Sleep-disordered breathing (SDB) and cardiovascular disease (CVD) are closely related; however, the effect of SDB on the long-term prognosis of patients with CVD is unknown. Our aim in this study was to assess the association between SDB and fatal cardiovascular events in patients with CVD. We performed a long-term follow-up study of 135 patients with CVD. The average observation period was 610 +/- 268 days. The patients were classified into 2 groups based on their apnea index: patients with apnea index >or=5/h (Group H) were diagnosed with SDB (n = 43), and those with apnea index <5/h (Group

Clinical experience shows that episodes of apnea can occur during regurgitations or vomiting. We questioned whether sleep apneas could be related temporally to documented falls in esophageal pH, when no clinical symptoms of emesis are witnessed. Twenty infants admitted after an apparently life-threatening event ('ALTE') during sleep, but with no clinical symptoms of vomiting or regurgitations at the time of the event, and ten control infants were studied. All infants had occasional episodes of regurgitations. Polygraphic monitoring of state of alertness, cardiorespiratory activity and low esop

One hundred twenty-six eligible consecutive Chinese heart failure (HF) patients classified by New York Heart Association (NYHA) Classes II-IV underwent historic data collection and a sleep study. Seventy-one percent of HF patients were diagnosed with sleep apnea (SA), of which 65% were central sleep apnea (CSA) and 35% were obstructive sleep apnea (OSA). Higher body mass index (BMI), metabolic syndrome, habitual snoring, and nocturia were independent risk factors for OSA; NYHA classes III and IV were independent risk factors for CSA in the HF patients. There was a high prevalence of SA in Chin

OBJECTIVES: Snoring is a common complaint in ENT and sleep medicine clinics, with significant social relevance and potentially indicating more serious conditions, such as upper airway resistance syndrome and obstructive sleep apnea. This prospective analytical study evaluated the impact of nasal surgery on reducing snoring duration in patients with a history of nasal obstruction.
METHODS: Thirty patients were selected and divided into two groups: Group 1, consisting of patients with complex septal deviations, nasal valve weaknesses, visible nasal structural changes, and a history of snoring;

RATIONALE: Measures of unstable ventilatory control (loop gain) can be obtained directly from the periodic breathing duty ratio on polysomnography in patients with Cheyne-Stokes respiration/central sleep apnea and can predict the efficacy of continuous positive airway pressure (CPAP) therapy.
OBJECTIVES: In this pilot study, we aimed to determine if this measure could also be applied to patients with complex sleep apnea (predominant obstructive sleep apnea, with worsening or emergent central apneas on CPAP). We hypothesized that loop gain was higher in patients whose central events persisted

The early use of continuous positive airway pressure ventilation has been shown to be effective and is recommended for patients with obstructive sleep apnea. The complications of continuous positive airway pressure ventilation are not well described. We report two cases of pneumocephalus following the use of continuous positive airway pressure ventilation after transsphenoidal surgery. One patient had an obstructive sleep apnea and the other suffered acute respiratory failure. In both cases, pneumocephalus caused major morbidity and required specific treatment and prolonged considerably hospit

In this paper, we studied 3 children with moderate hypertrophy of the palatine tonsils and adenoids and serious congenital diseases. The first patient was a 4-year-old girl with Fallot's tetralogy, the second was a 6-year-old girl with Goldenhar syndrome and ventricular septal defect, aortic stenosis and patent ductus arteriosus, and the third was a 3-year-old boy with cerebral palsy and epilepsy. We examined obstructive apnea during sleep in these patients using all-night polysomnogram or apnomonitor before surgical therapy. After tonsillectomy or adeno-tonsillectomy we confirmed significant

Clinicians are becoming more aware of the risks of sleep deprivation and unrecognized sleep-disordered breathing in hospitalized patients, most importantly in those patients planning to undergo surgical procedures. Polysomnography is difficult to perform in the hospital setting, such that actigraphy or urinary metabolites of melatonin are often used as surrogate measures, and show that sleep is markedly impaired. Patients in the medical intensive care unit with sepsis or requiring mechanical ventilation may show complete absence of the normal circadian rhythm pattern, and many centers have ini

BACKGROUND: Increased nasal resistance as measured by 4-phase rhinomanometry is associated with a paucity of apneas relative to hypopneas in obstructive sleep apnea (OSA) patients. The ratio of hypopneas to apneas for a given apnea hypopnea index (AHI) may influence treatment choice. This study aimed to investigate if OSA patients with an elevated hypopnea apnea ratio are associated with differences in acoustic rhinometry measurements.
METHODS: One hundred and thirty-nine (n = 139) OSA patients (AHI >5) were enrolled in this prospective case-control study (all male). OSA Diagnosis was establi

BACKGROUND: Upper airway obstruction (UAO) often remains undetected in infants and may result in an increased work of breathing, failure to thrive and fragmented sleep, potentially impairing neurocognitive development.
METHODS: In a prospective randomized crossover study, we evaluated our neonatal care team's subjective assessment of obstructive sleep apnea severity in infants with Robin sequence admitted to Tuebingen University Children's hospital (April 2021-May 2023). Assessments were done in supine and prone position using a standardized questionnaire. Ratings were compared with polygraph

Sleep-related breathing disturbances are exacerbated at altitude in patients with Obstructive Sleep Apnea (OSA). The objective of this case report was to determine if a portable auto-CPAP device effectively treated sleep apnea across different altitudes. We report the severity of sleep apnea from 60 to 12,000 feet high in a man with severe OSA (Apnea Hypopnea Index at diagnosis = 60 events/hour) during the 2017 Dakar rally over the Andes mountains. The man was equipped with a lightweight portable auto-CPAP device with a narrow window [6-8 cmH2O]. Pressures delivered and corresponding residual

INTRODUCTION: Down syndrome (DS) is a genetic form of Alzheimer's disease (AD), with a high prevalence of sleep disorders, but data in adults with DS and dementia are lacking. We aim to assess sleep in adults with DS across the AD continuum.
METHODS: We studied 78 healthy controls and 229 adults with DS (154 asymptomatic, 25 with prodromal AD, and 75 with AD) with subjective sleep measures and objective nocturnal polysomnography.
RESULTS: Adults with DS presented worse sleep quality and higher prevalence of unnoticed obstructive sleep apnea (OSA) than controls. Sleep disruption and OSA sever

Sleep-disordered breathing (SDB) is a constellation of breathing disorders that occur during sleep. Obstructive sleep apnea (OSA), the most common form, is characterized by complete or partial airway obstruction, hypoventilation, and central apneas, all of which lead to recurrent episodes of hypoxia, hypercapnia, sleep fragmentation, and elevated sympathetic tone. OSA occurs throughout the life span and is associated with significant cognitive, metabolic, and cardiovascular consequences that impair the quality of life. Building on observations that the upper airway collapses during sleep, obes

STUDY OBJECTIVES: To assess changes in Hospital Anxiety and Depression Scale (HADS) scores after continuous positive airway pressure (CPAP) in patients with obstructive sleep apnea.
METHODS: Consecutive patients attending the Alfred Health sleep clinic, diagnosed with obstructive sleep apnea, and prescribed CPAP were recruited. The primary outcome was a change in the HADS depression (HADS-D) and anxiety (HADS-A) subscales from the time of diagnosis to follow-up. Secondary analysis compared high (> 4 hours) and low (< 4 hours) CPAP adherence groups and change in depression cases, defined by HA

A 51-year-old man with type I Arnold-Chiari malformation presented with severe autonomic impairment. Polysomnography disclosed central and obstructive sleep apneas. During REM sleep apneas were accompanied by hypotension and led to convulsive nonepileptic attacks. Marked blood pressure oscillations and central apneas persisted after tracheostomy.

Objective.Sleep apnea has a high incidence and is a potentially dangerous disease, and its early detection and diagnosis are challenging. Polysomnography (PSG) is considered the best approach for sleep apnea detection, but it requires cumbersome and complicated operations. Thus, it cannot satisfy the family healthcare needs.Approach.To facilitate the initial detection of sleep apnea in the home environment, we developed a sleep apnea classification model based on snoring and hybrid neural network, and implemented the well trained model in an embedded hardware platform. We used snore signals fr

Central Sleep Apnea (CSA) is characterized by intermittent apneas and hypopneas during sleep that result from absent central respiratory drive. CSA occurs almost exclusively during non-rapid-eye-movement (NREM) sleep due to enhanced neuronal ventilatory drive during REM sleep that makes central apneas highly unlikely to form. A 45-year-old obese African American female presented with co-existing Obstructive Sleep Apnea (OSA) and CSA, not in the form of mixed or complex sleep apnea. Peculiarly, her CSA occurred only during rapid-eye-movement (REM) sleep, which is exceedingly rare. The patient's

Measuring asynchrony between the thorax and abdomen during sleep provides information about inspiratory effort and upper airway obstruction in children, aiding in the diagnosis of obstructive sleep apnea (OSA). This study aimed to define a normative range for thoracoabdominal asynchrony (TAA) in healthy children by studying polysomnographies of 113 children aged 10-13 years. The percentage of total sleep time (TST) spent with elevated TAA, > 40°, was measured, as well as obstructive apnea-hypopnea index (OAHI), sleep position, sleep stage and snoring. Subjects without measurable OSA (N = 94) w

Central sleep apnea is a disorder characterized by apneic episodes during sleep with no associated ventilatory effort. More commonly than not these apneas are seen in patients who also have obstructive and mixed events. Although patients with this disorder frequently complain of insomnia and depression, frank hypersomnolence is rarely encountered. As these complaints are common ones seen in numerous clinical situations, and since sleep studies are rarely conducted to investigate their etiology, the true incidence of central sleep apnea has not been determined. The etiology of central apnea rem
