Distracted cervical spinal fusion for management of caudal cervical spondylomyelopathy in large-breed dogs.

Using an autogenous bone graft (obtained from the iliac crest), 4-mm cancellous bone screws, and polymethylmethacrylate, a distracted cervical spinal fusion technique was performed on 10 dogs with myelographic evidence of caudal cervical spondylomyelopathy. All dogs had evidence of dynamic soft tissue spinal cord compression, as indicated by flexion, extension, and traction myelographic views. Of the 10 dogs, 4 previously had undergone surgery by use of ventral slot or cervical disk fenestration techniques, and their neurologic status had deteriorated after the original surgery. Preoperative n

Cervical spine: MR imaging with a partial flip angle, gradient-refocused pulse sequence. Part I. General considerations and disk disease.

A magnetic resonance imaging pulse sequence with a short repetition time (TR), short echo time (TE), partial flip angle, and gradient refocused echo was evaluated for the detection of cervical disk disease in a prospective study of 90 patients. These parameters were manipulated to adjust signal-to-noise ratio (S/N) and contrast: flip angle (3 degrees-18 degrees), TR (22-60 msec), and TE (12.5-25 msec). Flip angle had the greatest effect on S/N and contrast; its effect differed between axial and sagittal imaging. Cerebrospinal fluid S/N reached a peak at a smaller flip angle in sagittal imaging

Traumatic spondylolisthesis of the axis in a patient presenting with torticollis. A case report.

In a 34-year-old man with an acute traumatic spondylolisthesis (ATS) of the axis, the presenting physical sign was severe torticollis associated with reactive spasm of the right sternomastoid muscle. Computed axial tomography (CAT) was useful both in assessing the nature of ATS (which involved more displacement of the axis fracture and adjacent soft-tissues on the right side of the neck) and in ascertaining reduction, realignment, and healing. The reduction was sustained by halo-vest immobilization.

Cervical intervertebral disc calcification in children.

Intervertebral disc calcification in children is a rare occurrence. The clinical symptoms and signs are distinctively confined to the cervical area with pain, limitation of motion, and torticollis. Long tract signs or radicular involvement are extremely unusual. CT scan and cervical spinal X-ray films shown the calcification to be in the nucleus pulposus with anterior or posterior mild protrusion into the spinal canal. Recovery without neurological sequelae is the rule in most of the pediatric cases with conservative treatment. Intervertebral disc calcification does not necessarily disappear w

[A diagnostic sign in so-called epicondylitis humeri radialis].

With a simple test - the "finger-snapping-test" - it is possible to narrow down the etiology of lateral epicondylalgia of the humerus. On the basis of a group of 196 patients it is shown that a positive result of this test is characteristic for an enthesopathy of the extensor carpi radialis brevis muscle, whose insertion is at the radial epicondyle. If the test is negative it must be assumed that the pain is caused by spondylogenic, arthrogenic, or neurogenic phenomena. The importance of the test for the therapeutic approach and with regard to the indication for operation after Hohmann ist poi

Dysphagia and micrognathia in a patient with juvenile rheumatoid arthritis.

This report describes the case of a 35-year-old man with juvenile rheumatoid arthritis. The patient experienced swallowing difficulties that were initially thought to be a result of the disease affecting the cervical spine. A maxillofacial examination, however, showed a total loss of the mandibular condyles, short rami and bodies, and a retrusion of the jaw. This was considered to be the main cause of the dysphagia, and not a glossopharyngeal nerve compression. The surgical-orthodontic management of these difficult cases is discussed.

The correlation of myelographic root sleeve deformity, uncovertebral spondylosis and radiculopathy.

Myelographic root sleeve deformities, uncovertebral spondylosis and myelopathy were correlated in 56 patients. Good correlation between root sleeve deformities and uncovertebral spondylosis was found. However, in some cases with spinal canal stenosis root sleeve deformities were found without corresponding bone changes. The correlation of radiculopathy to root sleeve deformity and uncovertebral spondylosis was 61 and 67%, respectively. Radiologic changes at an adjacent root level were often seen in patients without clinically correlated radiculopathy. The myelographic changes were verified at

Injuries to the neck and upper extremities of dancers.

In general, injuries to dancers involving the cervical spine and upper extremities are quite mild and cause only limited disability. When such injuries do not involve specific elements of the musculoskeletal system, their origin may be in the more subtle neurovascular structures of the neck. The importance of a thorough examination is emphasized.

[The uncus exostoses syndrome and its treatment by means of the decompression operation of the vertebral artery (author's transl)].

In degenerative changes of the mobile segments of the cervical vertebral column, deformations of the uncovertebral gaps (joints), resembling exostoses, with a lateral or dorsolateral spread, are frequently seen. This involves irritation of neighbouring structures, such as the arteria and vena vertebralis including the nervus vertebralis or the corresponding nerve roots. This produces a disease pattern with a cervico-encephalic and cervico-brachial mixed pattern of symptoms. Angiography of the a. vertebralis proves the localisation and extent of the disturbance. The decompression of the a. vert
