Proximal row carpectomy.

Proximal row carpectomy is extremely useful as a wrist reconstructive technique for cases of degenerative joint arthritis of the radiocarpal joint cause by scapholunate advanced collapse, scapholunate advanced collapse, schaphoid nonunion advanced collapse, trans-scaphoid perilunate fracture dislocations, lunate dislocations, and Kienböck disease. It should be selected with caution for patients younger than 35 years old. The procedure can be performed with or without temporary internal fixation with with Kirschner wires, and adjunctive techniques of dorsal capsule interposition, proximal capit…

Fractured neck of femur in the mobile independent elderly patient: should we treat with total hip replacement?

PURPOSE: To report the outcome of displaced sub-capital neck of femur fractures in the independent elderly (>70 years) managed with total hip arthroplasty through a modified Hardinge approach. METHODS: Between 1998 and 2000, a surgeon performed a cemented hip replacement using a modified lateral approach in 36 consecutive patients in the Hawke's Bay Regional Hospital, Hastings. Medical charts and out-patient follow-up clinic records were reviewed with respect to outcomes, with particular reference to complications. Independent review of functional outcome was completed at one year postsurgery

Effect of Glenohumeral Joint Bone Morphology on Anterior Shoulder Instability: A Case-Control Study.

PURPOSE: Glenohumeral joint compatibility and bone morphology are among the most critical factors in shoulder stabilization. Our study investigated the effect of the bone morphological structure of the shoulder joint on anterior shoulder dislocation. METHODS: In our study, people with a history of shoulder dislocation were selected as the patient group. In the control group, patients with shoulder MRIs for any reason and no history of shoulder dislocation were included. Those who have a fracture around the shoulder, a congenital deformity in the shoulder region, arthrosis of the shoulder, tho

[Recurrent anterior instability of the shoulder associated with full thickness rotator cuff tear: results of surgical treatment].

PURPOSE OF THE STUDY: Recurrent anterior dislocations associated with full thickness rotator cuff tear (RCT) carry a difficult therapeutic problem: should we treat instability and rotator cuff tear at the same time or only one of both pathologies? The goal of this study was to analyse a retrospective series of patients operated on to try to answer this question. MATERIAL AND METHODS: Twenty-eight shoulders (27 patients) were operated on between 1988 and 2002. The mean age at first dislocation was 47 years (16-65), the average delay between first dislocation and operation was 6.1 years. Twenty

Prognostic value of the Neer and Mayo-Fundacion Jimenez Diaz classifications in reverse shoulder arthroplasty for proximal humerus fractures.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is the preferred surgical treatment for non-reconstructible 7 proximal humerus fractures in the elderly population. The most common classification for 8 these fractures is the Neer classification. A more recent system is the Mayo-Fundación Jiménez 9 Díaz (Mayo-FJD) classification, which has demonstrated higher intraobserver and interobserver 10 reliability compared to the Neer system. However, their predictive utility remains unclear. METHODS: The study consisted of a retrospective analysis of prospectively collected data from 40 patients

Traditional bone setter practices and the interaction with biomedical care in the treatment of hip fractures in The Gambia: A qualitative study.

A growing ageing population in Africa underscores the need to reorient health systems to provide care for non-communicable conditions impacting older populations such as hip fractures. In The Gambia, the number of hip fractures are expected to quadruple for both men and women in the next 30 years. This is comparable to hip fracture incidence rates in other South and West African countries. Moreover, mortality rates post-hip fracture are 2-3 fold higher in populations from the African Region than for those in other high-income countries. Globally, in many countries including in The Gambia, musc

Short-term mobility in low-energy pelvic ring fractures after conservative treatment.

BACKGROUND: The growing population of elderly people is leading to a rising number of pelvic ring fractures. These often involve combination … d as the sole independent predictor of non-mobility at 2 weeks (p = 0.0037). Age, BMI, pubic branch fracture dislocation >5 mm, analgesic use of opiates, corticosteroid therapy, alcohol and smoking showed no association with mobility recovery. Fracture consolidation was observed in 73.2 % of patients (F/M; 21/9), while one female had no consolidation. Ten patients (F/ …

Percutaneous Instrumentation of a Complex Lumbar Spine Fracture with Bilateral Pedicle Dissociation: Case Report and Technical Note.

BACKGROUND AND STUDY OBJECTIVE: Complex traumatic lumbar spine fractures are difficult to manage and typically occur in younger patients. Surgical immobilization for unstable fractures is an accepted treatment but can lead to future adjacent-level disease. Furthermore, large variations in fracture morphology create significant difficulties when attempting fixation. Therefore, a surgical approach that considers both long-term outcomes and fracture type is of utmost importance. We present a novel technique for percutaneous fixation without interbody or posterolateral fusion in a young patient wi

Frequent Sural Nerve Injury with Posterior Approach for Ankle Fracture Fixation.

INTRODUCTION: The purpose of this study was to report the incidence of iatrogenic sural nerve injury in a large, consecutive sample of surgically managed ankle fractures and to identify factors associated with sural nerve injury and subsequent recovery. We hypothesize that a direct posterior approach may be associated with higher risk of iatrogenic sural nerve injury. METHODS: A retrospective cohort study of 265 skeletally mature patients who sustained ankle fractures over a 2-year period was done. All were treated with open reduction and internal fixation of fractured malleoli. Patient, inju

Thumb Injuries and Instabilities. Part 1: Anatomy, Kinesiology, and Imaging Techniques of the Thumb.

The unique anatomical characteristics of the thumb offer a broad range of motion and the ability to oppose thumb and finger, an essential function for grasping. The motor function of the thumb and its orientation make it particularly vulnerable to trauma. Pathologic lesions encountered in this joint are varied, and imaging techniques play a crucial role in injury detection and characterization. Despite advances in diagnostic accuracy, acute thumb injuries pose a challenge for the radiologist. The complex and delicate anatomy requires meticulous and technically flawless image acquisition.
