Brachytherapy has confirmed its prevailing role in conservative treatment of oral cavity carcinomas. To describe late toxicity in long-term surviving patients, comparisons with other series are necessary. Study of series of patients implanted for floor of the mouth or mobile tongue shows the need for more detailed data. Dental prophylaxy and lead protection of the mandibule, good indications and techniques of brachytherapy are necessary to avoid late complications. A case of radiation nephritis was diagnosed by the effect of delayed excretion of 99Tcm MDP on the superomedial aspects of both kidneys, which had been included in a radiation port to the thoracolumbar spine 6 months previously. Knowledge of the past history of radiotherapy and the radiation port, together with the classic scintigraphic features enable the diagnosis of this rarely demonstrated condition to be made.

The outcome of 75 cases of post-radiotherapy pericarditis severe enough to lead to surgery, was reviewed from 1970 to September 1995. Four clinical forms were identified: acute pericarditis resistant to medical treatment (12 cases), large chronic pericardial effusions resistant to medical treatment (16 cases), chronic, compressive effusions (35 cases) and signs of pericardial constriction (12 cases). Most documented cases of radiation-induced optic neuropathy are unilateral and occur more than 1 year after radiotherapy to the sellar region. We describe a patient with bilateral radiation optic neuropathy 3 months following the completion of radiotherapy. MRI 13 months after the onset of visual failure showed bilateral optic atrophy with residual gadolinium enhancement.

We performed augmentation gastrocystoplasty in five adult female patients with radiation cystitis after excising the injured bladder wall. Between 1984 and 1989 240 patients underwent radical abdominal resection of a rectal carcinoma. Out of 201 patients surviving 12 months or more postoperatively, two groups are surveyed. A woman suffering from carcinoma of the cervix went through radical hysterectomy and radiotherapy. The treatment was complicated by obstruction of the lower left ureter which was dealt with by transureteroureterostomy. Eighteen months later she was admitted with severe stricture of the common pelvic ureter and massive retroperitoneal fibrosis. The afflicted ureteral segment was successfully reformed by intubated ureterotomy.

Seven instances of insufficiency fractures of the sacrum due to metabolic bone disease or secondary to pelvis radiation therapy were retrospectively reviewed. The contribution of the different investigation procedures was compared. Identification of stress fractures during clinical investigation or in the assessment of metabolic disease should enable early and adequate response and possibly prevent unnecessary biopsies.

Photic retinal injuries have been described after surgical exposure to the intense illumination systems of operating microscopes. The overall incidence of such injuries has been postulated at less than 10%. The authors prospectively studied 43 consecutive patients to determine the incidence of photic macular injuries during cataract surgery. Between 1967 and 1986, 319 patients, judged unsuitable for cystectomy, were scheduled to receive curative radiation treatment for transitional cell cancer of the urinary bladder. In a prospective inventory analysis of 169 irradiated patients concerning the incidence and course of Acute Radiation Sickness (fatigue, loss of appetite, nausea and vomiting), it was found that 2/3 of the patients had these complaints. The efficacy of various therapeutic schemes: medicinal (basic therapy--BT), acupuncture (AP) and laser therapy (LT) against a background of basic therapy--was assessed and compared in 36 patients with radiation limb edema. It was established that a degree of a decrease in edemas, the improvement of indices of rheovasography grew in the following order: BT----AP----LT. The recovery of the lymph flow and immunological indices were the same in all therapeutic schemes.
