Childhood bone tuberculosis from Roman Pécs, Hungary.

A child from a Roman necropolis in Pécs, Hungary (4th century CE) was initially diagnosed with severe spinal osteomyelitis. The post-cranial skeleton displayed bone alterations in the lower thoracic and upper lumbar segments, including vertebral body destruction, collapse and sharp kyphosis, and additional multiple rib lesions, suggesting a most likely diagnosis of pulmonary and spinal tuberculosis. This study discusses a number of selected diagnoses in the context of our pathological findings, complementing the macroscopic examination with radiological and biomolecular analyses.

[The complications of laser surgery for the treatment of chronic vasomotor rhinitis].

The objective of the present study was to reveal cases of complications developing after laser coagulation of inferior turbinated bones. This treatment was given to 322 patients presenting with vasomotor rhinitis. A surgical ND:YAG laser with a quartz light guide 400 mcm in diameter was used for the purpose. In the majority of the cases, a single irradiation session produced a persistent positive effect, both functional and morphological. At the same time, two patients developed an unusual complication in the form of osteomyelitis of an inferior turbinated bone with its sequestration.

Treatment of Pott's Puffy tumor with balloon sinuplasty: report of three cases.

Pott's Puffy tumor (PPT) is a rare entity characterized by subperiosteal abscess associated with osteomyelitis of the frontal bone. It is usually managed by surgical curettage of the osteomyelitic bone and long-term antibiotic therapy. Balloon catheter dilatation is a new technique which was recently introduced for the treatment of chronic rhinosinusitis. In this article, we present three PPT cases (two of them were secondary to endoscopic sinus surgery) who were successfully treated with balloon catheter dilatation and long-term antibiotherapy.

Foreign body granuloma: a diagnosis not to forget.

Penetrating wounds of the foot are not uncommon. Many are caused by thorns or by fragments of wood that are retained in the foot, creating a foreign-body granuloma. The differential diagnosis for bony reaction to an unrecognised organic foreign body includes osteoid osteoma, chronic and acute osteomyelitis, tuberculosis granuloma, bone cyst, aneurysmal bone cyst, cortical fibrous defect, and neoplasm. We report the case of a boy suffering from a thorn inducing a lytic lesion of the fifth metatarsal that demonstrates the diagnosis difficulties of foreign body granuloma.

Spectrum of radiological manifestations of paediatric cat-scratch disease.

Cat-scratch disease (CSD) is usually a self-limiting condition that may mimic more serious disorders due to its variable clinical and imaging presentations. Many cases are diagnosed throughout the United States annually, with the majority occurring in children and adolescents who have had contact with a cat, usually in the form of a scratch or bite. This manuscript will discuss the spectrum of radiologic manifestations of CSD emphasising key imaging findings of lymphadenopathy, hepatosplenic lesions, osteomyelitis, discitis, encephalitis, neuroretinitis and cranial neuritis.

Management of Marjolin's ulcer in a chronic pressure sore secondary to paraplegia: a radical surgical solution.

Marjolin's ulcer refers to malignant degeneration in a chronic wound. Although originally described in an area of burns scar, many other chronic wounds such as osteomyelitis sinus tracts, venous stasis ulcers and chronic pressure sores have the potential to undergo malignant transformation. We present an interesting case of malignant degeneration in a male paraplegic patient with chronic sacral and ischial pressure sores. By discussing our radical surgical solution to this problem, we aim to highlight the importance of prompt diagnosis.

Tuberculosis of the hand.

There has been recent interest in tuberculosis of the hand because of a rising incidence owing to increasing numbers of immigration, an aging population, and immunosuppressed people including affected patients with human immunodeficiency virus. In this article, we review the epidemiology, bacteriology, pathophysiology, diagnosis, and principles of treatment of tuberculosis of the hand. The second part of the report emphasizes the classification of hand tuberculosis (cutaneous lesions, tenosynovitis, bursitis, osteomyelitis, arthritis, and tuberculous hypersensitivity reactions) along with the …

Involvement of methicillin-susceptible Staphylococcus aureus related to sequence type 25 and harboring pvl genes in a case of carotid cavernous fistula after community-associated sepsis.

Staphylococcus aureus encoding Panton-Valentine leukocidin (PVL) genes has become the cause of life-threatening infections. We describe a case of carotid cavernous fistula after bacteremia in a 12-year-old male, caused by a methicillin-susceptible S. aureus isolate carrying the pvl, fnbA, and ebpS genes and related to sequence type 25 (ST25). The patient's condition was complicated by pleural empyema and osteomyelitis in the right femur. The patient was discharged in good clinical condition after 160 days of hospitalization.

Hypercalcemia complicating Marjolin's ulcer.

A 54-year-old male with chronic osteomyelitis presented with new-onset hypercalcemia. Extensive initial evaluation did not reveal the underlying cause of his hypercalcemia. Subsequent biopsies revealed squamous cell carcinoma transformation consistent with Marjolin's ulcer. Timely surgery had been shown to be effective in achieving remission in patients with Marjolin's ulcers, even in the presence of hypercalcemia. Therefore, we recommend that patients with chronic wounds and new-onset or otherwise unexplained hypercalcemia should be evaluated for development of Marjolin's ulcers.

Imaging of the Charcot foot.

Charcot foot is a serious problem that causes considerable morbidity and may lead to limb loss. Arriving at a definitive diagnosis can be challenging. Given the progressive, destructive nature of Charcot's arthropathy, this delay can result advancing deformity, ulceration, infection, and place the limb at risk for amputation. Although clinical signs and symptoms and historical information are crucial, this article focuses on the imaging modalities that can aid practitioners in arriving at an early diagnosis, and how to differentiate Charcot's arthropathy from osteomyelitis of the feet.

Reversible oxacillin-associated hepatitis in a 9-month-old boy.

Herein we present a case of a 9-month-old boy admitted to our hospital for acute femoral osteomyelitis and hip septic arthritis treatment. Acute hepatitis with concomitant skin rash developed on the sixth day of intravenous oxacillin therapy. No other known virus- or drug-related skin eruption or hepatitis was suspected. The boy recovered completely after antibiotics were shifted to teicoplanin, and no sequelae were noted. To our knowledge, this boy is the youngest case of reported oxacillin-related hepatitis in the literatures.

Skeletal infections in cirrhotics.

There are few reports of skeletal infections in patients with cirrhosis. We present two such cases, both with alcoholic liver disease, seen over a period of one year. The first, a 46-year-old man, presented as pyrexia of unknown origin, and was found to have pyogenic discitis; he responded to antibiotic and surgery. The second, a 42-year-old man, presented with chest wall abscess and was diagnosed to have tubercular osteomyelitis; he expired despite treatment with non-hepatotoxic anti-tubercular drugs.

Pyogranulomatous myocarditis due to Staphylococcus aureus septicaemia in two harbour porpoises (Phocoena phocoena).

Staphylococcus aureus septicaemia was diagnosed in a dead, stranded harbour porpoise from the German Baltic Sea and in a live harbour porpoise by-caught in inner Danish waters and taken into captivity. Lesions included pyogranulomatous myocarditis, necrotising suppurative bronchopneumonia, pyelonephritis, osteomyelitis and leptomeningitis, and abscesses in lymph nodes and skeletal muscles. The captive animal had fibrinous suppurative epicarditis and pyogranulomatous myocarditis with abscesses. In both animals the organism was suspected to have entered through skin lesions or via the respirator…

Neonatal psoas pyomyositis simulating pyarthrosis of the hip.

Osteomyelitis with concomitant pyarthrosis or isolated pyarthrosis heads the differential list for a newborn with limb disuse and a flexion deformity of the hip. The diagnosis of psoas abscess, a primary suppurative myositis, should additionally be entertained. Stronger consideration should be given to this clinical entity after pyarthrosis of the hip has been excluded by an arthrocentesis performed under fluoroscopy. Radiologic imaging may facilitate the diagnosis. A blood culture may yield the organism responsible for the skeletal muscle abscess, even in nontoxic patients.

[Rehabilitation of invalids with sequelae and late complications of gunshot wound of the locomotor system].

The authors have generalized their experience in treating 2178 invalids of the Great Patriotic War with consequences of gunshot injuries and diseases of the locomotor system. 49.24% of the patients had chronic gunshot osteomyelitis, 36.38% had diseases of stumps of the extremities, 6.5% had false joints and 7.81% had ulcerated cicatrices and trophic ulcers. The authors give examples of late complications of the consequences of the gunshot injuries, changes in the morbidity structure among the invalids in the recent years, associated diseases and methods of their treatment and prevention.

[Complications following surgery for female urinary incontinence].

The two competitive methods available for operative treatment of female stress incontinence are the suspension procedures according to Marshall-Marchetti and according to Burch. With respect to continence, equally favorable results are achieved using both dethods. One way of assessing their value is by measuring the complications involved. A report is presented on osteomyelitis of the pubic symphysis in two cases after the Marshall-Marchetti operation and hydronephrosis in one case after the Burch operation. In all three patients, the complications were accompanied by postoperative fever as an…

Plasma cell sclerosis of bone: a spectrum of disease.

We describe the clinical, radiographic, and pathologic manifestations of a group of disorders characterized by osteosclerosis in association with plasmacytic infiltration of bone marrow. These conditions include multiple myeloma, plasma cell granuloma, sternocostoclavicular hyperostosis, the POEMS syndrome (plasma cell dyscrasia with polyneuropathy, organomegaly, endocrinopathy, M-protein, and skin changes), and chronic symmetric plasma cell osteomyelitis of childhood. Although clinically unrelated in many respects, features shared by these diseases support the existence of a specific factor l…

[Endocarditis due to Pseudomonas aeruginosa (author's transl)].

A case of pseudomonas endocarditis of biliary origin with impairment of the mitral and tricuspid heart valves is reported. Former history of the patient did not reveal narcotic addiction or previous open-heart surgery. Osteomyelitis is an uncommon complication of pseudomonas endocarditis. Echocardiography was a useful diagnostic method in the present case, showing the vegetation on the tricuspid valve. The poor prognosis of cases with left valvular heart disease which are resistant to medical treatment is emphasized. Differential count of the colonies did not localize the affected heart valve …

Primary suppurative myositis in children.

Primary suppurative myositis is rare in the United States when compared with the incidence of disease in the tropics. Clinically, it may mimic many of the more common diseases, such as hematoma, osteomyelitis, arthritis, or appendiceal abscess. It usually has a benign course, with complete recovery after appropriate treatment. Prolonged morbidity and an increased mortality may result from unfamiliarity with this entity. Six children had primary suppurative myositis; one died as a result of perforation of a psoas abscess into the dural sac causing staphylococcal meningitis.
