Enteral Access in Pediatric Cardiac Patients: A Survey of Pediatric Surgeons at the Top 50 Cardiac Hospitals.

PURPOSE: Maintaining the nutritional status of pediatric cardiac patients is essential as these children often have higher metabolic needs and predilection for feeding difficulties. Despite the importance in preventing malnutrition in these patients, there remains a lack of consensus in management. We surveyed pediatric surgeons at top pediatric cardiac hospitals to query their practices in enteral access for infants with cardiac disease. METHODS: We utilized the US News & World Report rankings to identify the top rated 50 "Pediatric Cardiac" hospitals and surveyed pediatric surgeons at each

De-prescribing opioids among Medicaid patients with long-term opioid use.

BACKGROUND: Guidelines encourage deprescribing opioids for long-term opioid patients, especially those using opioids and benzodiazepines, z-drugs, or muscle relaxants ("other respiratory depressants"). OBJECTIVE: Were long-term opioid patients who were prescribed other respiratory depressants more likely to have deprescribing opioid trajectories? DESIGN: Cross-sectional retrospective study using pharmacy and professional claims from 2015 to 2019. Adjusted logistic regression models were stratified on low (<50 morphine milligram equivalents; MME) and high (>50 MME) starting opioid doses. We r

[Use of adjuvant treatment options in tobacco and nicotine counseling: results of an anonymous survey].

Counseling and treatment of tobacco and nicotine users include classical behavioral therapy and cognitive-behavioral therapy methods. More than half of the quitters after an intervention relapsed into old behavior after 12 months. Discussions with experts have shown that evidence-based interventions are individually supplemented with treatment options without scientific evidence or with inconsistent efficacy. This study examined the use of adjuvant treatment options in the area of smoking cessation such as aromatherapy. The sample for our anonymous online survey (Sosci survey) consisted of 351

Decisions About Suppressive Antibiotics Among Clinicians at Veterans Affairs Hospitals After Prosthetic Joint Infection.

IMPORTANCE: Indefinite suppressive antibiotic therapy (SAT) is sometimes prescribed after initial antibiotic treatment for prosthetic joint infection (PJI). Limited evidence on outcomes after SAT exists, and using SAT for patients at low risk who may not need it could be associated with antibiotic resistance and adverse events. OBJECTIVES: To characterize clinical decision-making about SAT after PJI and identify stewardship intervention opportunities to stop or reduce SAT for patients who may not benefit. DESIGN, SETTING, AND PARTICIPANTS: In this qualitative study, interviews were conducted

Comparing the diagnostic considerations between general practitioners with a special interest in cardiovascular disease and those without in patients with symptoms suggestive of heart failure: a vignette study.

BACKGROUND: General practitioners (GPs) specialized in cardiovascular disease (GPSI-CVD) may suspect heart failure (HF) more easily than GPs not specialized in CVD. We assessed whether GPSI-CVD consider investigations aimed at detecting HF more often than other GPs in two clinical scenarios of an older male person with respiratory and suggestive HF symptoms. METHODS: In this vignette study, Dutch GPs evaluated two vignettes. The first involved a 72-year-old man with hypertension and a 30 pack-year smoking history who presented himself with symptoms of a common cold, but also shortness of brea

Inappropriate Ordering of Multitarget Stool DNA Tests for Colon Cancer Screening.

BACKGROUND: CRC screening is recommended for adults aged 45-75. Mt-sDNA is indicated for asymptomatic individuals between the ages of 45 and 85, but not for those with rectal bleeding, iron deficiency anemia, adenomatous polyps, previous colonoscopy within 10 years, family history of CRC, positive results from CRC screening tests within the past 6 months, or age less than 45 and greater than 85. We aimed to determine the prevalence of mt-sDNA use when not indicated and factors associated with inappropriate testing. METHODS: 7,345 patients underwent mt-sDNA testing and were randomized using EM

The Impact of reference pricing on prescribing patterns, costs, and health services utilization of proton pump inhibitors: A quasi-experimental study in British Columbia, Canada.

INTRODUCTION: The Reference Drug Program (RDP) was established to steer patients toward equally safe and cost-effective medication under British Columbia's public drug coverage. Each RDP class covers at least one reference drug, and non-reference drugs are reimbursed up to the cost of the reference drug. In 2016, the RDP updated to include proton pump inhibitors (PPIs). This study evaluated the impact on drug expenditures, prescription patterns, and health services utilization. METHODS: We identified a cohort of individuals covered by Fair Pharmacare who used PPIs, and a control group of H2 B

Opioid utilization after orthopaedic trauma hospitalization among Medicaid-insured adults.

Opioids are vital to pain management and sedation after trauma-related hospitalization. However, there are many confounding clinical, social, and environmental factors that exacerbate pain, post-injury care needs, and receipt of opioid prescriptions following orthopaedic trauma. This retrospective study sought to characterize differences in opioid prescribing and dosing in a national Medicaid eligible sample from 2010-2018. The study population included adults, discharged after orthopaedic trauma hospitalization, and receiving an opioid prescription within 30 days of discharge. Patients were i
