Multiple system atrophy manifesting as complex sleep-disordered breathing.

A 56-year-old female presented with multiple system atrophy (MSA) and pure obstructive sleep-disordered breathing (SDB) with an apnea hypopnea index of 29.6/h. However, after the elimination of obstructive events, central apneas emerged with a central apnea index (CAI) of 45.3/h as determined by continuous positive airway pressure titration. Adaptive servoventilation titration decreased the CAI; however, it remained at 14.2/h. There are two different underlying causes of SDB in MSA patients, one is upper airway obstruction at the glottic level, and the other is central components owing to the …

Morphological examination of upper airway in obstructive sleep apnea.

OBJECTIVES: An aim of this study was to assess the predictive power of an otorhinolaryngological examination of the upper airway to identify risk factors of obstructive sleep apnea syndrome (OSAS) in the patients. METHODS: We examined 141 consecutive patients with OSAS. The morphological features were assessed by the designated otorhinolaryngologist while the subjects were sitting relaxedly with tidal breathing. The bilateral nasal resistance was measured using the active anterior rhinomanometry during daytime wakefulness. RESULTS: The body mass index (BMI), fauces's narrowness, neck circumf…

Why does gender influence survival in obstructive sleep apnoea?

The study aim was to compare the mortality risk of men and women diagnosed with obstructive sleep apnoea (OSA) and started on treatment with continuous positive airway pressure (CPAP). From August to December 2003 we reviewed the hospital records of patients who had started on CPAP for OSA between July 1995 and June 1998. Mortality rates were compared between men and women. Associations with mortality risk were determined using univariate and multivariate Cox's proportional hazards regression. The sample comprised 292 men and 47 women. Eight percent of the men and 23% of the women died (p=0.00…

Selective effects of CPAP on sleep apnoea-associated manifestations.

BACKGROUND: Visceral adiposity and obstructive sleep apnoea (OSA) may be independently associated with daytime sleepiness/low performance, insulin resistance, hypercytokinaemia, and/or hypertension. The objectives of this study are to simultaneously test these associations at baseline and after 3 months of continuous positive airway pressure (CPAP) therapy. MATERIALS AND METHODS: Sixteen obese men with OSA; 13 non-apnoeic, obese controls, and 15 non-obese controls were monitored in the sleep laboratory for four consecutive nights. Objective measures of daytime sleepiness and performance, seri…

Obesity and sleep disturbances: meaningful sub-typing of obesity.

Obesity, excessive daytime sleepiness (EDS), and self-reported short sleep duration appear to be on the rise, while there is evidence that obesity and these sleep disorders are strongly connected. In this paper, we review data that challenge the common belief that the sleep apnoea and sleep loss, frequently associated with obesity, are the primary determinants of obesity-related objective daytime sleepiness and subjective fatigue (tiredness without increased sleep propensity). Specifically, obesity is associated with objective and subjective EDS regardless of the presence of sleep apnoea. The …

Dental arch morphology in children with sleep-disordered breathing.

The aim of the present study was to examine the effects of nocturnal breathing disorders such as obstructive sleep apnoea (OSA) and snoring on developing dental arches. The study group comprised 41 children (22 males, 19 females, mean age 7.2 years, standard deviation 1.93) with diagnosed OSA. Age- and gender-matched groups of 41 snoring and 41 non-obstructed control children were selected. Orthodontic examination was carried out and dental impressions were taken. Malocclusions were diagnosed clinically and 13 linear variables were measured from the dental casts. The differences between the de…

Genetic approaches for target identification in sleep/wake systems.

Attempts to therapeutically induce sufficient amounts of sleep have occurred since the advent of the medical arts, but the health implications of failing to maintain a correct balance of sleep/wake has only recently become widely appreciated both in the medical arena and among the general public. Inappropriate amounts of sleep have been linked to cognitive impairment, immune suppression and metabolic changes, as well as to the sequelae of reduced vigilance and increased likelihood of involvement in automobile accidents or other causes of fatalities. Currently approved treatments for insomnia a…

Interrater and intrarater reliability in multiple sleep latency test.

Multiple sleep latency test (MSLT) remains the golden standard for the clinical diagnosis and management of excessive daytime sleepiness. However, there was limited data on the reliability measurement of MSLT. Forty-four (M/F ratio: 26/18, mean age of 43.4 +/- 13.9 years) subjects of an ongoing family study of narcolepsy underwent a standard polysomnogram and MSLT. Three trained staff of various level of experiences independently and blindly scored each nap (n = 219). To test for intrarater reliability, 100 naps (n = 20 subjects) were re-scored half a year later. The interrater reliability for…

Sleep impairment in allergic rhinitis, rhinosinusitis, and nasal polyposis.

Sleep impairment is a significant problem for patients with inflammatory disorders of the upper respiratory tract, such as allergic rhinitis, rhinosinusitis, and nasal polyposis. Nasal congestion, one of the most common and bothersome symptoms of these conditions, is associated with sleep-disordered breathing and is thought to be a key cause of sleep impairment. This review examines sleep impairment associated with allergic rhinitis, rhinosinusitis, and nasal polyposis. It explores the adverse effects of disturbed sleep on patients' quality of life and how these inflammatory nasal conditions c…

[How to investigate sleep related breathing disorders in children?].

Sleep related breathing disorders (SDB) in children has a major negative impact on their neurocognitive development and should be identified and treated early as to decrease morbidity. Children do not usually present daytime fatigue and sleepiness like adults, but rather show abnormal behavior patterns and learning disabilities. The presence of sustained nightime snoring is a good screening toll for SDB. Polysomnography is the gold standard for diagnosis even though nightime oxymetry, with its high positive predictive value, can also be used. The most frequent SDB in childhood is sleep obstruc…

Nighttime sleep problems and daytime sleepiness in Parkinson's disease.

Our objective is to evaluate nighttime sleep problems (NSP) and daytime sleepiness (DS) in patients with Parkinson's disease (PD) compared to controls, and to assess relations with demographic, disease-related, and clinical characteristics in patients. NSP and DS were evaluated with the SCOPA-SLEEP questionnaire in PD patients and controls. In patients, other disease-related and clinical characteristics were also evaluated. Four hundred twenty PD patients [mean (SD) age 61.1 (11.5) years] and 150 controls [mean (SD) age 60.9 (9.9) years] participated in the study. Compared to controls, a signi…

Relationships between affect, vigilance, and sleepiness following sleep deprivation.

This pilot study examined the relationships between the effects of sleep deprivation on subjective and objective measures of sleepiness and affect, and psychomotor vigilance performance. Following an adaptation night in the laboratory, healthy young adults were randomly assigned to either a night of total sleep deprivation (SD group; n = 15) or to a night of normal sleep (non-SD group; n = 14) under controlled laboratory conditions. The following day, subjective reports of mood and sleepiness, objective sleepiness (Multiple Sleep Latency Test and spontaneous oscillations in pupil diameter, PUI…

CPAP compliance in patients with obstructive sleep apnea syndrome.

Obstructive sleep apnea syndrome (OSAS) is characterized by repeated cessations of breathing during sleep. Major symptoms of this disease are excessive daytime sleepiness, snoring, and witnessed apnea. Most of the patients are treated with CPAP. In this study, we aimed to evaluate the factors affecting adherence to the CPAP treatment. Seventy-one patients were enrolled to this study. Patients were divided into three groups according to CPAP usage. Group I consisted of patients who had never used CPAP, group II consisted of patients who had used CPAP occasionally, and group-III patients had use…

Sleep quality and depression during pregnancy: a prospective study.

For the first time, the relationship between depressive symptoms and sleep quality was explored prospectively during pregnancy. Participants (n = 273) completed the Pittsburgh Sleep Quality Index and Beck Depression Inventory at three 8-week intervals, starting from 15-23 weeks gestation. In addition to sleep quality and depression remaining relatively stable during pregnancy, findings revealed that sleep quality earlier in pregnancy predicted higher levels of depressive symptoms at later stage in pregnancy (after controlling for prior depression levels). In contrast, there was no evidence to …

Daytime sleepiness and polysomnography in obstructive sleep apnea patients.

BACKGROUND: Excessive daytime sleepiness (EDS) is the major complaint in subjects with obstructive sleep apnea syndrome (OSAS). However, EDS is not universally present in all patients with OSAS. The mechanisms explaining why some patients with OSAS complain of EDS whereas others do not are unknown. OBJECTIVE: To investigate polysomnographic determinants of excessive daytime sleepiness (EDS) in a large multicenter cohort of patients with obstructive sleep apnea (OSAS). METHODS: All consecutive patients with an apnea-hypopnea index greater than 5h(-1) who were evaluated between 2003 and 2005. …
