A sample of 228 patients with facial pain is reported in the present paper. Among them, 112 cases are typical idiopathic trigeminal neuralgias, 111 are cases of secondary facial pain (including 7 cases symptomatic trigeminal neuralgias) and 5 patients were classified as vascular facial pain. Among the 119 cases of clinically diagnosed trigeminal neuralgias there were 112 (94, 11%) with no evidence of organic cause. The recognition of an association between trigeminal neuralgia and ipsilateral hemifacial spasm has been delayed by confusion over the nomenclature of the two conditions. Three patients are presented who had facial pain associated with hemifacial spasm. The findings on investigation of these patients, and an analysis of the literature, suggests that the combination is almost inevitably associated with pathological processes in the posterior fossa, particularly anomalous, ectatic or aneurysmal blood vessels.

The pathological course of the most common diseases of the shoulder includes pain as a main symptom. Besides other methods of treatment the infiltration of the suprascapular nerve can improve the therapeutic methods in many cases. The technique of Gordh, used as an adjuvant procedure, is shown. In this perplexing case, a child with signs of apparent hip pain was treated for a sewing needle embedded in the sole of the foot. The diagnosis of foreign body easily could have been missed, because the external signs were minimal and the clinical findings indicated hip disease. However, the child's extreme distress seemed inconsistent with the initial normal findings. Only reexamination and looking beyond the obvious led to the diagnosis.

Case histories of 15 patients with dental pain who were examined at the Royal Adelaide Hospital Pain Clinic were reviewed. These were the only patients with dental pain seen at the clinic over a 10-year period and represent less than 1% of the total patients fully examined at the clinic. The etiology of painful papule of the external ear, chondrodermatitis nodularis chronica helicis, has been an enigma for over half a century. I reviewed 84 skin biopsy specimens from 82 patients with this histopathologic diagnosis and found evidence that the infundibular portion of the hair follicle is probably the primary cutaneous structure involved in this condition. This lesion appears to evolve from an acute, suppurative, granulomatous dermatitis into a later-fibrosing dermatitis having the clinical and histologic features of perforating folliculitis and prurigo nodularis.

Therapeutic splenic embolization has been commonly performed for hypersplenism, tumor palliation, and other indications, but has not been reported as a therapy specifically for pain relief. We describe 1 patient with chronic pain secondary to severe splenomegaly in whom surgery was contraindicated. Selective partial splenic embolization achieved a pain-free state for the patient without significant complications.

This paper reports the results of bone scans on 78 painful feet. Scanning helped in the diagnosis of persistent foot pain following injury and it enabled stress fractures, fractures of the sesamoids and subtalar arthritis to be diagnosed earlier. It reliably excluded bone infection and was useful as a screening test when radiographs were normal.
