Comprehensive assessment of vaginal infections using a single swab.

BACKGROUND: The decision to use a particular test to diagnose patients presenting with symptoms of vaginitis and/or STI is based primarily on the prevailing standards of care in the clinic at which the patient evaluation takes place. As a result, laboratory testing of vaginal samples for these patients often involves either an STI or a vaginitis test, but rarely both options simultaneously, which complicates the diagnosis and management of concurrent infections. METHODS: Using de-identified remnant vaginal specimens from symptomatic patients previously tested for STI (Chlamydia trachomatis (C…

Genomic Investigation of Disseminated Gonococcal Infections, Minnesota, USA, 2024.

This article summarizes a genomic investigation of a 4-fold increase in disseminated gonococcal infections in Minnesota, USA, in 2024. We detected the emergence of a Neisseria gonorrhoeae strain of a rarely observed sequence type, which carries a porB1a allele previously associated with disseminated disease and lacks a gonococcal genetic island.

Complicated and deep bacterial skin and soft tissue infections.

Complicated erysipelas, cellulitis, and necrotizing soft tissue infections belong to the spectrum of bacterial infections affecting the skin and underlying soft tissues. Their frequency as well as risk of recurrence and complicated disease course pose a significant health burden. The latter is further impacted by global travel activity, associated variations in the prevalence of microbes, the emergence of multi-resistant bacteria, demographically driven aging with an increase in critical comorbidities, immunosuppressive therapies, and excessive (including non-medical) use of antibiotics. Again…

[Conditions mimicking acute bacterial skin and soft tissue infections].

Acute bacterial skin and soft tissue infections represent a common clinical problem, and accurate diagnosis is crucial for initiating appropriate therapy. However, conditions such as cellulitis and erysipelas can be clinically mimicked by a variety of non-infectious conditions, including eczematous, venous, lymphatic, and autoimmune diseases. This review summarizes the key differences in clinical presentation, patient history, and laboratory findings that help distinguish true infections from their non-infectious mimickers. Special attention is given to conditions such as microbial eczema, her…

Diagnosis and management of upper limb soft tissue infections.

Soft tissue infections are common in the upper limb, particularly in the hand, which is exposed to the environment. They include entities that are diverse in their severity, their progression, and their frequency. However, what they have in common - if not managed correctly - is that they can have a major functional impact, amputation risk, or be life-threatening. The bacteria involved in upper limb infections are mainly Gram-positive cocci, but Gram-negative bacilli colonize bite wounds, which are common in the upper limb. From the most frequent and benign to the most serious, we distinguish:…

An investigation into breast infections caused by Actinomyces species.

We investigated the management of Actinomyces breast infection. Eight different Actinomyces species were isolated from 26 patients during the study period (December 2018-November 2021). All tested isolates were susceptible (at standard or high dose) to penicillin and 75 % to clindamycin. Twenty-three patients had a history of smoking. Three had coexisting breast cancer. Prescribers need to be educated about the lack of activity of metronidazole on Actinomyces.

New Frontiers in Fighting Mycobacterial Infections: Venom-Derived Peptides.

Notwithstanding the indefatigable endeavors to develop effective anti-mycobacterial therapies, mycobacterial infections still present a tough problem for medicine today. The problem is further complicated by the disquieting surge of drug-resistant mycobacterial pathogens, which considerably narrows the existing therapeutic options. Thus, there is a genuine need to discover novel anti-mycobacterial drugs. Animal venoms are considered a treasure trove of structurally variable and biologically active peptides, which may hold promise for therapeutic applications. Over the past two decades, abundan…

Atypical mycobacteria infections of surgical sites: a case series.

BACKGROUND: Atypical mycobacteria (ATB) are a rare cause of surgical site infections (SSIs). They present with clinical features of erythema, induration, and sinus formation, which recur despite conventional treatment. Early diagnosis is difficult and requires a high index of suspicion for further investigations to confirm atypical mycobacterial infection. Treatment encompasses a combination of polymicrobial agents and surgical debridement. OBJECTIVE: To describe the clinical presentation, diagnostic challenges, and treatment outcomes of atypical mycobacterial infections in surgical sites. M…

Next-generation rifamycins for the treatment of mycobacterial infections.

Mycobacterium abscessus is a rapidly growing nontuberculous Mycobacterium causing severe pulmonary infections, especially in immunocompromised individuals and patients with underlying lung conditions like cystic fibrosis (CF). While rifamycins are the pillar of tuberculosis treatment, their efficacy against M. abscessus lung disease is severely compromised by intrabacterial ADP-ribosylation. Additionally, rifamycins induce cytochrome P450 3A4 (CYP3A4), a major human drug-metabolizing enzyme, further limiting their use in patients with comorbidities that require treatment with CYP3A4 substrates…

Liposome-Encapsulated Antibiotics for the Therapy of Mycobacterial Infections.

About a quarter of the world's population is infected with Mycobacterium tuberculosis. Growing antibiotic resistance by this microorganism is a major problem in the therapy of the disease. M. avium-M. intracellulare that emerged as a major opportunistic infection of HIV/AIDS continues to afflict immunocompromised individuals. We describe the use of liposome-encapsulated antibiotics in the experimental and clinical therapy of mycobacterial infections, as well as recent experimental liposomal vaccines against tuberculosis. Liposome-mediated intravenous or inhalational delivery of antibiotics enh…

Host-directed insights into mycobacterial infections: Toward targeted immunomodulation.

Mycobacterium tuberculosis, the etiological agent of tuberculosis, is a significant worldwide health threat, especially in resource-limited environments. This review emphasizes that manipulating host metabolic and epigenetic pathways can bolster immune defences and potentially shorten the duration of tuberculosis treatment. The growing prevalence of multidrug-resistant TB and the protracted nature of standard treatments underscore the urgent need for alternative therapeutic approaches. Host-directed treatment has arisen as a promising approach that aims to enhance or redirect the host's innate

Advances in tuberculosis co-infections: Mechanisms, diagnosis, and therapeutics.

Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis, which is widespread and harmful worldwide. It can attack multiple organs throughout the body, with pulmonary tuberculosis being the most common. The Global Tuberculosis Report 2024 shows an increase in the incidence of TB from the previous year, and its transmission is closely linked to the response of the host immune system. In clinical settings, co-infections with other pathogens are becoming more common. 10.8 million new TB cases were diagnosed globally in 2023, of which 662,000 were co-infected with HIV, and t…

Heat, Humidity, and Hematuria: Glomerular Complications of Tropical Infections.

BACKGROUND: Tropical regions, home to nearly 40% of the world's population, face a high burden of infectious diseases due to climate, socioeconomic factors, and limited healthcare access. Many tropical infections - including malaria, dengue, leptospirosis, schistosomiasis, filariasis, scrub typhus, and human immunodeficiency virus (HIV) - have been increasingly recognized as causes of glomerular disease. These infections can directly or indirectly affect the kidney, resulting in a diverse spectrum of glomerular pathologies. SUMMARY: This review highlights the epidemiology, clinical manifestat…

Genetic diversity of Salmonella enterica during acute human infections.

The ubiquitous bacterial pathogen Salmonella enterica is the causative agent of both enteric fever and gastroenteritis. Despite its significant global health burden, we lack an understanding of its genetic diversity during acute infection, with ramifications for treatment and prevention. Here, we investigated within-host infection diversity of acute salmonellosis using whole-genome sequencing of blood or stool isolates obtained from 23 different patients. We found that intestinal infections exhibited greater genetic variation than blood infections, including in their plasmid content. While sam…

Naegleria fowleri Infections: Bridging Clinical Observations and Epidemiological Insights.

PURPOSE: Naegleria fowleri is the main etiologic agent implicated in primary amoebic meningoencephalitis (PAM). It is also known as the brain-eating amoeba because of the severe brain inflammation following infection, with a survival rate of about 5%. This review aims to identify Naegleria fowleri infections and evaluate patients' progression. This literature review emphasizes the importance of rapid diagnosis and treatment of infected patients because only prompt initiation of appropriate therapy can lead to medical success. Compared to other articles of this kind, this one analyzes a large n…
