Enhancing Voiding Trial Adherence Using QR Code-Based Video Education.

IMPORTANCE: Voiding trials are critical for identifying postoperative urinary retention following pelvic surgery. However, inconsistent nursing execution due to confusing protocols or complex electronic order sets can lead to poor adherence, patient harm, and unnecessary catheter use. OBJECTIVE: The objective of this study was to determine whether providing nursing staff with quick response (QR) code-linked instructional videos improves adherence to postoperative voiding trial protocols. STUDY DESIGN: A pre-post quality improvement study was conducted over 3 months at 2 tertiary care hospita

Budget impact analysis of a multifaceted nurse-led intervention to reduce indwelling urinary catheter use in New South Wales Hospitals.

BACKGROUND: In hospitals, catheter acquired urinary tract infection causes significant resource waste and discomfort among admitted patients. An intervention for reducing indwelling catheterisations - No-CAUTI - was trialled across four hospitals in New South Wales, Australia. No-CAUTI includes: train-the-trainer workshops, site champions, compliance audits, and point prevalence surveys. The trial showed reductions on usual care catheterisation rates at 4- and 9-month post-intervention. This result was statistically non-significant; and post-intervention catheterisation rates rebounded between

Reducing catheter-associated urinary tract infections through best practice: Sherwood Forest Hospitals' experience.

A programme to standardise catheterisation practice was introduced in Sherwood Forest Hospitals NHS Trust in 2016, with the aim of reducing the incidence of catheter-associated urinary tract infections (CAUTIs). The initiative involved the use of a catheterisation pack (Bard®Tray). Within the first year following its introduction, the CAUTI rate had been reduced from 13.3% to 2.1% (between July 2016 and June 2017), which is a reduction of more than 80%. Standardisation has also brought cost savings for the Trust of about £33 000 a year. The Trust has maintained its standardisation approach and

Urinary Tract Infections.

Urinary tract infection (UTI) is the second most common bacterial infection in children and is considered a public health threat given the mounting rates of antibiotic-resistance among uropathogens. This article highlights recent encouraging developments in UTI research. Further work is necessary to translate the discoveries into accessible, cost-effective technologies that will aid clinicians in real-time decision-making.

Urinary tract infection due to anaerobic bacteria in a two-month-old infant.

The significance of anaerobic bacteria as a pathogen in urinary tract infection (UTI) in children is unclear. A two-month-old infant presenting with poor feeding received a diagnosis of polymicrobial anaerobic UTI by next-generation sequencing and was found to have obstructive uropathy. Anaerobic bacteria may be a cause of UTI in children with urinary tract obstruction.

[Misdirection of an Indwelling Urethral Catheter into the Ureter in a Female Patient with Neurogenic Bladder : A Case Report].

We report a case of misdirection of an indwelling urethral catheter into the ureter. An 86-year-old women with neurogenic bladder had been undergoing chronic indwelling urethral catheter exchange for 1 year. She was referred to our hospital owing to lower abdominal pain and gross hematuria. A computed tomographic scan showed an indwelling urethral catheter in the right ureter. Retrograde ureterography showed that the ureter had no leakage and the catheter was removed under fluoroscopic guidance. Urethral catheterization is a common procedure. We reviewed 23 cases of an indwelling urethral cath

Dysfunctional voiding: the importance of non-invasive urodynamics in diagnosis and treatment.

In Dysfunctional voiding, failure of the external sphincter-pelvic floor complex to relax during micturition results in bladder outflow obstruction with a spectrum of presentation from more benign lower urinary tract dysfunction including recurrent urinary tract infections, to significant upper tract pathology and end-stage renal failure. There is no underlying neurological or anatomical cause and the condition is postulated to be a largely learnt behavior. Diagnosis relies on non-invasive urodynamics and in particular uroflowmetry, plus or minus EMG, which is also used in biofeedback, the mai

Mayo Clinic Reduces Catheter-Associated Urinary Tract Infections Through a Bundled 6-C Approach.

The primary CAUTI reduction strategies of ensuring aseptic technique during catheter placement and reducing urinary catheter utilization were already in place at our institution. A multidisciplinary team approach, which entailed the use of QI methodology and engagement of frontline staff, resulted in the identification of additional strategies to reduce CAUTI. By implementing these strategies, we successfully reduced CAUTIs and have sustained this reduction through March 2016. The tools created during this project can be easily adapted for use at other institutions.

Balloon Dilation of the Ureter: A Contemporary Review of Outcomes and Complications.

PURPOSE: During ureteroscopy ureteral balloon dilation may be necessary to allow for passage of endoscopic instruments or access sheaths. We assessed the efficacy and complications associated with ureteral balloon dilation. MATERIALS AND METHODS: We retrospectively reviewed the records at 2 institutions from 2000 to 2012 to identify patients who underwent ureteral balloon dilation during ureteroscopic treatment of upper tract stones. An 18Fr balloon dilator was used in all cases. Patients with documented ureteral stricture, radiation therapy or urothelial cancer were excluded from analysis. P

Staff Nurses Eliminated Catheter-Associated Urinary Tract Infections in a Neurosurgical Intensive Care Unit.

Catheter-associated urinary tract infections (CAUTIs) are among the leading causes of health care-associated infections in neurosurgical populations. Successful reduction of CAUTIs involve the development of staff nurses as front-line change agents equipped with preventative strategies, educational interventions, and sustainable maintenance for positive patient outcomes.

Urethral (indwelling or intermittent) or suprapubic routes for short-term catheterisation in hospitalised adults.

BACKGROUND: Indwelling urethral catheters are often used for bladder drainage in hospital. Urinary tract infection is the most common hospital-acquired infection, and a common complication of urinary catheterisation. Pain, ease of use and quality of life are important to consider, as well as formal economic analysis. Suprapubic catheterisation can also result in bowel perforation and death. OBJECTIVES: To determine the advantages and disadvantages of alternative routes of short-term bladder catheterisation in adults in terms of infection, adverse events, replacement, duration of use, particip

Laparoscopic Mitrofanoff continent catheterisable stoma in children with spina bifida.

BACKGROUND: In 1980, Mitrofanoff described the creation of an appendicovesicostomy for continent urinary diversion. This procedure greatly facilitates clean intermittent catheterisation in patients with neurogenic bladder. The purpose of our study was to determine the clinical efficacy of the laparoscopic Mitrofanoff catheterisable stoma for children and adolescents with spina bifida. MATERIALS AND METHODS: Review of hospital records revealed that 11 children with spina bifida underwent a laparoscopic Mitrofanoff procedure with at least 1-year of follow-up. A four-port transperitoneal laparos
