"Unhappy triad" of the trauma elbow in children: Diagnosis, classification, and mid-term outcomes.

BACKGROUND: The aim of this study was to describe the epidemiology, physiopathology, and outcomes of elbow "unhappy triad" trauma in children, combining a posterior dislocation, a proximal radius fracture, and a third lesion (i.e. bony or capsuloligamentous injury). METHODS: A retrospective bicentric study was conducted between 1999 and 2020. All skeletally immature children who presented to the emergency department and underwent surgery for a proximal radius injury were selected. Among this selection, only patients with two associated ipsilateral elbow injuries (i.e. posterior elbow dislocat

Subacute Traumatic Ascending Myelopathy in a 28-Year-Old Man: A Rare Case.

BACKGROUND: Subacute posttraumatic ascending myelopathy (SPAM) involves the rise in high signal intensity on T2-weighted images ≥4 vertebral segments above the initial injured site, and it usually occurs within the first few weeks after the injury. The pathophysiologic mechanisms of traumatic spinal cord damage are not clearly understood; however, there are some pathophysiologic processes such as arterial thrombosis, venous thrombosis, congestive ischemia, inflammatory or autoimmune reaction, and infection in the form of meningitis or myelitis that could lead to SPAM. CASE DESCRIPTION: We pre

Adolescent Femoral Chondral Fragment Fixation With Poly-L-Lactic Acid Chondral Darts.

Large chondral injuries without attached bone are uncommon. This report describes a 14-year-old boy who had a unique stress reaction between the bone and the overlying cartilage, predominantly of the anterior lateral femoral condyle, during a week-long basketball camp, resulting in complete displacement of a 2.5 × 2.5-cm full-thickness articular cartilage lesion. There was a 6-day interval from the time of the injury to the first office appointment. Scheduling of magnetic resonance imaging and insurance approval took another week, and then surgery scheduling, including insurance approval and a

Neurophysiological identification of position-induced neurologic injury during anterior cervical spine surgery.

This study was a retrospective review of 3,806 patients who underwent anterior cervical spine surgery with multi-modality neurophysiological monitoring consisting of transcranial electric motor evoked potentials, somatosensory evoked potentials and spontaneous electromyography between 1999-2003. The objectives of this study were twofold: (1) to evaluate the role of transcranial electric motor evoked potential tceMEP and ulnar nerve somatosensory evoked potential (SSEP) monitoring for identifying impending position-related stretch brachial plexopathy, peripheral nerve entrapment/compression or

Arthroscopic double row partial articular supraspinatus tendon avulsion bridge repair technique for shoulder: A transtendinous approach.

Partial articular supraspinatus tendon avulsion (PASTA) lesions, a subset of partial rotator cuff tears, pose a surgical challenge, disrupting the integrity of the supraspinatus tendon. Transtendinous repair is the preferred choice in young individuals for limiting tear progression and preserving intact, high-quality cuff tissue, thus preventing tendon shortening, as compared to the tear completion and repair technique. Our approach leverages these advantages, specifically those indicated for Ellman's Grade 3 tears and cases where conservative treatments have failed. In our technique, we emplo

Determining the minimal clinically important difference of the hand function sort questionnaire in vocational rehabilitation.

OBJECTIVE: To estimate the Minimal Clinically Important Difference (MCID) of the French version of the Hand Function Sort questionnaire (HFS-F). As a comparison, the MCID of the Disabilities of the Arm, Shoulder, and Hand (DASH) was also estimated. MATERIALS AND METHODS: We included French-speaking patients hospitalized in a multidisciplinary rehabilitation program for chronic pain of the upper limb after an accident. HFS-F and DASH scores were collected at admission and discharge; the Patient Global Impression of Change measure (PGIC; 7 levels) was collected at discharge. The MCID was estima

Tears of the rotator cuff. Causes--diagnosis--treatment.

Rotator cuff ruptures are the most common degenerative tendon injury and occur mainly in older patients as multifactorial disorders manifesting the main symptoms of pain and restricted range of motion. Thorough clinical examination of the shoulder includes testing the function of the rotator cuff and leads to a tentative clinical diagnosis that is the prerequisite for diagnostic imaging procedures. Sonography of the shoulder gives rapid access to a very good sensitive overview of the rotator cuff. Conventional radiological imaging permits differential diagnosis since a reduced acromiohumeral i

Avulsion injury of the serratus anterior: a case history.

OBJECTIVE: We report the first documented case of avulsion of the serratus anterior muscle. A young man had difficulty in abducting his arm the day after using a rowing ergometer. Examination showed a firm mass on his lateral thoracic wall. This became firm with protraction of the scapula. Avulsion was confirmed by magnetic resonance imaging (MRI) scanning. Rest and modified activities resulted in resolution of symptoms. DISCUSSION: This case highlights the role of serratus anterior in the rowing action and supports its implication in stress fractures of the ribs. An imbalance between rhomboi

[The acromio-humeral interval. A study of the factors influencing its height].

PURPOSE OF THE STUDY: The narrowing of the Acromio Humeral Interval (AHI) under 6-7 mm, lower limit reported in normal shoulder, has been considered to be a specific indicator for full-thickness cuff tears. The purpose of this study was to analyse the factors influencing the AHI. METHODS: 264 shoulders were operated on between 1984 and 1994 for full thickness tear of the supraspinatus and infraspinatus associated or not with an anterior cuff lesion. All patients had X-ray with A.P. view of the shoulder in neutral rotation. The AHI was defined as the shortest distance measured between the infe

Anterior dislocation of the shoulder and rotator cuff rupture.

Thirty-seven patients older than 40 years of age were seen after sustaining primary anterior dislocations of the shoulder. An associated rupture of the rotator cuff in each patient had been missed, often being mistaken for an axillary neuropathy. Eleven of these patients developed recurrent anterior instability that was due to rupture of the subscapularis and anterior capsule from the lesser tuberosity. In no patient was there a Bankart lesion. Repair of the capsule and subscapularis restored stability in all of the patients with recurrence.

Evaluation of impact loading in overhead work using a bolt pistol support.

This paper evaluates the effects of a bolt gun support stand on impact loading of the hand-arm system while firing spiked bolts into a steel beam. Ten healthy male subjects fired two bolts into the centre of the beam, one with and one without the aid of the support stand. During firing of a bolt gun, the shock sequences were recorded by two accelerometers. The analysis showed that peak values of acceleration decreased more than eightfold during firing with the bolt pistol support. The shock damping effect due to the bolt support was noted at the thumb (metacarpal level) and elbow bones.

Shoulder surgery for rotator cuff tears. Ultrasonographic 3-year follow-up of 97 cases.

We reexamined 97 of 116 shoulders operated on for rotator cuff lesions after an average follow-up time of 37 months. Seventy percent had a good or excellent clinical result, and 14 percent were graded as poor. Upon ultrasonographic examination of the 97 shoulders, 37 had a normal rotator cuff, 31 had thinning and/or hyperdensity, and 29 had a complete rupture of the cuff. Patients with concomitant anterior acromioplasties did better than those without. There was a poor correlation between clinical and ultrasonographic results. We recommend that rotator cuff tears should be closed only if this

[Internal fixation with soft-tissue lesions: fracture with nerve damage (author's transl)].

Typical nerve damage after fractures due to the topographical proximity of the nerves to the bones, are shown and therapeutic principles are discussed. Primary traumatic lesions are differentiated from so-called "creeping paralysis". The methods of choice are neurolyses, nerve suture or transplantation, each at the appropriate time. Evaluation of 83 of the 128 Patients undergoing microsurgical operations on the nerves in recent years is presented after re-exmination by independent neurologists. One-third showed definite signs of regeneration, more than one-third had good functional results, an

Relationship Between Restraint System Use and Brachial Plexus Injuries in Motor Vehicle Accidents: A Case-Control Study from the National Trauma Databank.

INTRODUCTION: High-speed motor vehicle accidents (MVAs) are an important cause of brachial plexus injury (BPI). Some case reports have demonstrated shoulder seat belt use resulting in traction injuries to the brachial plexus. We used a national trauma registry to determine the association between seat belt use and brachial plexus injury in MVAs. METHODS: The authors queried the National Trauma Databank between 2016 and 2017 for patients with a hospital admission following an MVA. Cases with BPI were identified using International Classification of Diseases, Tenth Edition, Clinical Modificatio

At-Risk Tackling Techniques in American Football.

BACKGROUND: In American football, fewer fatalities and severe injuries have been seen annually since 1976, after data from 1971 through 1975 were retrospectively reviewed to better understand the mechanisms involved in catastrophic cervical spine injury and rules were enacted to prohibit certain types of aggressive tackling. The National Football Head and Neck Injury Registry was established in 1975. PURPOSE: To assess (1) tackling techniques that coaches were teaching at 3 levels-youth level (YL; 4th to 5th grades), middle school (MS; 6th to 8th grades), and high school (HS; 9th to 12th grad

Primary Ligament Repair for Acute Elbow Dislocation.

The elbow is the second most common site for joint dislocation after the shoulder. Elbow dislocations are classified according to the direction of dislocation, with most (80%) being posterolateral. A spectrum of soft-tissue injury may also be present, depending on the direction of dislocation and the energy applied. Most dislocations of the elbow can be treated nonoperatively, but recurrent instability and/or stiffness occur in up to 10% and 40% of patients, respectively. The aim of early surgical stabilization is to prevent these long-term complications. To avoid overtreatment, magnetic reson
