Epidermolysis bullosa care in Mexico.

Epidermolysis bullosa (EB) in Mexico continues to be a rare genodermatosis that is still unknown for most of the health care professionals in the country. The spirit of DebRA MEXICO was born in 1994 when the Mexican health care team started to see patients with the main purpose to provide medical care, genetic counseling, and advice to patients with EB and their families; to promote collaboration and exchange information among people with EB; to research and find new therapeutic approaches; and finally, to diffuse knowledge and raise awareness of the issues of EB in general public and health c

Toward the use of genomics to study microevolutionary change in bacteria.

Bacteria evolve rapidly in response to the environment they encounter. Some environmental changes are experienced numerous times by bacteria from the same population, providing an opportunity to dissect the genetic basis of adaptive evolution. Here I discuss two examples in which the patterns of rapid change provide insight into medically important bacterial phenotypes, namely immune escape by Neisseria meningitidis and host specificity of Campylobacter jejuni. Genomic analysis of populations of bacteria from these species holds great promise but requires appropriate concepts and statistical t

Monogenic Mendelian disorders in general neurological practice.

An observational study of monogenic Mendelian disorders seen in general neurology outpatient clinics over a 6-year period was undertaken. Fifty-three patients with 16 different diagnoses were identified, the commonest being Huntington's disease and neurofibromatosis type 1. This low frequency of monogenic Mendelian disorders has implications for both continuing medical education and for neurological training. All such patients are probably best referred to specialist clinics where expertise in diagnosis, genetic counselling and testing, and management has been developed.

Polymorphism of detoxification genes and cell resistance to mutagens in patients with Ehlers-Danlos syndrome.

Study of polymorphism in 3 genes of the glutathione S-transferase family (GSTM1, GSTT1, and GSTP1) in children with Ehlers-Danlos syndrome whose cells were defective in repair of gamma-induced DNA damages revealed accumulation of GSTM1(+) genotypes compared to children of the control group. Generation of reactive oxygen species by neutrophils from patients with this syndrome was higher than in healthy donors. Our results indicate that glutathione S-transferase genes are involved in the resistance to mutagenic agents and demonstrate medical and genetic peculiarities of patients with Ehlers-Danl

Spinal muscular atrophy.

Spinal muscular atrophy is a common genetic disease of the motor neuron (frequency of eight cases per 100,000 live births) with a high mortality during infancy and no known treatment. Death is caused by severe and progressive restrictive lung disease. New information regarding the nature and function of the SMN protein and the availability of new pharmacologic agents now make it possible to consider clinical trials in this disease. Rehabilitation and proper management of medical complications have improved both the quality and duration of life for children with spinal muscular atrophy.

A family affected by mitochondrial encephalomyopathy: a nursing protocol.

Mitochondrial encephalomyopathies are rare genetic diseases that affect organs and tissues with high oxidative activity, such as the brain, striated muscles, and heart. Although these conditions have received attention in the medical literature, there is an absence of published information on this topic in the nursing literature. The purpose of this article is twofold: (1) to provide an overview of mitochondrial encephalomyopathies and (2) to present a protocol to guide nursing practice. Protocol development is based on a case history and guided by Dungan's model of dynamic integration. The pr

[Genetic counseling in a case of Takayasu arteritis].

Two cases affected with Takayashu arteritis are presented. The adult probanda born in 1960 after genetic counselling and appropriate prenatal care had a healthy liveborn boy. Her previous six pregnancies were terminated on the basis of forced medical reason. The infant probanda died at the age of 4 months with the features of sudden infant death syndrome (SIDS). Pathohistological examination detected her Takayashu arteritis. It seems to be the youngest published case in the international literature and Takayashu arteritis may be the rare cause of SIDS.

[Apert syndrome. Ultrasonic diagnosis, obstetrical management].

We report two observations of antenatal diagnosis of Apert syndrome. This uncommon genetic disorder suggest an autosomal dominant inheritance, but almost all cases described are sporadic; the responsible gene is yet not located. Ultrasonographic detection is difficult, based on the following signs: brachycephalic skull (unusually detected), flat facial profile with a nasal bridge depression, tall appearance of the forehead (inconstant), total bilateral and symmetrical syndactylies of the hands and feet. At last we present our arguments for medical abortion, when this disorder is detected.

Rheumatoid arthritis, food, and allergy.

One of the most promising lines of research on the etiology and pathogenesis of rheumatoid arthritis (RA) is its association with the genetically determined MHC class II antigens. The function of these macromolecules, presentation of antigens to the T-helper cells, supports the possibility that external antigens influence RA. We review available literature concerning the relationship between RA and food. Circumstantial evidence suggests that some foods or food components might influence subgroups of RA patients, although many of the publications on this subset do not meet acceptable standards

Pineal melatonin in schizophrenia: a review and hypothesis.

It has long been suggested that abnormal functions of the pineal gland may be implicated in the pathophysiology of schizophrenia. We present evidence proposing that diminished melatonin secretion may be associated with the pathophysiology of a subgroup of schizophrenic patients characterized by cerebral atrophy and ventricular enlargement, negative symptoms, impaired cognitive and psychosexual development, onset at pubescence, poor response to neuroleptic medication, and possible increased risk of extrapyramidal symptoms. This view holds that a subnormal plasma melatonin level may be a marker

Analysis of the changing birth prevalence of neural tube defects in Hungary.

The birth prevalence of neural tube defects had a significant decrease in Hungary from the sixties till 1978. After this a slight but not significant increase was registered. The changing pattern had no correlation with the developmental (different groups of neural tube defects, still- and livebirth rate, sex ratio, birth weight distribution and twin rate), demographic (monthly and territorial distribution, maternal age, birth order) and medical (prevention program, genetic counselling) factors. There was only one obvious finding, a negative correlation with the standard of living of the popul

[Genodermatoses as an indication for the interruption of pregnancy: hereditary epidermolysis].

From a medical point of view, an eugenic indication is under consideration when the fetus is suffering from a lethal, semi-lethal or subvital congenital disease. Of the different types of hereditary epidermolysis, Herlitz syndrome and recessive dystrophic epidermolysis bullosa including the inverse form, are indications for interruption. In all three diseases inheritance is autosomal-recessive. Eugenic postulates for interruption are realized only partly or not at all by the remaining types of epidermolysis. A prenatal examination being impossible in epidermolysis, genetic counseling after the

[Myoclonic-astatic seizures (Lennox syndrome) in the course of juvenile neuronal ceroid-lipofuscinosis (M. Batten-Spielmeyer-Vogt) (author's transl)].

EEG findings and the course of epileptic seizures in two patients with neuronal ceroid lipofuscinosis (Batten Spielmeyer Vogt syndrome) are presented. Both patients, during the course of disease, developed therapy resistant epileptic reactions with myoclonicastatic seizures. These seizures in connection with diffuse encephalopathy and EEG pattern with 2.5 to 3.5/sec slow-spike-wave meet the criteria of the Lennox syndrome. Pathogenetic questions regarding possible additional genetic predisposition for epileptic seizures are discussed. Since therapeutic effect of different medications is uncert

[Epilepsy and Dupuytren's contracture--a syntropy of 2 diseases?].

Dupuytren's contracture was observed in 21,6 percent of 524 patients with epilepsy. This combination is characterised by a steadily progression by relatively benign course with a consequenly low frequency of operation and rate of recurrence. Possible genetic association has to be taken into consideration, because idiopathic epilepsy is the type most frequently found with Dupuytren's contracture. In this material no correlation has been found between Dupuytren's contracture and liver disease. Diabetes mellitus and other aetiologic factor which have been suggested. Effect of avian myeloblastosis virus in the Japanese quail.

Moscovici, Carlo (University of Colorado Medical Center, Denver), and E. H. Macintyre. Effect of avian myeloblastosis virus in the Japanese quail. J. Bacteriol. 92:1141-1149. 1966.-Avian myeloblastosis virus (AMV) induced a spectrum of neoplasms in Japanese quail (Coturnix coturnix japonica) which was similar to that observed in the chicken, with one exception: the total absence of acute myeloblastic leukemia in quail. Studies in vivo as well as in vitro suggested that the cause for this difference may be ascribed to the heterogeneity of AMV and to the genetic makeup of the quail cell.

Mapping the Parkinson's pandemic: Unveiling and addressing the global burden.

Worldwide, the incidence, prevalence, disability, and mortality associated with Parkinson's disease (PD) have increased substantially, placing an immense strain on healthcare systems across countries of all income levels, prompting the use of the term "Parkinson pandemic" to emphasize the urgent need for coordinated strategies to address its worldwide impact. This narrative review explores the regional differences in the dramatic increase in PD burden, along with its genetic, environmental, and socioeconomic determinants worldwide. Furthermore, it discusses the healthcare access in different r

New insights into enlarged parietal foramina: an anatomical, radiological, and histological study.

INTRODUCTION: Enlarged parietal foramina (EPF) are rare, symmetric calvarial defects. Although typically benign, they are clinically significant due to possible associations with venous malformations and inherited genetic variants. However, the anatomical, radiological, and histological details of these entities are scant in the medical literature. METHODS: Two hundred and fifty adult human skulls from multiple anatomical collections were examined for EPF. Gross morphometric data were supplemented by high-resolution microcomputed tomography (microCT) and histological analysis (H&E, PAS, and M
