In this cross-sectional study a comparison was made of rheumatoid factor (RF) isotypes in 203 RF positive patients with arthritis. Of these, 129 had rheumatoid arthritis (RA) and 74 a milder disease that would formerly have been classified as probable RA. The majority (74%) of the RA patients had elevations of two or three RF isotypes compared with only 34% of the patients with the milder form of arthritis. A striking feature was that combined elevation of IgM RF and IgA RF was found in 67% of the RA patients compared to only 20% of the patients with milder arthritis who most frequently had an…

78 patients in whom the cause of a monoarticular arthritis remained unclear after an initial workup were contacted for a follow-up interview (and an additional clinical examination in 64 cases) after 6 to 11 years (mean 8 years). The mean age at the onset of symptoms was 39 years, with 51% of the patients presenting in the range between 20 and 40 years. There was a slight male preponderance (56%). Large joints, mainly the knee, and less often the wrist, ankle or hip, were affected in 79%. Finger, toe and other small joints were involved in only 21%. During the course of the disease 14% of the …

Increased intestinal permeability secondary to treatment with non-steroidal anti-inflammatory drugs (NSAIDs) and raised levels of anti-gliadin antibodies (AGA) have been reported in adults with rheumatoid arthritis. We have therefore retrospectively investigated the presence of serum AGA of the IgA and IgG classes in 70 patients with juvenile chronic arthritis (JCA). Serum IgA (but not IgG) AGA were found to be higher in JCA patients than in controls (6.2 +/- 8.7 vs 2.1 +/- 1.5 AU/ml; p less than 0.0001). This finding was observed independently of the JCA onset subtype or disease activity; how…

As a result of better patient selection, implant design, and surgical technique, the long-term results of total elbow arthroplasty in patients with rheumatoid arthritis has improved significantly over the past 10 years. The semiconstrained hinge implant is used in the presence of severe bone and ligament destruction. The non-constrained surface replacement requires intact bone and ligamentous supports. Best results are obtained if surgery is undertaken before severe soft tissue contractures, muscle atrophy, and neurologic disability of the involved extremity develop.

Two patients suffering from systemic onset juvenile chronic arthritis (JCA) with a polyarticular course developed an acute illness shortly after commencing a new second line drug (gold and penicillamine). The clinical picture consisted of fever, rash, lymphadenopathy and hepatomegaly with elevated liver enzymes and hematological changes. The possible relationship of the disease to second line drugs is discussed.

Between 1978 and 1988 a total of 27 operations were performed on 26 patients for cervical myelopathy due to rheumatoid disease in the subaxial spine. Three different causes were recognised: the first group had cord compression due to subluxation of the cervical spine itself (6 patients); the second had cord compression occurring from in front, with rheumatoid lesions of vertebral bodies or discs (6); the third had compression from behind the cord due to granulation tissue within the epidural space (14). Group I was treated by closed reduction of the subluxation followed by surgical fusion eith

IgM, IgG and IgA class antibodies against Yersinia, Salmonella, Campylobacter and Borrelia were determined by enzyme-linked immunosorbent as … r Salmonella, Campylobacter and Borrelia were 7.0, 6.2 and 1.8%, respectively. Patients with entero-arthritis or clinically typical reactive arthritis who had not had gastrointestinal or urogenital symptoms previously had the highest prevalence of the microbial antibodies (67.6 and 40.7%, respectively). These findings indicate that arthritis may often have …

A 54-year-old woman with malignant rheumatoid arthritis was admitted with acute congestive heart failure and pulmonary hypertension. The precipitating antibody to nuclear ribonucleoprotein (nRNP) was highly positive. Her hands and feet showed typical rheumatoid deformities and Raynaud's phenomenon. In a few days her feet became to necrosis due to vasculitis. "Pulse" methylprednisolone and prednisone therapy resulted in normalization of seropositivity, and left ventricular ejection fraction with symptoms of congestive heart failure, and the elimination of ECG abnormalities. Recognition of sympt…

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1991eng
We present a case of rheumatoid arthritis that affected the right shoulder and was associated with chondromatosis and multiple loose body formation. The arthritis was treated arthroscopically with satisfactory results after a follow-up period of 15 months. In our case, arthroscopic debridement and partial synovectomy not only relieved the pain but also improved the range of motion the night after surgery. The multiple loose bodies irritating the synovium and causing effusion, crepitus, and locking were also removed. One may need to change portals of the scope and suction cannula to remove loos…

Of 20 patients who underwent arthroscopic synovectomy for refractory chronic Lyme arthritis of the knee, 16 (80%) had resolution of joint inflammation during the first month after surgery or soon thereafter, and they have remained well during the 3-8-year followup period. Three of these 16 patients who were more disabled preoperatively, still had mild functional limitation at long-term followup. The remaining 4 patients (20%) had persistent or recurrent synovitis. We conclude that arthroscopic synovectomy is effective in treating chronic Lyme arthritis in patients in whom the disease does not …

A juvenile rhesus macaque (Macaca mulatta) developed a symmetrical erosive polyarthritis involving both large and small diarthrodial joints. Neither an infectious nor a metabolic etiology could be determined. This case shares many clinical and pathological features with the polyarticular form of human juvenile rheumatoid arthritis.

Sera from 58 children with juvenile rheumatoid arthritis were examined for the presence of antibodies to DNA topoisomerase II. Eight sera were reactive in immunoblotting with purified human topoisomerase II and a protein encoded by a cloned cDNA expressed in Escherichia coli which represents the carboxy-terminal domain of the human enzyme. In addition, the sera detect topoisomerase II in mitotic chromosomes and chromosome scaffolds. Five of the sera bind to the native enzyme in solution and deplete such solutions of the active enzyme. All eight sera also contain antibodies to nuclear antigens …

The aim of this investigation was to determine the craniofacial characteristics of children affected with juvenile rheumatoid arthritis. The analysis concerned the profile cephalometric roentgenograms of 11 children of both sexes aged between five and ten years. Statistically significant differences between affected and healthy children were found for saddle angle, articular angle, Björk's sum of posterior angles and maxillo-mandibular angle.

The knowledge about rheumatic diseases in Latin American children is scanty. The features of Juvenile Rheumatoid Arthritis (J.R.A.) in the islands of the Caribbean had not been described. In this retrospective study, fifty cases of J.R.A. in a pediatric population of Puerto Rico and the Virgin Islands followed at a formal pediatric rheumatology service in San Juan, Puerto Rico, were demographically and clinically characterized. There was a female predominance of 1.4:1. The peak of occurrence was at 1-2 years of age. Pauciarticular onset was the most common through all ages. Antinuclear antibod…

Juvenile rheumatoid arthritis (JRA) is the most common pediatric rheumatic disease and is a leading cause of childhood disability. Physical therapists play a crucial role in the treatment of these children and serve as essential members of the interdisciplinary treatment team. An understanding of the etiology and background of this disease is critical to appropriate evaluation, goal setting, and treatment planning. This review article will provide an overview of the epidemiology, immune system pathophysiology, and clinical characteristics of JRA. Physical therapy principles of care, evaluation…

Scedosporium (monosporium) apiospermum is the most common causative agent of maduro mycosis. Recent research reports claim that this fungus can also induce arthritis. We report on the disease in a woman from a rural area who was admitted to our clinic because of arthritis symptoms. We were able to detect Scedosporium apiospermum, i.e., it was probably a fungus-induced arthritis which had developed after an intraarticular steroid injection.
