[The theory-practice problem of blood pressure assessment].

The theoretical basis for assessment of blood pressure described in the guidelines of the ISH-ESC and in JNC 7 ignores the variability of blood pressure and is therefore not applicable in general practice. Although the guidelines do not meet the daily requirements, they are not questioned by the physician, and are therefore confusing both physicians and millions of patients taking self-recordings. The unsolved problems are: The confidence interval for the single readings and the mean values are ignored. For this reason, the assignment of the patient to the different categories of hypertension

Superficial fungal infections.

Superficial fungal infections arise from a pathogen that is restricted to the stratum corneum, with little or no tissue reaction. In this Seminar, three types of infection will be covered: tinea versicolor, piedra, and tinea nigra. Tinea versicolor is common worldwide and is caused by Malassezia spp, which are human saprophytes that sometimes switch from yeast to pathogenic mycelial form. Malassezia furfur, Malassezia globosa, and Malassezia sympodialis are most closely linked to tinea versicolor. White and black piedra are both common in tropical regions of the world; white piedra is also end

Paraneoplastic Cushing's syndrome associated to locally advanced thymic carcinoid tumor.

BACKGROUND: Thymic carcinoid is a frequent cause of Cushing's syndrome due to ectopic adrenocorticotropin secretion. Histology and immunohistochemistry allow differential diagnosis from other epithelial thymic tumors, such as thymomas and thymic carcinomas. The term used to name this tumor is confusing, since it is a malignant neuroendocrine neoplasm, and therapeutic approaches need to bear that in mind. CASE REPORT: Unlike most cases of thymic carcinoid associated to Cushing's syndrome that had distant metastases at diagnosis, we report a 50-year-old male who presented with Cushing's syndrom

Parkinson's disease: the treatment of drug-induced hallucinations and psychosis.

Drug-induced psychosis is one of the most disabling complications of advancing Parkinson's disease. It has also been one of the most difficult to treat. Clozapine was the first medication shown to be safe and effective in this setting, and it remains the standard by which newer atypical antipsychotics are measured. However, due to the small but significant risk of agranulocytosis and the need for frequent blood testing, alternatives have been sought. Risperidone, olanzapine, and quetiapine are new atypical antipsychotics that have each been proposed as an alternative to clozapine, but the lite

Motor neuron disease: classification and nomenclature.

The classification and nomenclature of motor neuron disease, whether sporadic or familial, is confused. For example, both the sporadic and familial motor neuron diseases are phenotypically heterogeneous and, in familial ALS, phenotypic heterogeneity correlates only weakly with different underlying mutations in the SOD1 gene. We propose a classification which is based on underlying causative mechanisms, where these are known, but which also recognizes different clinical phenotypes when the cause is unknown. This classification is flexible, and allows reattribution of clinical syndromes when the

A familial case of P-ANCA glomerulonephritis presenting in a father and daughter.

Antineutrophil cytoplasmic antibodies (ANCA) have proved to be useful serological markers for a subset of vasculitic diseases, including Wegener's granulomatosis, microscopic polyangiitis, and the Churg-Strauss syndrome. The pathogenesis of the ANCA vasculitides remains less clear, including what role, if any, genetic factors play in the expression of ANCA-associated diseases. Familial cases of systemic vasculitis have been reported, and a number of studies have addressed HLA associations of Wegener's and microscopic polyangiitis, but the results have been confusing and inconsistent. We report

Type I posterior laryngeal clefts.

Posterior laryngeal clefts (PLCs) are described in the literature as rare laryngeal abnormalities. The authors believe type I clefts are much more common than previously reported. In two busy pediatric tertiary care centers, such clefts are the second most common congenital laryngeal finding at rigid endoscopy, second only to laryngomalacia. PLCs frequently present with symptomatology that can be attributed to other common disease processes and are often undiagnosed unless the surgeon maintains a high index of suspicion and specifically examines the posterior glottis by palpation during microl

Benign mimics of prostatic adenocarcinoma on needle biopsy.

Various benign processes can mimic prostatic adenocarcinoma on needle biopsy. These processes include glandular lesions such as adenosis, atrophy, VMGH, and BCH; inflammatory conditions such as acute and chronic or granulomatous prostatitis; and the effects of therapy such as external beam radiation or androgen deprivation. Normal benign prostate tissues including seminal vesicles, paraganglia, and ganglion cells may also be confused histologically with prostatic adenocarcinoma in needle biopsy specimens. With careful attention to architectural and cytologic features, these lesions can be read

Limitations of cognitive status exams: a case-based discussion.

Despite their extensive use in psychiatric and medical settings, brief mental status examinations have significant limitations that are easily overlooked in the pressure-cooker environments within which they are commonly used. Although undoubtedly of value as quick screening devices, the sheer brevity of these instruments all but guarantees limited validity. Brief examinations perform best with grossly impaired cases, alerting clinicians to the fact that something is badly amiss in patients who are significantly confused, disoriented, aphasic, or otherwise severely impaired. Very poor scores a

Tuberculous meningitis in patients with nasopharyngeal carcinoma.

Mycobacterial infection has been recognized as a complication in patients with malignancy. Tuberculous (TB) meningitis has not been reported in patients with nasopharyngeal carcinoma (NPC); it may have been overlooked or confused with the underlying malignancy or meningitis caused by other microorganisms. We describe the occurrence of culture-proven TB meningitis in 2 NPC patients. The time lag between the diagnosis of NPC and the occurrence of TB meningitis was 4 years in 1 patient and 6 years in the other. In both patients, the diagnosis of TB meningitis was delayed; they were initially trea

[Acute schistosomiasis (Katayama fever)].

Acute schistosomiasis (Katayama fever) may present with a broad spectrum of symptoms three to six weeks after primary infection by Schistosoma (S) mansoni, S. japonicum or, more rarely, S. haematobium. The acute phase of schistosomiasis is frequently confused with other feverish diseases. It occurs almost exclusively in nonimmune visitors to endemic areas. We describe seven cases of acute S. mansoni infection. The pathogenesis, clinical features, diagnosis and treatment are briefly discussed. Katayama fever should be considered in patients returning from endemic areas with fever and eosinophil

Giant cell tumour of the seventh cervical vertebra.

A 43-year-old man presented with bubbly expansion and rarefaction of the body of the seventh cervical vertebra. The lesion involved the right side of the vertebral arch and had a soft-tissue component extending mainly anterior to the vertebra. Histopathologic examination of the specimen obtained by open biopsy revealed a giant cell tumour. Giant cell tumours of the spine, excluding the sacrum, are rare. Radiographically, they may be confused with metastatic carcinoma, plasmacytoma, lymphoma, chordoma and even benign lesions, particularly aneurysmal bone cyst and brown tumour of hyperparathyroi

Can absence status epilepticus be of frontal lobe origin?

Five women with an unclassifiable nonconvulsive status epilepticus (NCSE) characterized by young age at onset, prolonged confusions, focal motor seizures, and both generalized spike-and-wave discharges and focal epileptic discharges on the EEG were studied with video-EEG monitoring. Electrographically, the NCSE originated from the left frontal lobe in 4 patients, and the left hemisphere with multifocal seizure discharges in 1 patient. Focal motor seizures seemed to originate from the left hemisphere in all 5 patients, particularly from its anterior part in 3 of them. Results show that the NCSE…

Campylobacter jejuni enterocolitis causing peritonitis, ileitis and intestinal obstruction.

Patients with Campylobacter enterocolitis may come to laparotomy due to the severity of abdominal symptoms and signs, although only two patients with intestinal inflammation have been described and in neither was the histopathology documented. A case of a 52-year-old male who had a typical diarrhoeal illness of Campylobacter enterocolitis diagnosed on stool culture is reported. Despite appropriate treatment he developed signs and symptoms consistent with small intestinal obstruction. Laparotomy revealed peritonitis and thickened distal ileum with transmural inflammatory changes on histopatholo

Infected urachal remnants in the adult: case report and review.

Abnormalities of the urachus in adults are uncommon. Urachal tract remnants that abnormally remain patent are subject to infection. Urachal infection is frequently confused with a wide spectrum of midline intraabdominal or pelvic inflammatory disorders. Because the literature on urachal infection is primarily limited to articles in urology and surgical specialty journals, many physicians may not be familiar with the varied clinical manifestations. We describe a case of infection of a patent urachus in an adult and review the embryology and anatomy of the urachus as it relates to clinical prese

Constipation--a sign of a disease to be treated surgically, or a symptom to be deciphered as nonverbal communication?

Constipation is not a sign but a symptom, which is not measurable scientifically. It has emotional components and must be dealt with in a holistic manner. The scientific approach aims only at the physiological derangement and serves to place the complaint in a more objective perspective. An algorithm can be constructed to select treatment including surgery, but there is no gold standard at present because the natural history of the symptoms is unknown. Most physicians confuse normality with epidemiology, neglecting in the process the evolution of mankind in terms of behavior. Constipation now

[Recurrent unilateral pulmonary edema due to left heart failure].

Unilateral pulmonary edema is uncommon and is usually associated with prolonged surgical procedures or rapid evacuation of a hydro- or pneumothorax. Unilateral pulmonary edema due to left heart failure in the absence of known lung disease is rare. It is therefore not readily recognized and is often confused with other unilateral alveolar or interstitial infiltrates. We describe 2 patients, a 69-year-old man and a 78-year-old woman, who had repeated episodes of unilateral pulmonary edema due to left heart failure. In both cases several other diagnoses were considered, but the cardiac origin of

The spectrum of cavitary optic disc anomalies in a family.

The current classification of cavitary optic disc anomalies including the morphologically related entities--optic nerve pit, morning glory disc anomaly, coloboma of the optic nerve, and retinochoroidal coloboma involving the optic nerve--is inexact and confusing. Traditionally, these disc abnormalities have been regarded as distinct morphologic anomalies. Thirty-five members of a five-generation kindred with autosomal dominantly inherited optic disc anomalies were examined. Observed abnormalities in this pedigree comprised a spectrum of morphologic variants ranging from large anomalous discs t

The validity of the Bender-Gestalt emotional indicators.

We evaluated the concurrent validity of the revised Bender-Gestalt emotional indicators (Koppitz, 1975) among three groups of children in the 7- to 10-year age range: adjustment disorder, behavior disorder, and normal control. The concurrent validity of the emotional indicators total score was demonstrated. Notably, with the developmental level controlled, both clinical groups produced significantly more emotional indicators than the control group, although the two clinical groups could not be differentiated. Nominal-data analyses of the 13 individual emotional indicators indicated that three

Complete interruption of the aortic arch in a dog.

Complete interruption of the aortic arch was diagnosed by angiocardiography in a dog with exercise intolerance, a cardiac murmur, polycythemia, and cardiomegaly. The defect was accompanied by a patent ductus arteriosus, ventricular septal defect, and subaortic stenosis. The dog was euthanatized, and the clinical diagnosis was documented by gross dissection of the heart. The site of aortic interruption was between the left carotid and left subclavian arteries, which is the most common location in human beings. The similarity with the pathologic findings of interrupted aortic arch in human being
