Implantation of stents to ensure an adequate interatrial communication in patients with hypoplastic left heart syndrome.

AIMS: To assess the feasibility of interatrial stenting for left atrial decompression in infants with hypoplastic left heart syndrome treated by a "hybrid-approach", with bilateral surgical banding of the pulmonary arteries and percutaneous stenting of the arterial duct. PATIENTS AND METHODS: We stented the atrial septum in 5 infants aged from 21 to 77 days, making the intervention as an elective procedure in 4, but as a rescue procedure in the fifth patient, who had a restrictive foramen. The stents, comprising 2 Jo-stents of 17 millimetres hand-crimped on a balloon catheter with dimensions

A randomized controlled trial of surgery vs steroid injection for carpal tunnel syndrome.

BACKGROUND: Decompressive surgery and steroid injection are widely used forms of treatment for carpal tunnel syndrome (CTS) but there is no consensus on their effectiveness in comparison to each other. The authors evaluated the efficacy of surgery vs steroid injection in relieving symptoms in patients with CTS. METHODS: The authors conducted a randomized, single blind, controlled trial. Fifty patients with electrophysiologically confirmed idiopathic CTS were randomized and assigned to open carpal tunnel release (25 patients) or to a single injection of steroid (25 patients). Patients were fol

Surgical Management of Pericardial Effusion of Non-cardiac Etiology: Etiologic Spectrum, Diagnostic Contribution, and Early Outcomes in a Single-Center Retrospective Case Series.

Background Pericardial effusion has many causes, ranging from inflammatory and infectious diseases to tuberculosis, malignancy, renal failure, autoimmune conditions, and systemic illness. Pericardiocentesis provides rapid relief of pericardial effusion. However, in certain non-cardiac etiologies, fluid analysis alone may be insufficient to establish the underlying diagnosis. Surgical drainage through pericardiotomy or pericardiectomy allows controlled evacuation and direct sampling of the pericardium, which can help establish the diagnosis and guide the treatment. Objective The main objective

Long-Term Clinical and Radiologic Outcome Following Surgical Treatment of Lumbar Spondylodiscitis: A Retrospective Bicenter Study.

BACKGROUND AND STUDY AIMS: Spinal instrumentation for spondylodiskitis (SD) remains highly controversial. To date, surgical data are limited to relatively small case series with short-term follow-up data. In this study, we wanted to elucidate the biomechanical, surgical, and neurologic long-term outcomes in these patients. MATERIAL AND METHODS: A retrospective analysis from two German primary care hospitals over a 9-year period (2005-2014) was performed. The inclusion criteria were (1) pyogenic lumbar SD, (2) minimum follow-up of 1 year, and (3) surgical instrumentation. The clinical and radi

Fibrolipomatous Hamartoma of the Median Nerve Misdiagnosed as a Palmar Space Abscess: A Case Report.

BACKGROUND: Fibrolipomatous hamartoma (FLH), also categorized under lipomatosis of nerve, is a rare, is an uncommon benign fibrofatty lesion of the peripheral nerves that most commonly affects the median nerve. Its slow progression and atypical presentation often lead to misdiagnoses and improper surgical management. CASE PRESENTATION: A 17-year-old female presented with a 5-year history of progressive left-hand swelling and paraesthesia and a 2-year history of pain and secondary reduction in grip strength. The mass was previously misdiagnosed as an acute mid-palmar space abscess, which resul

A Rare Presentation of Concurrent Typical, Atypical, and Aggressive Thoracic Vertebral Hemangiomas: Insights into Histological Variability and Multidisciplinary Management.

BACKGROUND: Vertebral hemangiomas are typically benign and asymptomatic, but a small subset, known as aggressive vertebral hemangiomas, may present with neurological deficits due to extraosseous extension and spinal cord compression. These cases require timely diagnosis and a multidisciplinary management approach. CASE PRESENTATION: A 42-year-old man presented with acute-onset severe mid-back pain for 2 months. Neurological examination revealed exaggerated reflexes without motor or sensory deficits. Magnetic resonance imaging showed expansile lesions at T7, T9, and T10, with additional angiom

Nasogastric Tubes-Indications, Placement, and Management: A Review.

IMPORTANCE: Nasogastric (NG) tube placement is used in a variety of clinical indications. Although there have been few modifications to the tubes themselves, for several decades now the practice and use of NG tubes has continued to evolve. NG tube decompression is one of the tenets of management of small-bowel obstructions that are not complicated by intestinal compromise or peritonitis. Additionally, these tubes are often used following a variety of gastrointestinal surgical procedures. This review provides a comprehensive understanding of the NG tube, the technique for proper placement, indi

Invasive sellar extraventricular neurocytoma: illustrative case.

BACKGROUND: Sellar extraventricular neurocytomas are exceedingly rare neuronal tumors that closely mimic more common sellar lesions, including pituitary adenomas and craniopharyngiomas. Their clinical presentation, most often visual field deficits with variable hypopituitarism, and nonspecific imaging characteristics make preoperative diagnosis difficult. Because few cases have been reported, optimal management strategies and long-term outcomes remain incompletely defined. OBSERVATIONS: A 40-year-old man presented with progressive bitemporal hemianopia, headaches, and partial hypopituitarism.

Delayed herniation of cauda equina root through occult dural tear following unilateral biportal endoscopic decompression: illustrative case.

BACKGROUND: Incidental dural tears are a well-known but potentially serious complication of spinal surgery. The authors present a unique case of delayed cauda equina herniation through an occult dural tear without any evident CSF leakage, emphasizing diagnostic challenges and preventive measures specific to unilateral biportal endoscopy (UBE). OBSERVATIONS: A 60-year-old man underwent UBE decompression for lumbar canal stenosis and experienced immediate symptom relief. However, 3 weeks postoperatively, he developed acute severe radicular pain, initially presumed to be caused by disc herniatio

Trigeminal neuralgia caused by dolichoectatic vertebral artery: Reports of two cases.

INTRODUCTION: Trigeminal neuralgia (TN) caused by a dolichoectatic vertebral artery is a rare and atypical form of TN, where an enlarged and tortuous vertebral artery compresses the trigeminal nerve at its root entry zone. The term dolichoectatic is of Greek origin and means elongated, torturous or dilated. The vascular anomaly may lead to the characteristic severe, stabbing facial pain. This type of TN is often identified through advanced imaging techniques such as magnetic resonance imaging or angiography, which reveal the vascular anomaly impinging on the nerve. Treatment options for the ty

Full-Endoscopic Transforaminal Debridement and Decompression for Brucellar Thoracic Spinal Epidural Abscess: A Minimally Invasive Alternative to Open Surgery.

OBJECTIVE: Thoracic spinal epidural abscess (SEA) is a rare but dangerous condition, and traditional surgical methods are accompanied by extensive trauma and approach-related complications. Here we introduce the technique of full-endoscopic transforaminal debridement and decompression and evaluate its feasibility for treating brucellar thoracic SEA. METHODS: We performed thoracic full-endoscopic transforaminal decompression and debridement on two patients with neurological deficits caused by brucellar SEA, which is mainly composed of granulation tissue rather than pus. Postoperative MRI was c

Idiopathic calcinosis cutis causing cubital tunnel syndrome: A case report and review of literature.

Calcinosis cutis is a type of heterotopic calcification where abnormal calcium deposition occurs in skin or subcutaneous tissue. Among the subtypes of calcinosis cutis, the idiopathic variety occurs without underlying biochemical calcium abnormality. We report a rare case of idiopathic calcinosis cutis causing cubital tunnel syndrome. A 63-year-old female presented with pain and numbness in the ulnar aspect of her left hand. The X-ray of the left elbow showed deposition of radiopaque material on the posteromedial aspect. Her nerve conduction study showed evidence of ulnar nerve compression at

Thoracolumbar surgery for degenerative spine diseases complicated with tethered cord syndrome: A case report.

BACKGROUND: Tethered cord syndrome (TCS) secondary to split cord malformation (SCM) is rare in adulthood. There is as yet no consensus about the optimal treatment method for adult patients with SCMs and degenerative spine diseases such as lumbar stenosis, spondylolisthesis and ossification of the ligamentum flavum (OLF). The tethered cord poses a great challenge to the decompression and fusion procedures for the intraoperative stretching of the spinal cord, which might lead to deteriorated neural deficits. Here, we report on a case to add our treatment experience to the medical literature. CA

Damned If You Drain, Damned If You Don't: A Case of Pericardial Decompression Syndrome.

Pericardial decompression syndrome (PDS) is an unusual clinical scenario with a reported incidence of 5% in all surgical or percutaneously managed pericardial tamponade patients. It is defined as a paradoxical hemodynamic instability leading to left ventricular (LV), right ventricular (RV), or biventricular dysfunction. An 84-year-old female with a history of a chronic pericardial effusion presented with symptoms of tamponade. She had had multiple prior admissions with an extensive and unyielding workup for the etiology of her pericardial effusion. During the present admission, a transthoracic

Escalate and De-Escalate Therapies for Intracranial Pressure Control in Traumatic Brain Injury.

Severe traumatic brain injury (TBI) is frequently associated with an elevation of intracranial pressure (ICP), followed by cerebral perfusion pressure (CPP) reduction. Invasive monitoring of ICP is recommended to guide a step-by-step "staircase approach" which aims to normalize ICP values and reduce the risks of secondary damage. However, if such monitoring is not available clinical examination and radiological criteria should be used. A major concern is how to taper the therapies employed for ICP control. The aim of this manuscript is to review the criteria for escalating and withdrawing ther
