Water content and other aspects of brittle versus normal fingernails.

BACKGROUND: Previous authors have claimed that dehydration of the nail plate causes brittle nails. Some experts claim that normal nails contain 18% water, and brittle nails contain less than 16%. OBJECTIVE: We sought to test the hypothesis that brittle nails contain 2% less water than normal nails. We also examined the relationship between a number of health and behavioral variables and brittle nails. METHODS: In all, 102 participants with either brittle or normal nails had two nails clipped and then analyzed for water content by a blinded investigator in the laboratory. Participants filled …

Nail disorders in pediatric patients: Part 1. Clinical overview, diagnosis and management of physiological variants, and congenital, hereditary, and infectious etiologies.

Nail disorders in children are relatively common and anxiety-provoking for families, yet frequently underrecognized in clinical practice. Many nail disorders in children require no treatment or are readably treatable; however, unfamiliarity may lead to unnecessary, inappropriate, or delayed treatment. In this part 1 of a 2-part continuing medical education series we review physiological variants and congenital, hereditary, and infectious nail disorders, and provide diagnostic and management tips for dermatologists managing these conditions.

Restoration of Radial Bowing After Elastic Stable Intramedullary Nailing in Pediatric Forearm Fractures: The Importance of Bowing Location in Addition to Magnitude.

Background/Objectives: Elastic stable intramedullary nailing (ESIN) is widely used for the treatment of pediatric diaphyseal forearm fractures. Although fracture union is commonly achieved, restoration of physiological radial bowing is essential for optimal forearm biomechanics. Most previous studies have focused on bowing magnitude, whereas the anatomical location of maximal radial bowing has received limited attention. Methods: This retrospective observational study included pediatric patients aged 4-16 years who underwent ESIN for diaphyseal radius and ulna fractures and had a minimum follo

Proximal forearm antegrade elastic stable intramedullary nailing for pediatric distal radius metaphyseal-diaphyseal junction fractures: A modified technique.

BACKGROUND: Distal radius metaphyseal-diaphyseal junction (MDJ) fractures in children are technically demanding, and the optimal fixation strategy remains controversial. The clinical performance of proximal-forearm antegrade elastic stable intramedullary nailing (A-ESIN) was compared with conventional retrograde crossed Kirschner-wire fixation (R-KW). METHODS: This retrospective study included 79 children treated for distal radius MDJ fractures at [blinded for review] between January 2018 and April 2024, with a minimum follow-up of 12 months. Thirty-eight underwent A-ESIN and 41 received R-KW

Anterior clamp-assisted reduction for retrograde nailing of infra-isthmic femoral fractures: technique and preliminary results on a short case series.

Infra-isthmal femoral fractures in elderly patients are difficult to stabilize due to the broad metaphyseal anatomy. In the absence of consensus on optimal management, this study assesses a minimally invasive technique combining anterior clamp-assisted reduction with retrograde intramedullary nailing. A retrospective case series was conducted on ten patients treated between January 2023 and April 2024. All underwent percutaneous fracture reduction via a limited anterior approach, followed by retrograde nailing. The mean patient age was 84.9 ± 6.2 years. All fractures were caused by low-energy

Association between time to weight bearing and rates of nonunion, delayed union, and malunion after intramedullary nailing of tibial shaft fractures.

INTRODUCTION: Intermedullary nailing (IMN) is the gold standard for tibial shaft fractures in adults with the goal of allowing early weight bearing. The purpose of this study was to determine the association between time to weight bearing as tolerated (WBAT) after tibial shaft IMN fixation and fracture nonunion, delayed union, and tibial deformity at final follow-up. METHODS: A retrospective cohort of patients who sustained extra-articular tibial shaft fractures treated with IMN at two level 1 trauma centers were collected from 2010 to 2021. A minimum follow-up of six months was required unle

Retrograde Nailing of an Unstable Distal-Third Femoral Shaft Fracture Using Pure Indirect Reduction: A Case Report in Osteoporotic Bone.

INTRODUCTION: Distal-third femoral shaft fractures in the elderly are rarely straightforward. Reduction and fixation are challenging due to … ed by 5 months. DISCUSSION: Most reports on distal-third femoral fractures treated with retrograde nails describe the use of some form of mechanical assistance to control reduction, especially in osteoporotic bone or long-oblique patterns. The concern is always the same - loss of alignment during reaming. In this case, reduction was maintained by payin …

Short segment coronal plane stability screws in retrograde nailing of proximal femoral shaft fractures: a technical trick and case series.

Retrograde intramedullary nailing is commonly used for fixation of femoral shaft fractures due to high healing rates and ease of placement. However, for proximal femur fractures, there is concern that the limited distance between the fracture and proximal interlock fixation may predispose these fractures to varus deformity and malunion. Coronal plane stability screws placed just adjacent to the outer diameter of the intramedullary nail can be used to counteract this coronal plane deformity but can be difficult to place proximally due to limitations from body habitus and difficulty with fluoros
