Salvage for intramedullary nailing breakage after operative treatment of trochanteric fractures.

BACKGROUND: Trochanteric fractures are one of the most common fractures in elderly people. The use of intramedullary nails is an option for their treatment, especially in unstable patterns. Nail breakage is a rarely reported complication. The aim of this study was to determine the prevalence of nail breakage in our center. Secondary objectives are to show the management of this complication in our institution as well as the technical problems, complications and final outcomes of these patients in our hands. MATERIAL AND METHODS: In a retrospective case series review between 2010 and 2015, we

Open Reduction of Pediatric Femoral Neck Fractures Reduces Osteonecrosis Risk.

Pediatric femoral neck fractures are rare injuries that are associated with a high risk of osteonecrosis of the femoral head. This study compared pediatric patients with fully displaced femoral neck fractures treated with either open reduction and internal fixation (ORIF) or closed reduction and internal fixation (CRIF). After institutional review board approval was obtained, the authors identified 53 patients who were treated for femoral neck fracture between 2003 and 2012. Inclusion criteria were as follows: (1) diagnosis of a fully displaced femoral neck fracture with no anatomic cortical c

The effect of squat depth on multiarticular muscle activation in collegiate cross-country runners.

The squat is a closed-chain lower body exercise commonly performed by many athletes. Muscle activity has been examined during partial and parallel squats in male weightlifters, but not in male and female runners. Therefore, this study measured muscle activity with surface electromyography (EMG) during partial and parallel squats in 20 Division I collegiate cross-country runners (10 males and 10 females) in a randomized crossover design. We hypothesized the parallel squat would increase extensor muscle activitation (i.e. hamstrings and erector spinae). Furthermore, we sought to determine if cha

Symptomatic venous thrombo-embolism in low-energy isolated fractures in hospitalised patients.

INTRODUCTION: In the prevention of venous thrombo-embolic events (VTEs) in isolated low-energy fracture patients, management guidelines are conflicting and prior literature is lacking. We aimed to determine the incidence and factors associated with the development of symptomatic VTE in this patient cohort. MATERIALS AND METHODS: To identify patients with isolated, low-energy fractures, we studied billing records from all admissions to our tertiary care orthopaedic hospital from 2007 to 2009. We used International Classification of Diseases, 9th Revision codes to identify patients who develope

Functional electrical stimulation elliptical stepping versus cycling in spinal cord-injured individuals.

BACKGROUND: The cardiorespiratory responses and mechanical efficiencies of two modalities of functional electrical stimulation augmented leg exercises - isokinetic cycling and isokinetic elliptical stepping - were compared amongst individuals with spinal cord injury. METHODS: Five subjects performed seated isokinetic evoked cycling and elliptical stepping leg exercise at 10, 20 and 30rev·min(-1) pedal cadences. 3-D motion analysis and force transducers attached onto the foot pedals quantified the external forces and power outputs developed by each lower extremity. Hip, knee and ankle joints p

Emerging ideas: Novel 3-D quantification and classification of cam lesions in patients with femoroacetabular impingement.

BACKGROUND: Femoroacetabular impingement (FAI) can lead to labral injury, osseous changes, and even osteoarthritis. The literature contains inconsistent definitions of the alpha angle and other nonthree-dimensional (3-D) radiographic measures. We present a novel approach to quantifying cam lesions in 3-D terms. Our method also can be used to develop a classification system that describes the exact location and size of cam lesions. QUESTIONS/HYPOTHESES: We asked whether automated quantification of CAM lesions based on CT data is a reasonable way to detect CAM lesions and whether they may be cl

Joint laxity is related to lower extremity energetics during a drop jump landing.

PURPOSE: To examine the relationships between anterior knee laxity (AKL), genu recurvatum (GR), and general joint laxity (GJL) with sagittal plane energetics in males and females during a drop jump task. METHODS: A total of 68 females and 50 males were measured for AKL, GR, and GJL and were instrumented to obtain neuromuscular and biomechanical data on their dominant limb during the initial landing phase of a 45-cm drop jump. Multiple linear regressions determined the extent to which the three joint laxity variables combined to predict hip, knee, and ankle work absorption and stiffness. Assoc

The in-the-crease inferior gluteal artery perforator flap for breast reconstruction.

BACKGROUND: Perforator free flaps harvested from the abdomen or buttock are excellent options for breast reconstruction. They enable the reconstructive surgeon to recreate a breast with skin and fat while leaving muscle at the donor site undisturbed. The gluteal artery perforator free flap using buttock tissue was first introduced by the authors' group in 1993. Of the 279 gluteal artery perforator flaps, the authors have performed for breast reconstruction, 220 have been based on the superior gluteal artery and 59 have been based on the inferior gluteal artery. The authors have found that for

Ejaculation induced by penile vibratory stimulation in men with spinal cord injuries. The importance of the vibratory amplitude.

A total of 66 men with a spinal cord injury (SCI) and ejaculatory dysfunction were included in two different but comparable study populations I (n = 25) and II (n = 41). The level of lesion ranged from C2 to L1 (44 complete). Penile vibratory stimulation (PVS) to induce ejaculation was performed with two different types of vibrators in population I and considerably different ejaculation rates (antegrade+retrograde) occurred depending on the vibrator used. Our experience suggested discrepancies between the manufacturers' specifications and the actual vibrator outputs concerning frequencies and

The Predictive Factors for Post-Traumatic Hip Osteoarthritis After Surgical Treatment of Acetabular Fractures: A Retrospective Cohort of 130 Patients.

Background/Objectives: Post-traumatic hip osteoarthritis (PTHOA) remains the principal long-term complication after operatively treated acetabular fractures. The relative weight of patient-, injury-, and surgeon-dependent variables that drive its development is still debated. We aimed to identify independent predictors of PTHOA after open reduction and internal fixation (ORIF) of acetabular fractures. Methods: We retrospectively reviewed 130 consecutive patients who underwent ORIF for an acetabular fracture at a single tertiary trauma centre between November 2012 and January 2023 and had a min

Exertional Rhabdomyolysis Following Concurrent Exercise Including Pelvic Floor (Kegel) Training in a Previously Healthy 36-Year-Old Male Patient: A Case Report.

Rhabdomyolysis is a potentially life-threatening condition characterized by skeletal muscle breakdown and release of intracellular contents into the systemic circulation. Exertional rhabdomyolysis (ER) most commonly occurs after high-intensity exercise involving large muscle groups, but cases related to lower intensity or repetitive activity have also been reported. We describe a case of acute ER in a previously healthy 36-year-old man who developed the condition after initiating a workout regimen for erectile dysfunction that included gluteal exercises and pelvic floor muscle training (PFMT),

Angiography-Derived Fractional Flow Reserve During Percutaneous Coronary Intervention.

BACKGROUND: Angiography-derived fractional flow reserve (Angio-FFR) is an emerging tool for guiding percutaneous coronary intervention (PCI). Its uptake and outcomes compared with pressure wire (PW)-based assessment in the United States are unknown. METHODS: We conducted a cohort study using US Medicare beneficiary data from January 1, 2019 to December 31, 2024. Propensity score matching (1:3) of Angio-FFR to PW was performed in patients who underwent PCI during the same procedure, and separately among those who did not undergo PCI during the same procedure. The primary outcome was the cumula

Femoral elevation variability in direct anterior THA: mechanical assessment of a common table-integrated lift system.

BACKGROUND: The direct anterior (DA) approach for total hip arthroplasty (THA) is increasingly used by surgeons. Although Hana traction tables are commonly used in DA THA, no study has quantified the mechanical performance and uniformity of the femur lift. Characterising femoral lift variability is clinically important for patient safety during femoral exposure. PURPOSE: To assess whether femoral lift velocity differs across increasing loads, femur lift laterality, and table age. METHODS: 7 Mizuho OSI Hana traction tables (14 actuators) were tested using a custom jig and loads between 25 and
