Intentional nail ingestion with caecal impaction: colonoscopic removal using a cap-assisted technique.

Most foreign body ingestions in adult patients pass spontaneously while sharp objects often require endoscopic intervention. Endoscopic foreign body removals are usually performed in the upper gastrointestinal (GI) tract, with few reports involving the lower GI tract. We present a case of a woman with recurrent nail ingestion who presented with abdominal pain after swallowing multiple nails. Initial conservative management demonstrated progressive migration, with eventual clustering near the ileocaecal valve. In this case, a cap-assisted colonoscopy was successfully used to remove all ingested

Unusual Complication of Percutaneous Gastrostomy Tube Replacement.

Percutaneous endoscopic gastrostomy tubes are commonly used for long-term nutritional support in patients with impaired swallowing due to neurological conditions, malignancy, or critical illness. Although placement, removal, and replacement are generally safe, complications may occur. We report a case of complete gastric outlet obstruction due to unusually deep percutaneous endoscopic gastrostomy tube insertion during replacement in a 63-year-old man with esophageal cancer. Symptoms resolved after repositioning the tube. This case highlights the need for proper training and postreplacement ver

Case Report: Cholecystitis and gastric perforation caused by a fishbone and treated by laparoscopic surgery.

Rare cases of swallowed fish bones leading to cholecystitis and gastric perforation have been reported. Here, we present the case of a 71-year-old male patient who experienced 10 days of right upper quadrant pain after eating fish. Laparoscopic repair of gastric perforation and cholecystectomy was performed and successfully removed a fishbone of 3 cm in length from the region between the gallbladder cavity and the gastric antrum. The patient was 10 days after surgery and recovered well. No sign of recurrence was observed at the 3-month follow-up.

Motor Features of Parkinson's Disease.

The most characteristic motor feature of Parkinson's disease is bradykinesia. Other cardinal motor features of Parkinson's disease include tremor (particularly rest tremor), rigidity, and postural instability/gait changes. Additional motor features common to the disease include dystonia, postural abnormalities, speech and swallowing dysfunction, and levodopa-related dyskinesias. The pathophysiology of many of these motor features remains poorly understood. During the natural course of the disease, nonmotor features (discussed elsewhere) often precede the onset of motor features and meaningfull

Medical and surgical management of aorto-esophageal fistula after denture erosion due to esophageal stent placement.

This case reports on the management of a patient who initially presented to an outside hospital with an esophageal perforation secondary to swallowed dentures. The patient was subsequently treated with an esophageal stent that eroded the dentures into his thoracic aorta, creating an aorto-esophageal fistula. The patient underwent successful management of an aorto-esophageal fistula utilizing antibiotic and antifungal medications and endovascular therapy as the definitive surgical treatment of the aorta, followed by esophagectomy with foreign body removal and staged esophageal reconstruction. Segmental Megaesophagus Secondary to Extraluminal Esophageal Stenosis Caused by Transitional Seventh Cervical Vertebra and Supernumerary Ribs in a Goat.

A 4-year-old Nigerian Dwarf wether presented for chronic regurgitation and cervical swelling of three years duration. Physical examination revealed a large ventral cervical mass. The goat made repeated attempts to swallow and regurgitate, but the mass did not change significantly in size. Cervical radiographs revealed segmental esophageal dilation to the level of the thoracic inlet. CT revealed a transitional seventh cervical vertebra with fused right and left ribs, resulting in extramural esophageal compression and obstruction at the level of the thoracic inlet. This is the first report of a

A Functional Larynx Dissection Utilizing Mandibular Rotation: A Technical Report.

In gross anatomy labs, the larynx is usually visualized with either a sagittal hemisection or a posterior disarticulation approach. Both fail to maintain the functional in-situ relationships between the intact larynx and adjacent structures. This makes it challenging for students to understand how suprahyoid and infrahyoid muscles, pharyngeal muscles, the tongue, epiglottis, and intrinsic laryngeal muscles are all involved with airway protection and swallowing. We describe a novel approach to cadaveric larynx exposure using a pharyngotomy and mandibular rotation technique, which keeps the lary

Laparoscopy-Assisted Transgastrointestinal Endoscopic Retrieval of a Denture Impacted in the Esophagus: A Case Report.

We report the management of a complex case involving a 66-year-old male with significant medical comorbidities who inadvertently swallowed a denture, leading to unsuccessful endoscopic removal attempts. CT scans revealed an esophageal perforation. Following an initial unsuccessful conventional endoscopic attempt, we employed a laparoscopy-assisted transgastrointestinal endoscopic approach. This novel technique facilitated the successful retrieval of the denture without further esophageal damage, underscoring its utility in managing challenging esophageal foreign bodies. The patient's postopera

Successful Multidisciplinary Treatment of Severe Pyogenic Liver Abscess Caused by Fish Bone: A Case Report.

Pyogenic liver abscess (PLA) is a potentially fatal infection that can lead to sepsis and shock. Among the various causes of PLA, ingestion of foreign bodies such as fish bones is relatively rare. Unless there are specific symptoms such as painful swallowing, patients rarely remember having ingested foreign bodies, making it often difficult to identify the cause of PLA. In addition, the treatment strategy and perspectives for PLA caused by foreign bodies are controversial. Herein, we present a successful case of multidisciplinary treatment for sepsis due to a PLA caused by a fish bone with a l

Clinical Neurology in Practice: The Tongue (Part 1).

BACKGROUND: The tongue is an essential organ for the development of certain crucial functions, such as swallowing and language. The examination of the tongue can be very useful in neurology, as the various types of lingual alterations can lead to certain specific diagnoses, the tongue being a kind of "mirror" of some neurological function. REVIEW SUMMARY: In this study, we reviewed the literature on anatomy, physiology, and the various aspects of the examination of the tongue. CONCLUSIONS: Examination of the tongue should be an integral part of the clinical examination of the cranial nerves.

Drug Therapeutics Delivery to the Salivary Glands: Intraglandular and Intraductal Injections.

Salivary gland hypofunction and xerostomia following pathological conditions like Sjogren's syndrome or head and neck radiotherapy usually lead to tremendous impairment of oral health, speech, and swallowing. The use of systemic drugs to alleviate the symptoms of these conditions has been associated with various adverse effects. Techniques of local drug delivery to the salivary gland have grown enormously to address this problem properly. The techniques include intraglandular and intraductal injections. In this chapter, we will provide a review of the literature for both techniques while incor

[The consensus among experts on the diagnosis and treatment of pediatric vocal cord paralysis].

Pediatric vocal ford paralysis is a vocal cord movement disorder caused by damage to the pediatric laryngeal motor nerves. It is mainly characterized by voice, breathing, and swallowing difficulties, and in severe cases,it can lead to choking in affected children. Currently, the diagnosis and treatment of this condition pose a significant challenge for pediatric otolaryngologists, as the goal is to minimize damage to the vocal folds and laryngeal framework. In order to standardize the diagnosis and treatment of pediatric vocal cord paralysis, the Pediatric Otolaryngology Committee of the Chinese M

Maintenance Therapy With the Lowest Effective Dose of Oral Viscous Budesonide in Children With Eosinophilic Esophagitis.

Eosinophilic esophagitis (EoE) is an immune-mediated condition characterized by symptoms of esophageal dysfunction and an eosinophilic inflammation of the esophagus.1 Swallowed topical steroids represent one of the possible strategies for inducing and maintaining remission in EoE.2 To date, a validated maintenance strategy has yet to be defined, especially in children. The available evidence suggests decreasing the dose after a successful induction therapy.3 No study has reported the efficacy of a continuous progressive dose reduction; thus, it is unknown if all patients need to use the same d

Upper Gastrointestinal Functional and Motility Disorders in Children.

Children with disorders affecting the sensory and motor functions of the esophagus will present primarily with swallowing dysfunction, dysphagia, and chest pain, and those with disorders affecting the normal function of the stomach will present with symptoms like abdominal pain, nausea, and vomiting. Recent advances in the mechanisms of disease and technology have increased our understanding of gastrointestinal physiology and that knowledge has been applied to develop new diagnostic studies and therapeutic interventions. We present an overview of the clinical presentation, diagnosis, and treat

Dysphagia Lusoria: A Little Known Cause of Chest Pain.

Dysphagia lusoria is a congenital abnormality characterized by an aberrant right subclavian artery. It often presents as either an incidental finding on imaging or chronic dysphagia. We describe the case of a 66-year-old female who presented with severe chest pain, worse with swallowing, along with an ongoing globus sensation. She was found to have a negative cardiac workup for ischemia with a subsequent computed tomography angiogram (CTA) of the chest showing an abnormal right subclavian artery. We emphasize the unique diagnostic approach of this rare anatomical anomaly and its potential pres

Pediatric Unilateral Vocal Fold Immobility.

Vocal fold immobility in children can affect breathing, swallowing, and speech function. Although sometimes idiopathic, it is often caused by injury to the recurrent laryngeal nerve during cardiac surgery. A detailed history and physical examination can identify risk factors, which affect the rate of resolution and overall prognosis. Fiberoptic laryngoscopy can be used to confirm the diagnosis and evaluate laryngeal anatomy. Many treatment options exist to improve function and quality of life, including vocal fold injection and laryngeal innervation. [Pediatr Ann. 2021;50(7):e286-e291.].

Unilateral lingual nerve and hypoglossal nerve injury caused by a novel laryngeal mask airway: a case report.

Cranial nerve injury by a laryngeal mask airway is rare but a serious complication. The nerve injuries must be prevented during the intubation using a laryngeal mask airway. We report a female patient who complained of tongue numbness, slurred speech, and slight difficulty in swallowing solid food after a hand surgery. She was then diagnosed with unilateral lingual nerve and hypoglossal nerve injuries. Extreme head rotation, relatively small oral cavity, and wide rigid composition at the lower part of the novel laryngeal mask probably resulted in cranial nerve injury.

Redox-related gaseous mediators in the gastrointestinal tract.

Redox-related gaseous molecular species in the gastrointestinal tract are derived from the chemical oxidation-reduction reactions, enzymatic reactions, swallowing, and bacterial production. Recent studies have demonstrated the crucial roles of the microbiota and gaseous molecules in the pathogenesis of gastrointestinal inflammatory and functional diseases. Especially in the hypoxic condition of the large intestine, various bacteria produce acetic acid, methane, and hydrogen sulfide using hydrogen molecules generated by the fermentation reaction as an energy source. In this review, we summarize

Transoral Marsupialization of a Large Dermoid Cyst.

True dermoid cysts are rare congenital entities with a predilection for periorbital, floor-of-mouth, and submental areas in the head and neck region. They are usually asymptomatic unless they substantially enlarge, causing pain or difficulty in swallowing, breathing, speech, or vision, depending on their location. Conventionally, complete excision is the treatment of choice. The authors describe a transoral approach to marsupialize a large dermoid cyst, with a successful outcome and minimal complications. Using this technique, it is hoped that surgeons can minimize the risk of damaging vital s
