BACKGROUND: Despite recent advances in management and preventive strategies, high rates of first line antibiotics treatment failure and case fatality for Severe Community Acquired Pneumonia (SCAP) continue to occur in children in low and middle-income countries. This study aimed to identify the predictors and outcome of first line antibiotics treatment failure among children under-five years of age with SCAP admitted at Bugando Medical Centre (BMC) in Mwanza, Tanzania.
METHODS: The study involved under-five children admitted with SCAP, treated with first line antibiotics as recommended by WHO

The imidazole-based H3R antagonist 2-18 with high in vitro H3R antagonist affinity, excellent in vitro selectivity profile, and high in vivo H3R antagonist potency was tested for its anticonvulsant effect in maximal electroshock (MES)-induced convulsions in mice having valproic acid (VPA) as a reference antiepileptic drug (AED). Additionally, H3R antagonist 2-18 was evaluated for its reproductive toxicity in the same animal species. The results show that acute systemic administration (intraperitoneal; i.p.) of H3R antagonist 2-18 (7.5, 15, 30, and 60 mg/kg, i.p.) significantly and dose depende

BACKGROUND: High risk newborns are most vulnerable to develop neuro-developmental delay (NDD). Early detection of delay in this group and identification of associated perinatal factors and their prevention can prevent disability in later life.
DESIGN AND METHODS: Observational cohort study. Field based tracking and neuro-developmental screening of high risk newborns discharged between January 2010 to June 2012 from a district Hospital in India was conducted by a team of developmental specialists, using standardized tools like Denver Developmental Screening Tool II, Trivandrum Developmental Sc

Severe myoclonic epilepsy in infants (SME) is one of the most malignant epileptic syndromes recognized in the latest classification of epileptic syndromes. The clinical details and electroencephalographic (EEG) characteristics have been elucidated by Dravet et al. The diagnosis of SME depends largely on the combination of clinical and EEG manifestations at different ages, of which the presence of myoclonic seizures appears to be the most important. However, because of the inclusion of different types of myoclonic attack and the lack of strict criteria for diagnosing SME, there has been some co

BACKGROUND: The purpose of neuroimaging of a patient with new onset of seizures is to demonstrate cause and explore the prognosis. It was recently recommended that emergency brain computed tomography (CT) be performed only in adult seizure patients with an increased likelihood of life-threatening lesions, i.e., those with new focal deficits, persistent altered mental status, fever, recent trauma, persistent headaches, history of cancer, history of anticoagulation, or suspicion of acquired immunodeficiency syndrome. The objective of this study was to determine the diagnostic utility of emergenc

The objectives of this study were to evaluate the possible mechanisms involved in prolongation of bleeding time in pre-eclamptic patients receiving a magnesium sulfate infusion to prevent convulsions. Eighteen pre-eclamptic patients near term or at term (4 cases 33 to 35 weeks; the remainder > 36 weeks) were studied. Fifteen of them received magnesium sulfate infusion; 3 did not and served as controls. Bleeding time (modified Ivy method with Surgicutt), platelet count, platelet aggregation pattern, as well as serum arachidonic acid metabolites [thromboxane B2 (TxB2) and 6-Keto-prostaglandin F1

Posterior reversible encephalopathy syndrome (PRES) is a neurological condition associated with seizures, visual disturbances and altered mental status. It is commonly linked to immunosuppressive therapies such as ciclosporin, widely used in recipients of a haematopoietic stem cell transplant (HSCT). Neuroimaging, especially MRI, is the most important diagnostic tool for PRES, as it typically shows bilateral and symmetrical involvement of the occipital and parietal regions with white matter oedema.Electroencephalography may be useful for the detection of (non-convulsive) epileptic seizures, st

Chandipura virus (CHPV) is a single-stranded negative-sense RNA virus of the family Rhabdoviridae. CHPV is transmitted mainly through infected sandflies. CHPV paediatric encephalitis reported in 2003-2004 in central and south-western parts of India had a case fatality rate of ∼70%. CHPV infection leads to high-grade fever, vomiting, altered sensorium, generalised convulsions, decerebrate posture and coma. Neuroinflammation is the hallmark of CHPV infection and has a pronounced effect on cerebral and brainstem regions. Currently, there are no vaccines or treatments available for CHPV infection.

Hypothyroidism is a common endocrine condition in the adult and pediatric population. Levothyroxine, used for the treatment of hypothyroidism, is one of the easily available drugs in any household as either parents or sometimes children are on levothyroxine treatment. Accidental levothyroxine ingestion is increasing nowadays. Unintentional thyroxine ingestion in children may follow diverse manifestations, ranging from asymptomatic presentation to thyrotoxic crisis. While asymptomatic children can be monitored at home, children with severe symptoms need intensive care support. A child can devel

Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is one of the most recognized forms of autoimmune encephalitis. We report the case of a 25-year-old woman with ovarian teratoma-associated anti-NMDAR encephalitis complicated by severe pneumonia. Despite treatment at multiple hospitals, she remained comatose with involuntary perioral myoclonus and severe pulmonary infection while intubated. After aggressive anti-infective therapy at our center, her condition deteriorated, and extubation failed; subsequent tracheostomy stabilized the infection and allowed uneventful laparoscopic removal of

Stokes-Adams syndrome is defined as a transient loss of consciousness with spontaneous recovery after a decrease in cardiac output that leads to a state of cerebral hypoxia and may cause seizures. It is a rare and poorly documented condition. High-degree atrioventricular block, paroxysmal ventricular arrest, and pulseless ventricular tachycardia have been described as causes, and depending on the case, temporary or permanent pacemaker implantation or cardiac defibrillation may be necessary. We present the case of a 78-year-old male patient with Stokes-Adams syndrome secondary to 2:1 atrioventr

Rheumatoid meningitis (RM) is a rare central nervous system complication of rheumatoid arthritis. Symptoms of RM are diverse, including headache, convulsions, hemiplegia, movement disorders, cognitive decline, and consciousness disorders. Sometimes, RM can present neurological symptoms prior to joint symptoms, which can be a diagnostic challenge. Here, we report a case of RM without obvious arthritis symptoms, which was later diagnosed as RM supported by musculoskeletal ultrasound findings. The diagnosis of RM was made based on characteristic brain MRI findings, positive anti-cyclic citrullina

Central pontine myelinolysis (CPM) typically arises from the rapid correction of hyponatremia. A literature search showed that CPM secondary to malignant lymphoma is rare, with only 11 reported cases. An 80-year-old woman presented with anorexia as her primary symptom and was subsequently diagnosed with diffuse large B-cell lymphoma (DLBCL) following an axillary lymph node biopsy. Shortly after treatment initiation, she experienced convulsions and altered consciousness. Brain MRI revealed a symmetrical lesion in the pons, but gadolinium enhancement was not observed and cerebrospinal fluid cyto

India is an endemic country for scrub typhus, a common rickettsial infection caused by Orientia tsutsugamushi contracted through the bite of infected mites. Though relatively common in the pediatric population, early diagnosis sometimes becomes difficult due to its atypical presentation. We reported five febrile children who had varied presentations like convulsion, jaundice, generalized edema, or features like Kawasaki disease, and only one had pathognomonic eschar. They were diagnosed with acute disseminated encephalomyelitis, nephrotic syndrome, hepatitis, or Kawasaki phenotype hyperinflamm

Development of cerebral venous sinus thrombosis (CVST) is a rare manifestation of heparin-induced thrombocytopenia (HIT). Herein, we present a case in which heparin administration for primary CVST caused paradoxical worsening of CVST secondary to HIT. A 53-year-old woman diagnosed with CVST was provided with intravenous unfractionated heparin therapy. After 12 days, the patient presented tonic convulsive seizures (TCS). Subsequent magnetic resonance image (MRI) scans revealed an exacerbation of cerebral edema with a subcortical hemorrhage on the left parietal lobe. Laboratory test results reve

Posterior reversible encephalopathy syndrome (PRES) is an increasingly recognized clinical entity associated with a variety of medical conditions. It is commonly considered in the presentation of uncontrolled, severe hypertension. However, more recently, it has been described in the setting of blood transfusion, particularly in those with chronic anemia, even in the absence of severe hypertension. We describe a patient who presented to the emergency department 12 days after large blood transfusion for severe, chronic anemia with headache, vision loss, expressive aphasia and a change in mental
