is more variable. Pyridostigmine is first-line treatment in generalized MG. Response seems to be better in patients with acetylcholine receptor (AChR) antibodies than in patients with antibodies against muscle-specific tyrosine kinase (MuSK). Pyridostigmine can be su…

[Paraneoplastic phenomena in patients with a thymoma].

A thymoma arises from the epithelial cells of the thymus. Local tumour growth may cause symptoms like coughing, dyspnoea or chest pain. Paraneoplastic phenomena can also occur in patients with a thymoma; myasthenia gravis is a well-known example. Other neurological, dermatological, cardiological and haematological disorders are not always recognised as being paraneoplastic phenomena. There is no clear relationship between tumour activity and the clinical course of paraneoplastic phenomena. The three cases in this article illustrate how the clinical presentation of these phenomena can vary.

T-lymphocyte-rich thymoma and myasthenia gravis in a Siberian tiger (Panthera tigris altaica).

A 10-year-old captive male Siberian tiger (Panthera tigris altaica) presented with acute onset collapse, vomiting and dyspnoea, preceded by a 6-month period of progressive muscle wasting. Following humane destruction, post-mortem examination revealed a large multilobulated mass in the cranial mediastinum, which was diagnosed as a T-lymphocyte-rich thymoma with the aid of immunohistochemistry. Retrospective serology for acetylcholine receptor antibodies (titre 3.90 nmol/l) confirmed a diagnosis of thymoma-associated myasthenia gravis. Thymomas are reported rarely in wild carnivores, but when de…

Altered expression of miR-146a in myasthenia gravis.

The purpose of this study was to analyze the expression of miR-146a in PBMCs obtained from patients with myasthenia gravis (MG) and healthy controls and to investigate the effect of the inhibition of miR-146a on the activation of AchR specific B cells obtained from mice. The expression of miR-146a levels in PBMCs obtained from patients with MG and healthy controls were determined by qRT-PCR. MiR-146a's complementary fragment, AntagomiR-146a, was synthesized as inhibitor, and the nonfunctional fragment, which has similar construction to AntagomiR-146a, was synthesized as negative control inhibi…

Port-site recurrence after complete resection of stage I thymoma by video-assisted thoracoscopic surgery: report of a case.

A 62-year-old female with myasthenia gravis and a mediastinal tumor underwent extended thymothymectomy by video-assisted thoracoscopic surgery (VATS). The tumor was diagnosed as a stage I type B2 thymoma. Eight years after the initial surgery, an abnormal shadow was detected on the annual follow-up chest radiograph, and computed tomography showed a homogeneous 35 × 15 mm hemispherical mass under a port-site scar in the right anterior chest wall. The mass was between the second and third ribs and was found to be adherent to the lung during surgery, and was excised via lung and chest wall resect…

[Autoantibody against the presynaptic P/Q-type voltage-gated calcium channel in Lambert-Eaton myasthenic syndrome].

Antibodies against the muscle acetylcholine receptor (AChR) were recognized as the cause of myasthenia gravis in the 1970s'. Since then, other neurological disorders associated with autoantibodies have been identified, each associated with an antibody against a ligand- or voltage-gated ion channel. Autoantibodies against P/Q-type voltage-gated calcium channels (VGCCs) are detected in patients with Lambert-Eaton myasthenic syndrome (LEMS). These antibodies interfere with the calcium-dependent release of acetylcholine from the presynaptic membrane. LEMS is an autoimmune disorder affecting the ne…

The coexistence of myasthenia gravis and myotonic dystrophy type 2 in a single patient.

BACKGROUND: Myasthenia gravis (MG) and myotonic dystrophy type 2 (DM2) are rare disorders individually, and their coexistence in the same patient is very rare. We present a patient in which these two diseases coexisted. CASE REPORT: The patient complained of diplopia, fluctuating limb weakness, and difficulties in swallowing and speaking. A neurological examination revealed diplopia, facial, weakness of the neck and proximal limb muscles, dysphagia, dysphonia, and myotonia. The patient's mother had DM2 and her maternal grandfather had cataracts. MG was confirmed in our patient by positive res…

[Drugs that may trigger or exacerbate myasthenia gravis].

Myasthenia gravis (MG) is an autoimmune disease causing muscle weakness due to impaired transmission at the neuromuscular junction. MG or a MG-like condition may be triggered or exacerbated by several drugs used for treatment of other diseases. Drugs may interfere with the neuromuscular transmission through several mechanisms, either by affecting pre- or postsynaptic ion channels or by affecting acetylcholinesterase. Based on a literature search in PubMed and the authors' own clinical experiences, we provide an overview focusing on the most frequently used drugs that may exacerbate weakness in…

[Myasthenia, from the internist's point of view].

Myasthenia gravis is an autoimmune disease due to specific antibodies inducing a neuromuscular transmission defect causing muscle fatigability. If onset of the disease may be at any age, myasthenia gravis concerns mostly young adults, in majority females. The disease characteristic features are the following: ocular symptoms (ptosis or diplopia) as main initial manifestation, extension to other muscles in 80 % of the cases, variability of the deficit, effort induced worsening, successive periods of exacerbation during the disease course, severity depending on respiratory and swallowing impairm…

Arecoline inhibits and destabilizes agrin-induced acetylcholine receptor cluster formation in C2C12 myotubes.

Areca nut (Areca catechu) is chewed as a medical and psychoactive food by roughly 10% of the world population. Areca nut chewing may lead to low birth weight, premature delivery and impaired muscle development. Our previous study showed that arecoline, a major alkaloid in the areca nut, inhibited the myogenic differentiation of C2C12 myoblastic cells. The clustering of acetylcholine receptors (AChRs) in the postsynaptic membrane at the neuromuscular junction (NMJ) by agrin, a signaling protein released by motor neurons, is critical for the development of functional muscles. Here, we further in

Bilateral pneumothoraces following a right subclavian catheter insertion after thymectomy for a patient with a myasthenic crisis.

BACKGROUND: Myasthenia gravis (MG) is an autoimmune disease involving the formation of antibodies against the nicotinic acetylcholine receptors. Thymectomy is the treatment in MG patients with thymoma. We report a case of an MG patient who developed postthymectomy bilateral pneumothoraces after the placement of a subclavian central venous catheter. CASE REPORT: The 21-year-old patient with MG underwent a thymectomy and, in a later admission, complained of myasthenic crisis symptoms. He was scheduled to receive plasma exchange therapy and electromyography the following day. Plasmapheresis was …

Neuro-ophthalmologic side-effects of systemic medications.

PURPOSE OF REVIEW: Systemic medications may cause side-effects manifesting primarily as neuro-ophthalmologic problems. It is paramount for the physician to be updated on both well recognized and novel associations between drugs and their potential adverse reactions. RECENT FINDINGS: There is a growing list of medications that can cause pupil dilation, pupil constriction, dyschromatopsia, worsening of ocular myasthenia gravis, posterior reversible leukoencephalopathy syndrome, pseudotumor cerebri, disturbances in eye movements, accommodation problems, or optic neuropathy. This is partly due to…

Myasthenia gravis and pregnancy.

Myasthenia gravis is an autoimmune disease of the neuromuscular junction characterized by painless fluctuating skeletal muscle weakness. Disease exacerbations are more likely to occur in the first trimester or puerperium. A number of medications commonly used in obstetric practice can exacerbate the disease. The effect of pregnancy on myasthenia varies substantially from woman to woman and also from pregnancy to pregnancy in the same woman. Mainstay treatments involve acetylcholine esterase inhibitors, corticosteroids and other immunosuppressants, and adequate rest. Newborns may suffer in uter…

MuSK-Ab positive myasthenia: not always grave.

Antibodies (Abs) to muscle-specific tyrosine kinase (MuSK) are detected in approximately 40% of generalized acetylcholine receptor antibody-negative myasthenia gravis (MG). Anti-MuSK Abs are nearly always associated with generalized symptoms, with prevalent involvement of craniobulbar, cervical and respiratory muscles and with a striking preponderance in women. The typical course of MuSK-MG is acute onset, rapid progression, brittle course in the first years, early respiratory crises and unprovoked relapses in spite of high-dose immunosuppression. Patients often require long-term management wi…

Muscle-specific kinase antibodies: a novel cause of peripheral nerve hyperexcitability?

INTRODUCTION: Antibodies that target the postsynaptic neuromuscular junction (NMJ) protein, muscle-specific kinase (MuSK), have been associated with myasthenia gravis (MG), often with cramps and fasciculations, after administration of acetylcholinesterase inhibitors (AChE-I). METHODS: In this report, 2 patients are described with elevated MuSK antibodies and evidence of peripheral nerve hyperexcitability (PNH) unrelated to AChE-I medication. RESULTS: Patient 1 presented with facial neuromyotonia and fasciculations, without overt weakness. EMG studies demonstrated myokymic discharges in facia…
