Effectiveness of interventions to enhance healing of chronic ulcers of the foot in diabetes: a systematic review.

The outcome of management of diabetic foot ulcers remains a challenge, and there remains continuing uncertainty concerning optimal approaches to management. It is for these reasons that in 2008 and 2012, the International Working Group of the Diabetic Foot (IWGDF) working group on wound healing published systematic reviews of the evidence to inform protocols for routine care and to highlight areas, which should be considered for further study. The same working group has now updated this review by considering papers on the interventions to improve the healing of chronic ulcers published between

Acute Charcot neuro-osteoarthropathy.

Charcot neuro-osteoarthropathy (CN) is one of the most challenging foot complications in diabetes. Common predisposing and precipitating factors include neuropathy and increased mechanical forces, fracture and bone resorption, trauma and inflammation. In the last 15 years, considerable progress has been made in the early recognition of the acute Charcot foot when the X ray is still negative (stage 0 or incipient Charcot foot). Recent advances in imaging modalities have enabled the detection of initial signs of inflammation and underlying bone damage before overt bone and joint destruction has

A Direct Relationship Between 'Blood Stasis' and Fibrinaloid Microclots in Chronic, Inflammatory, and Vascular Diseases, and Some Traditional Natural Products Approaches to Treatment.

'Blood stasis' (syndrome) (BSS) is a fundamental concept in Traditional Chinese Medicine (TCM), where it is known as Xue Yu (). Similar concepts exist in Traditional Korean Medicine ('Eohyul') and in Japanese Kampo medicine (Oketsu). Blood stasis is considered to underpin a large variety of inflammatory diseases, though an exact equivalent in Western systems medicine is yet to be described. Some time ago we discovered that blood can clot into an anomalous amyloid form, creating what we have referred to as fibrinaloid microclots. These microclots occur in a great many chronic, inflammatory dise

[Textual study of Baihuasheshecao (Hedyotis diffusa)].

Baihuasheshecao(Hedyotis diffusa) is a commonly used traditional Chinese medicine derived from the whole herb of H. diffusa and has been widely utilized in folk medicine. It possesses anti-tumor, antibacterial, and anti-inflammatory properties, making it one of the frequently used herbs in TCM clinical practice. However, Shuixiancao(H. corymbosa) and Xianhuaercao(H. tenelliflora), species of the same genus, are often used as substitutes for Baihuasheshecao. To substantiate the medicinal basis of Baihuasheshecao, this study systematically reviewed classical herbal texts and modern literature, e

Predictive Performance of pyDarwin-Selected Versus Expert-Developed Paediatric Tobramycin Population Pharmacokinetic Models for Model-Informed Precision Dosing.

PyDarwin is an open-source Python package integrating machine learning with NONMEM for automated model selection; however, its utility in model-informed precision dosing (MIPD) remains unvalidated. This study aims to evaluate pyDarwin for population pharmacokinetics (PopPK) model development in MIPD compared with expert-developed models, using paediatric tobramycin as a representative case. The model-building dataset included 450 plasma concentrations from 63 children with cystic fibrosis (CF). One group manually developed a model using stepwise covariate selection, while another applied pyDar

Next-Generation Sequencing-Based Molecular Profiling and Management of Advanced Solid Tumors: A Real-World Experience in a Community-Based Academic Cancer Center.

BACKGROUND: Advances in molecular and cell biology have led to a paradigm shift in cancer management. We implemented next-generation sequencing (NGS) at our community-based academic cancer center over a decade ago. We sought to optimize the use of genomics and understand its impact in identifying actionable genomic alterations to determine therapy options in patients with advanced solid tumors for whom standard therapies had been exhausted. METHODS: Through a prospective non-randomized trial, patients with advanced solid tumors who had exhausted standard therapies and who did not qualify for

A scalable framework for population-level precision health: Insights from sleep disorders.

Assessing population-level risk patterns of sleep disorders is challenging. Many studies rely on hospital-based samples, which can be biased toward individuals with severe symptoms, or on polysomnography, which is costly and impractical for large-scale screening. To address this, we used SLEEPS, a machine learning algorithm that estimates risks of insomnia, obstructive sleep apnea (OSA), and comorbid insomnia and sleep apnea (COMISA). Since its online launch, SLEEPS has been used by over 40,000 individuals, producing a large web-based dataset collected through voluntary participation from publ

Supramolecular STING-Hydrogel Spatiotemporally Boosts Tumor-Derived Extracellular Vesicle-Based Personalized Vaccine for Enhanced Cancer Immunotherapy.

Personalized vaccines represent a promising approach for cancer treatment by eliciting tumor-specific immune responses. However, their development faces persistent challenges, including the laborious and costly process of neoantigen identification, which substantially delays vaccine production. Moreover, conventional platforms suffer from rapid systemic clearance and inefficient delivery to antigen-presenting cells (APCs), leading to only transient and weak immune activation. Here, we report a STING-agonist-integrated hydrogel system designed for localized delivery of tumor-derived extracellul

Machine learning for personalized antimicrobial susceptibility breakpoints.

OBJECTIVES: Infection diagnoses are critical to the personalized interpretation of EUCAST aminopenicillin breakpoints for Enterobacterales, but microbiology laboratories cannot predict diagnosis when specimens are received. Here, we assess whether machine learning could facilitate personalized antimicrobial susceptibility breakpoint reporting by predicting urinary tract infection (UTI) diagnoses. METHODS: XGBoost models were trained using open-source electronic healthcare record data to predict complicated UTI in patients with Enterobacterales bacteriuria and to predict UTI in patients with E

Personalized infliximab rescue therapy to maximize colectomy-free survival in patients with acute severe ulcerative colitis.

BACKGROUND & AIMS: Infliximab is an established rescue therapy for patients with steroid-refractory acute severe ulcerative colitis (ASUC), yet optimal dosing strategies minimizing colectomy risk remain unclear. We aimed to develop a model-informed risk stratification algorithm to identify patients at high risk of colectomy within 90 days of initiating infliximab to support personalized dosing. METHODS: We conducted a multicenter, retrospective population pharmacokinetics (popPK) and exposure-response study using data from patients with ASUC. A parametric time-to-event model was developed to

Pharmacogenomics in the prediction of adverse effects of cardiovascular drugs: the PGx-CardioDrug project.

Although the role of pharmacogenomics (PGx) in personalized pharmacotherapy has been well established, its implementation in clinical practice lags behind. In this article, we present an overview of important achievements in the field of PGx of cardiovascular drugs (CVDs), and identify gaps in the existing research. We also present an outline of the PGx-CardioDrug project (ClinicalTrials.gov: NCT05307718) focused on PGx of three major classes of CVDs: P2Y12 antiplatelets, direct oral anticoagulants (DOACs), and statins. The project intends to evaluate pharmacogenes, concomitant treatments, and

Frequency of therapeutic drug monitoring in inpatient precision depression care and duration of hospitalization.

In psychiatry, therapeutic drug monitoring (TDM) is routinely used to improve treatment safety and efficacy, yet its association with hospitalization duration and cost-effectiveness remains unclear. We hypothesized that the frequency of routinely requested TDM is associated with hospitalization duration. In this retrospective study, patients with major depressive disorder receiving TDM of antidepressants between 2015 and 2021 (N = 383) were analyzed. Inclusion criteria were a hospital stay ≥7 days and a first TDM request at admission. Regression analyses assessed associations between TDM frequ

Data driven versus standard practice policies for personalized antidepressant treatment: a retrospective cohort study of electronic health records.

BACKGROUND: Depression is a heterogenous condition, with high societal and economic costs. Response to available treatments is varied, often trial-and-error, raising the need for personalization. Previous studies demonstrated success in predicting treatment outcomes for patients in psychiatric settings diagnosed with major depressive disorder, but have not done so in the general practice setting. METHODS: A retrospective cohort study analyzed electronic health records (EHRs) from the UK’s National Health Service, where depressive symptoms were recorded at two time points using the Patient Heal

Novel protein biomarkers for risk stratification and personalized medicine.

BACKGROUND AND HYPOTHESIS: Albuminuria is widely used to monitor chronic kidney disease (CKD) and treatment response but is subject to variability and primarily reflects glomerular injury. Urinary clusterin and epidermal growth factor (uEGF) are associated with tubular injury and repair, respectively, and may serve as mechanism-informed pharmacodynamic biomarkers. We investigated responses of urinary clusterin and uEGF to endothelin receptor antagonists (ERAs) and sodium-glucose cotransporter 2 inhibitors (SGLT2i), respectively, in CKD. METHODS: We evaluated changes in urinary clusterin-to-cr

A tumour-derived organoid biobank maps cancer gene dependencies.

Cancer cell lines remain foundational for research and drug discovery, yet they incompletely capture tumour diversity, lack linked patient context, and have undergone adaptation to culture. Tumour organoids are three-dimensional cultures derived from patient tissue that offer a powerful complement to cell lines1. Here we derived and characterized 256 clinically annotated tumour organoids directly from colorectal, oesophageal, ovarian, pancreatic and gastric cancers as renewable, genetically stable models. Extensive characterization of each model and matched patient tumour samples included whol
