The monocyte-to-osteoclast transition in rheumatoid arthritis: Recent findings.

Rheumatoid arthritis (RA) is an autoimmune disease characterized by joint inflammation leading to joint destruction and deformity. The crucial role of osteoclasts in the bone erosion in RA has been demonstrated. Deregulated osteoclastogenesis which is affected by environmental factors including the inflammatory state, as well as genetic and epigenetic factors, is one of hallmarks of RA pathogenesis. An enhanced-monocyte-to-osteoclast transition plays an important role in osteoclast upregulation in RA because under specific stimuli, circulating monocytes might migrate to a specific location in

The thumb ossification composite index is the optimal intersection between Sanders and low-dose scoliosis sterioradiography.

PURPOSE: Skeletal maturity assessment may be performed using low-dose scoliosis radiographs (LDSS) with simultaneous imaging of the hand or proximal humerus. We sought to compare the practicality, reliability and validity of the Sanders skeletal maturity staging (SMSS), proximal humerus ossification system (PHOS) and the thumb ossification composite index (TOCI) as skeletal maturity assessment tools using LDSS. METHODS: A survey including 85 LDSS and 42 hand radiographs was administered to four orthopedic clinicians. Each rater assessed the TOCI, SMSS and PHOS stage for each image. Standing L

Ex vivo study of detergent-assisted intraosseous bone wash treatment of osteonecrosis.

Avascular necrosis (AVN) involves ischemic cell death of the bone. AVN leaves an abundance of necrotic lipids and debris in the bone marrow, … val of nonsoluble debris from the bone marrow space than the saline wash without causing changes to osteogenesis or inflammatory reactions.

N-acetylcysteine (NAC) differentially affects arterial medial calcification and bone formation: The role of l-cysteine and hydrogen sulphide.

Arterial medial calcification (AMC) is the deposition of calcium phosphate in the arteries. AMC is widely thought to share similarities with physiological bone formation; however, emerging evidence suggests several key differences between these processes. N-acetylcysteine (NAC) displays antioxidant properties and can generate hydrogen sulphide (H2 S) and glutathione (GSH) from its deacetylation to l-cysteine. This study found that NAC exerts divergent effects in vitro, increasing osteoblast differentiation and bone formation by up to 5.5-fold but reducing vascular smooth muscle cell (VSMC) cal

Introduction to the focus session on posterior vault surgery in craniosynostosis.

Posterior cranial vault remodeling is a surgical procedure that is currently utilized in all craniofacial centers. It has a dual aim: a cosmetic and a functional one to correct the posterior flattening of the head and creating space for the underlying brain to develop. With the increasing use of the procedure in the last years, numerous variants of the technique have been developed, new different hardware have been introduced (distractors and springs), and new potential functional advantages are discussed, namely, those related to the cerebrovascular circulation and the cerebrospinal fluid mov

Maxillary Changes Following Facial Bipartition - A Three-Dimensional Quantification.

INTRODUCTION: Children with Apert syndrome have hypertelorism and midfacial hypoplasia, which can be treated with facial bipartition (FB), often aided by rigid external distraction. The technique involves a midline osteotomy that lateralizes the maxillary segments, resulting in posterior cross-bites and midline diastema. Varying degrees of spontaneous realignment of the dental arches occurs postoperatively. This study aims to quantify these movements and assess whether they occur as part of a wider skeletal relapse or as dental compensation. METHODS: Patients who underwent FB and had high qua

Controllable ligand spacing stimulates cellular mechanotransduction and promotes stem cell osteogenic differentiation on soft hydrogels.

Hydrogels with tunable mechanical properties have provided a tremendous opportunity to regulate stem cell differentiation. Hydrogels with osteoid (about 30-40 kPa) or higher stiffness are usually required to induce the osteogenic differentiation of mesenchymal stem cells (MSCs). It is yet difficult to achieve the same differentiation on very soft hydrogels, because of low environmental mechanical stimuli and restricted cellular mechanotransduction. Here, we modulate cellular spatial sensing of integrin-adhesive ligands via quasi-hexagonally arranged nanopatterns to promote cell mechanosensing

Au Nanoparticles Attenuate RANKL-Induced Osteoclastogenesis by Suppressing Pre-Osteoclast Fusion.

Osteoclasts are multinucleated terminal cells that originate from a hematopoietic monocyte/macrophage lineage. Excessive osteoclast formation in vivo can lead to bone metabolic diseases such as postmenopausal osteoporosis, multiple myeloma, rheumatoid arthritis, and lytic bone metastases of cancer cells. Au nanoparticles (AuNPs) are inorganic nanoparticles with outstanding biocompatibility. We assessed their effect on osteoclastogenesis and found that pre-osteoclast fusion induced by receptor activator of nuclear factor kappa-B ligand (RANKL) and macrophage colonystimulating factor (M-CSF) was

The Gradual and Acute Correction of Equinus Using External Fixation.

Equinus is one of the most common deformities noted in foot and ankle biomechanics that, at times, if not identified and managed properly may lead to significant lower extremity pathology. With that being said, this deformity is also one that may be both underdiagnosed and undertreated. Treatment for equinus can range from conservative therapy to more aggressive surgical therapy. The purpose of this article is to review the clinical workup to properly identify the deformity and to explore the various treatment options for its timely management, which include gradual or acute correction of equi

Novel systems for the application of isolated tensile, compressive, and shearing stimulation of distraction callus tissue.

BACKGROUND: Distraction osteogenesis is a procedure widely used for the correction of large bone defects. However, a high complication rate persists, likely due to insufficient stability during maturation. Numerical fracture healing models predict bone regeneration under different mechanical conditions allowing fixation stiffness optimization. However, most models apply a linear elastic material law inappropriate for the transient stresses/strains present during limb lengthening or segment transport. They are also often validated using in vivo osteotomy models lacking precise mechanical regula…

Syndromic Craniosynostosis.

Syndromic craniosynostosis affects up to 1:30,000 live births with characteristic craniofacial growth restrictions, deformities, and other associated abnormalities, such as carpal-pedal anomalies and cognitive function impairment. More than 150 syndromes are associated with craniosynostosis. This article describes some commonalities and distinguishing features and management of syndromic synostosis. Also addressed is secondary synostosis, which is often found in syndromic children with problems related to microcephaly, hydrocephalus, or shunt-induced craniosynostosis, although pathophysiologic

[CHANGES OF THE SUPERFICIAL PERONEAL NERVE IN RELATION TO THE FREQUENCY OF HIGH-FRACTIONAL AUTODISTRACTION IN ORTHOPEDIC LENGTHENING OF THE TIBIA IN DOGS WITH A DAILY RATE OF 3 mm].

The experiments on the elongation of the tibia in 18 dogs were performed for 10 days using the Ilizarov apparatus with an automatic drive providing the pace of 3 mm/day for 120 moves (group 1) and 180 moves (group 2). The impact of fractionation of distraction on the structure of the peroneal nerve was estimated. In group 1, perineurium micro-injuries and endoneural blood vessel destruction were detected. In group 2, perineurium preserved its integrity and fine-lamellar structure, endoneural blood vessels were also preserved, however, the decrease in the total area of nerve fiber bundles was m

Bone formation in axial spondyloarthritis.

The success of targeted therapies directed against tumor necrosis factor for patients with spondyloarthritis has shifted the focus of physicians and scientists towards the prevention of structural damage to the involved structures, in particular the sacroiliac joints and the spine, to avoid loss of function and disability. Structural damage to the skeleton as witnessed by radiography mainly consists of new bone formation potentially progressively leading to spine or joint ankylosis. This important long-term outcome parameter has been difficult to study, not alone because the time window for ch

[Total arthroplasty: the other surfaces; wear and tear and osteolysis].

Total replacement remains a successful solution for degenerative, inflammatory and postraumatic diseases in large joints. This treatment has considerably improved the quality of life of the patients. Despite recent innovations in biomaterials, implant design and surgical technique, there are still failures, most of them related to wear at the surface of some component. Particles are generated that produce inflammatoty and osteolytic reactions leading to implant loosening. In that case, revision surgery is mandatory to remove and substitute the worn component. The operation is far more difficul

Mechanotransduction in bone repair and regeneration.

Mechanotransduction is the process by which physical forces are converted into biochemical signals that are then integrated into cellular responses. It plays a crucial role in bone repair and regeneration and thus has attracted a great deal of interest from researchers in various fields. This report reviews the current clinical evidence that shows the role mechanotransduction plays in bone processes such as physical adaptation, pathological fracture healing, and therapeutic distraction osteogenesis. We also outline the progress that has been made in understanding bone mechanotransduction from …

Physiological periostitis in a 2.5-month-old baby.

Periosteal reaction has different etiologies in early infancy. Physiological periostitis is a well-documented X-ray finding seen in both preterm and term babies aged between 1-6 months and can easily be misdiagnosed as child abuse or pathological periostitis. Here, we present a 2.5-month-old infant admitted with a history of fever, swollen right upper arm after vaccination and X-rays findings revealing periosteal reactions on both sides of the humeri, radii, tibiae and femora. Initial diagnosis was child abuse or congenital syphilis. Due to the normal physical findings and normal serological-b
