Vascular calcification in patients with end-stage renal disease.

Vascular calcification is the most common type of extra-osseous calcification in end-stage renal disease (ESRD), manifesting as both medial and intimal calcification of large arteries. It is highly prevalent, often progressive and is associated with reduced arterial elasticity and increased mortality. Risk factors for calcification in ESRD include age, duration of dialysis, diabetes mellitus, most probably an elevated calcium-phosphorus product (Ca x P) level, the dose of calcium-containing phosphate binders and the induction of the systemic inflammatory response. Uraemic calcification was tho

[Diagnosis and treatment of osteopathic parathyroid adenoma].

OBJECTIVE: To introduce the advanced diagnostic technologies and share the surgical experience of parathyroid adenoma. METHODS: From November 1986 to August 2000, 9 patients with parathyroid adenoma who underwent operations were analyzed retrospectively. Out of them, there were 3 males and 6 females and their ages ranged from 12 to 55 years with an average of 32 years. The average disease course was 4 years and 9 months. General decreased density of the bone cortex and subperiosteum absorption were found in all 9 cases, while multi bone cyst lesion in 3 cases; obsolete fracture in 5 cases, in

Acute change in parathyroid function in primary hyperparathyroidism following ultrasonically guided ethanol injection into solitary parathyroid adenomas.

Ultrasonically guided percutaneous injection of 96% ethanol into solitary parathyroid tumours in patients with primary hyperparathyroidism may be used as an alternative to surgery in selected patients. Contrary to surgical parathyroidectomy, the acute changes in parathyroid function following ultrasound-guided chemical parathyroidectomy have never been described. Seven consecutive and highly selected patients with primary hyperparathyroidism were treated with ultrasonically guided injection of ethanol (96%) into solitary and biopsy-verified parathyroid tumours. Basic treatment included a maxim

C-cell hyperplasia in secondary hyperparathyroidism.

Calcitonin is a hypocalcaemia producing hormone and is secreted by C-cells of the thyroid. The current study was undertaken on a hypothesis that C-cell hyperplasia may develop in the secondary hyperparathyroidism of chronic renal failure in response to sustained hypercalcaemia. With an immunoperoxidase staining method for calcitonin, C-cell hyperplasia was noted in four of six cases of autosomal dominant polycystic kidney disease and in three of six cases of acquired renal cystic disease, an overall incidence of 58% compared with an incidence of 36% (five of 14) in cases of primary hyperparath

Elevated bone aluminum and suppressed parathyroid hormone levels in hypercalcemic dialysis patients.

We studied 21 dialysis patients who became hypercalcemic without vitamin D or calcium therapy and compared them to 28 dialysis patients who were not hypercalcemic. In the hypercalcemic group, the mean ionized-calcium level was elevated compared to normal subjects (5.4 +/- 0.4 vs. 4.9 +/- 0.1; p less than 0.001), while the ionized-calcium level in the control dialysis patients was below normal (4.5 +/- 0.4 vs. 4.9 +/- 0.1; p less than 0.001). Bone biopsies were performed in all patients. Two thirds of the hypercalcemic patients had low-turnover osteodystrophy (LTO, predominantly osteomalacia),

Co-purification of bone resorbing activity and adenylate cyclase stimulating activity from human tumours associated with the humoral hypercalcaemia of malignancy.

We have partially purified a tumour factor capable of stimulating both bone resorption in vitro and cAMP accumulation in osteoblastic ROS 17/2 cells from three human tumours associated with humoral hypercalcaemia of malignancy. Purification of tumour factor by sequential acid urea extraction, gel filtration and cation-exchange chromatography, reverse-phase high performance liquid chromatography followed by analytical isoelectric focussing provided a basic protein (pI greater than 9.3) with a molecular weight of approximately 13,000 as a major component of the final preparation which retained b

Primary hyperparathyroidism in paraneoplastic hypercalcaemia.

Hypercalcaemia is often associated with malignant disease. Causes of elevated serum-calcium levels in the absence of bony metastases include parathyroid-hormone production by the tumour, osteolytic factors made by the tumour, and coexistent primary hyperparathyroidism. By measuring nephrogenous cyclic-A.M.P. excretion to assess parathyroid-hormone function, we have determined the mechanism of such hypercalcaemia in 15 patients. Nephrogenous cyclic A.M.P. ranges from 0.05 to 2.40 mumol/g of creatinine in normal subjects, from 2.27 to 8.45 mumol/g in patients with primary hyperparathyroidism, an

Acute pancreatitis.

FOR MANY DECADES TWO TYPES OF ACUTE PANCREATITIS HAVE BEEN RECOGNIZED: the edematous or interstitial and the hemorrhagic or necrotic. In most cases acute pancreatitis is associated with alcoholism or biliary tract disease. Elevated serum or urinary alpha-amylase is the most important finding in diagnosis. The presence of methemalbumin in serum and in peritoneal or pleural fluid supports the diagnosis of the hemorrhagic form of the disease in patients with a history and enzyme studies suggestive of pancreatitis. There is no characteristic clinical picture in acute pancreatitis, and its complica

Right Coronary Artery Thrombosis Causing ST-Elevation Myocardial Infarction After a Coronary Artery Disease Reporting and Data System Score of 0, With Disseminated Intravascular Coagulation During Idiopathic Acute Pancreatitis: A Case Report.

Acute pancreatitis may cause electrocardiographic abnormalities and troponin elevation that mimic acute myocardial infarction. True coronary thrombosis is less common. The development of disseminated intravascular coagulation shortly after percutaneous coronary intervention creates a difficult balance between bleeding control and prevention of early stent thrombosis. A 68-year-old man with no previous pancreatitis presented with acute interstitial pancreatitis. Evaluation did not identify biliary disease, alcohol use, hypertriglyceridemia, hypercalcemia, medication exposure, autoimmune pancrea…

Denosumab increased bone mineral density but caused marked serum calcium fluctuations in a patient undergoing peritoneal dialysis.

Denosumab is increasingly used to treat osteoporosis in patients with advanced kidney disease, but clinically significant disturbances in ca … tion of dialysate calcium, vitamin D therapy, and calcium supplementation can precipitate overshoot hypercalcemia. Frequent monitoring and staged, stepwise prescription changes during the first month after denosumab are essential. Standardized PD-specific post-denosumab calcium management protocols are urgently needed.

Ammi visnaga (Visnaga daucoides) & Ammi majus as a novel thyrotherapeutic phytoalternative for rat with L-thyroxine-induced hyperthyroidism.

Hyperthyroidism is a worldwide health problem mainly associated with potential health consequences such as heart failure, liver dysfunction, thyroid eye disease, hypercalcemia, hyperglycemia, dehydration & nervousness; moreover, it could turn into emergent thyrotoxicosis. Right now, there are limited options to regress hyperthyroidism, including surgery, ablation, & Thionamides, while each of them is fraught with risk, making the clinical decision very complicated. Thus, finding a sustainable, safe alternative with Thionamides' potency but with no/ limited adverse effects is a distinct clinica…

IgG Kappa Multiple Myeloma with Cutaneous Plasmacytoma and Hypereosinophilic Leukocytosis in a Young Patient: A Rare Case Report.

BACKGROUND: Multiple myeloma (MM) is a malignant plasma cell disorder with a median age at diagnosis of 69 years. Its occurrence in individuals younger than 30 years is exceptionally uncommon, accounting for approximately 0.3% of all cases. Classic manifestations include bone pain, anemia, renal dysfunction, and hypercalcemia. Extramedullary disease (EMD), such as plasmacytomas, represents a more aggressive phenotype. Among these, cutaneous plasmacytomas-monoclonal plasma cell infiltrates in the skin-are exceedingly rare. CASE PRESENTATION: A 27-year-old man presented with fatigue, dysphagia,…
