Lactate-albumin ratio as an early predictor of mortality in critically ill children: A prospective cohort study.

Early risk stratification in critically ill children relies on severity scores such as PRISM-III and PELOD-2, requiring multiple variables. The lactate-albumin ratio (LAR), a two-parameter marker, shows prognostic value in adult and paediatric critical illness. This study evaluated serial LAR for predicting PICU mortality and compared its performance at admission against PRISM-III and PELOD-2. This prospective cohort study enrolled 335 of 563 screened children aged 1 month to less than 13 years admitted to a tertiary-care PICU in South India over 12 months. Serum lactate and albumin were measu

The impact of COVID-19 on all-cause mortality and life expectancy: findings from a population-based cohort in uMkhanyakude district, northern KwaZulu-natal.

BACKGROUND: Despite the comprehensive death reporting system in South Africa, significant gaps persist in our understanding of the pandemic's impact in rural areas. METHODS: We computed monthly death counts stratified by demographic factors from a population-based demographic surveillance cohort in rural KwaZulu-Natal. We fit seasonally adjusted negative binomial regression models to estimate excess mortality during the COVID-19 pandemic period (5 March 2020 to 31 December 2021) compared to the pre-pandemic period (1 January 2015 to 4 March 2020). We then assessed the relative risk of death b

Excess mortality and underlying causes of death during the COVID-19 pandemic in rural Bangladesh: insights from the Matlab health and demographic surveillance system.

BACKGROUND: Bangladesh, home to 165 million people, reported its first COVID-19 case in March 2020. This prompted a range of public health measures to control the epidemic. However, limited access to COVID-19 testing and incomplete or inaccurate death registration likely obscured the pandemic's true impact. We use longitudinal data from the Matlab Health and Demographic Surveillance System (HDSS) in Bangladesh to assess excess mortality and underlying causes of death during the COVID-19 pandemic. METHODS: We analysed mortality among 299,775 individuals residing within the Matlab HDSS catchmen

Socio-economic inequalities in all-cause mortality during the COVID-19 period in north-western Tanzania, 2018-2021.

BACKGROUND: Evidence suggests that the COVID-19 pandemic has exacerbated social and demographic inequalities in the communities through pathways of unequal exposure, vulnerability, and susceptibility. In Tanzania, evidence on COVID-19-related mortality is limited to health facility data, with little to no information on the mortality patterns in the general population. This study assessed sociodemographic inequalities in all-cause mortality during the COVID-19 period in north-western Tanzania. METHODS: We utilized available longitudinal data from the Magu Health and Demographic Surveillance S

Refractory Chylothorax: Descriptive Analysis and Predictive Model in Children with Postoperative Chylothorax.

Chylothorax following pediatric cardiac surgery increases morbidity and mortality. The clinical outcomes of patients with chylothorax with prolonged drainage compared to prompt resolution have not been described. This is a retrospective cohort study across eight United States pediatric cardiac intensive care units (ICU). Patients < 18 years old treated for chylothorax within 30 days of cardiac surgery were included, excluding Fontan palliations. Patients with chest tube duration ≥ 14 days were classified as long chylothorax (LC) vs. < 14 days as short chylothorax (SC). Univariable and multivar

Center Volume Not Associated with Survival Benefit of Inter-Hospital Transfer for Pediatric Cardiac Surgery.

To evaluate the relationship between center volume and inpatient mortality among patients transferred to another hospital for pediatric cardiac surgery using contemporary real-world data. The Kids' Inpatient Database (KID) was queried for cardiopulmonary bypass (CPB) cases for the years 2016 and 2019. Hospitals were divided into three groups based on terciles of volume: low: ≤ 103 cases/year, mid: 104-194 cases/year, and high: > 194 cases/year. Multilevel regression models were created to evaluate the association between volume and inpatient mortality for transferred patients, both for the ent

Analgesia, Sedation, and Neuromuscular Blockade in Pediatric Severe Traumatic Brain Injury: Secondary Analysis of the "Approaches and Decisions in Acute Pediatric TBI Trial" (ADAPT).

BACKGROUND: Sedative, analgesia, and neuromuscular blocking (NMB) medications may be necessary in the acute management of pediatric severe traumatic brain injury (sTBI), yet limited data exist to guide their use. We aimed to describe the use of continuous infusions of these medications in children with sTBI, to determine temporal trends during the first week of management, and to evaluate associations with in-hospital mortality. METHODS: We conducted a secondary analysis of the Approaches and Decisions in Acute Pediatric Traumatic Brain Injury Trial (NCT04077411, 2014-2017), a prospective mul
