Malnutrition can be detected by evaluating the nutritional situation of the patient. It can be influenced by special nutritional support in the pre- and postoperative period. A good nutritional status will lower the rate of postoperative complications.

Presented is the case of a motor vehicle accident (MVA) victim with hypotension and evisceration of small intestine. Vigorous resuscitation in the emergency department was required, after which a portion of small bowel was resected at laparotomy. Evisceration secondary to trauma in an MVA is a rare injury. Significant blood loss occurs. Although abdominal sepsis did not occur in our case, this is a complication for which the patient must be monitored closely.

A case of a 12-month-old child with chronic water diarrhoea, weight loss, abdominal distension, hypokalaemia and hypochlorhydria, which were associated with a vasoactive intestinal peptide secreting ganglioneuroma, is reported. Removal of the tumour led to complete clinical and biochemical recovery. Measurement of vasoactive intestinal peptide levels should be included in the evaluation of chronic diarrhoea after the more common causes have been ruled out.

A diverticulum of the small intestine is a condition with only a low morbidity. This is generally a Meckel's diverticulum (MD). There is, however, the possibility of another congenital malformation, which can closely resemble a MD but has another etiology, the tubular duplication of the small intestine. In six women with postmenopausal osteoporosis, most of whom responded to 50 micrograms/day 25-hydroxyvitamin D (25-OHD) therapy with a rise in intestinal calcium (Ca) absorption, 50 mg/day hydrochlorothiazide (TZ) were added to determine whether the resulting decline in urinary Ca would cause Ca retention in the skeleton. The effects of vasoactive intestinal peptide (VIP) on mechanical activity and electrical stimulation of the bovine ciliary muscle in vitro were compared. The generated contractions were enhanced by application of eserine and were abolished completely in the presence of atropine. VIP (10(-8)-10(-6) M) enhanced the response to electrical stimulation, while the contractile amplitude of the ciliary muscle to exogenous carbachol was not altered significantly by application of 10(-7)-10(-6) M VIP. The early diagnosis of intestinal non-Hodgkin lymphoma, stages I and II, lymphoblastic of Burkitt's type, was made by colonoscopy combined with selective radiologic visualization of the tumor in two children, ages 4 and 5.
