Patient, consumer and societal values, perceptions and preferences on high-cost gene therapies.

Novel cell and gene therapies hold great potential to alter the clinical course of a range of chronic diseases, yet uncertainties remain regarding their long-term efficacy and impact. These uncertainties pose challenges for Health Technology Assessment (HTA) committees, which systematically assess health interventions seeking public subsidy. Haematological conditions are a common target for gene therapies; however, the perspectives of those both directly and indirectly impacted by funding decisions for therapies in this disease area have not been investigated. To support future HTA considerati

Perineural Administration of Botulinum Toxin for Chronic Pain Management: A Narrative Review.

BACKGROUND: Neuropathic pain occurs as a direct consequence of a lesion or a disease affecting the somatosensory system. Because of the limited therapeutic arsenal, it is difficult to treat. Nevertheless, pain relief has been demonstrated in multiple clinical trials treating neuropathic pain patients with subcutaneous or intradermal administrations of botulinum toxin. However, this type of administration route shows limits; for example, the painful area may be too large for subcutaneous administration, or administration discomfort would be intolerable for direct injection into the painful area

[How can the suitability of a geriatric patient for uro-oncological treatment be determined?].

BACKGROUND: Geriatric patients undergoing uro-oncological treatments are particularly susceptible to complications in many organ systems. Prior to starting treatment, a careful risk-benefit analysis must be performed, taking into consideration the patient's remaining life expectancy and existing functional deficits. These should be weighed against the side effect profile of the planned treatment. OBJECTIVE: After an introduction of the three-step geriatric assessment to identify geriatric patients, systematically identify functional deficits, and evaluate them in depth, the example of androge

Early Lactate Trajectories and Outcomes in Veno-Arterial Extracorporeal Membrane Oxygenation-Supported Cardiogenic Shock: A Multicenter Study.

In this multicenter retrospective study (Chinese Society of Extracorporeal Life Support [CSECLS] registry), we evaluated early lactate trajectories in 1,465 adults with cardiogenic shock surviving greater than 24 hours on veno-arterial extracorporeal membrane oxygenation (VA-ECMO). Unsupervised k-means clustering of serial lactates (baseline, 4 hours, 24 hours) defined four metabolic phenotypes. The "Persistent High" phenotype (5.7%) exhibited refractory hyperlactatemia (median 24 hour lactate 17.0 mmol/L) and the highest in-hospital mortality (85.7% vs. 27.9% in the "Low Stable" group; p < 0.

Successful Foot Salvage in a Patient With Diabetes Following Split-Thickness Skin Grafting and Antidiabetic Therapy: A Case Report.

Diabetes mellitus remains a major global health burden, particularly affecting wound healing in patients with traumatic injuries. Chronic hyperglycemia impairs tissue regeneration and immune function, increasing the risk of infection, delayed recovery, and potential limb loss. We aim to demonstrate the critical importance of glycemic control and multidisciplinary management in enhancing wound healing outcomes among diabetic patients undergoing surgical interventions. using the case of a 65-year-old male with a crush injury to the left foot following a road traffic accident. Rapid Normalization of Amniotic Fluid Index Following Discontinuation of Olmesartan: A Case Report.

BACKGROUND: Angiotensin II receptor blockers (ARBs) are commonly used for hypertension but are contraindicated in pregnancy due to risks of oligohydramnios, renal dysgenesis, and pulmonary hypoplasia from suppression of the fetal renin-angiotensin system. Olmesartan, a frequently prescribed ARB, has a longer receptor binding half-life and higher affinity than other ARBs, producing more potent and sustained antihypertensive effects. Emerging evidence suggests that stopping ARBs during pregnancy may allow recovery of amniotic fluid and renal function. CASE: A 30-year-old primigravida with chron

Mesenchymal Stromal Cell-Based Therapies in Sepsis-Induced Acute Lung and Kidney Injury: Current Advances and Perspectives.

Sepsis is a life-threatening syndrome characterized by severe immune dysregulation, frequently culminating in acute respiratory distress syndrome (ARDS) and acute kidney injury (AKI). Current supportive therapies fail to reverse the underlying pathophysiological damage. However, mesenchymal stromal cells (MSCs) have emerged as a promising therapeutic frontier due to their robust immunomodulatory, anti-inflammatory, and tissue-regenerative properties. Despite compelling preclinical evidence, translating these benefits into consistent clinical efficacy remains a major challenge. This review crit

Protein-guided regulation of redox metabolic imbalance improves survival physiology in acute respiratory distress syndrome.

Background: Acute Respiratory Distress Syndrome (ARDS) is a life-threatening condition associated with high morbidity and mortality, characterized by severe inflammation, oxidative stress, impaired gas exchange, and progressive lung injury. Disruption of cellular redox homeostasis plays a central role in ARDS pathogenesis, making redox-regulatory mechanisms attractive therapeutic targets. Methods: This review evaluates experimental evidence on protein-mediated regulation of oxidative stress pathways in ARDS, focusing on proteins involved in antioxidant defense, reactive oxygen species (ROS) ge

[Guidelines for Serotonin Syndrome in Pediatrics].

This document is presented as a narrative guide based on the available evidence, adapted to the pediatric population. Since clinical trials in children are nonexistent, much of the information comes from case reports, narrative reviews, and extrapolation from adult studies. Serotonin syndrome is a potentially life-threatening condition caused by excessive serotonergic activity within the central nervous system. It is clinically characterized by a triad of neuromuscular hyperactivity, autonomic instability, and altered mental status. Diagnosis is clinical and relies on recent exposure to seroto

Aggressive surgery for metastatic spinal tumors.

Aggressive resection in Metastatic spinal tumors (MST) includes three main en bloc techniques: vertebrectomy, hemivertebrectomy, and corpectomy. En bloc excision aims to remove the tumor in one piece without violation, ensuring appropriate pathological margins. Vertebrectomy (marginal or wide en bloc excision) is indicated when the tumor is centrally located with at least one uninvolved pedicle. Thoracic lesions are often managed in a single stage, whereas lumbar lesions usually require a two-stage approach to reduce neurological risk. Posterior mobilization precedes anterior tumor delivery, w

Future of radiotheranostics.

Radiotheranostics is reshaping precision oncology by integrating molecular imaging with targeted radionuclide therapy through matched diagnostic-therapeutic radiopharmaceutical pairs. Landmark clinical successes, including Lutetium-177-DOTATATE (177Lu-DOTATATE) and 177Lu-prostate-specific membrane antigen-617 (177Lu-PSMA-617), have validated this approach and accelerated the shift from empiric dosing towards evidence-based practice supported by randomised trials. Beyond established somatostatin receptor and PSMA paradigms, emerging platforms targeting tumour-stroma and lineage-associated biolo

Heat Stroke or Acute Coronary Syndrome? A Summer Collapse With an Unexpected Cardiac Twist.

Heat stroke is a life-threatening medical emergency characterised by severe hyperthermia and central nervous system dysfunction. Cardiac manifestations, including troponin elevation and electrocardiographic abnormalities, are well recognised and may mimic acute coronary syndrome (ACS). Distinguishing heat-related myocardial injury from true ACS can therefore be challenging. We report the case of a 60-year-old man who presented after being found unresponsive in his garden during a hot summer day. His pre-hospital core temperature exceeded 42.2°C, and he had a markedly reduced level of conscious

Chaos in the Equilibrium: A Rare Complication of Dialysis.

Dialysis disequilibrium syndrome (DDS) is a rare but potentially life-threatening neurologic complication associated with hemodialysis (HD), particularly during initial treatments in patients with severe uremia. It is thought to result from rapid osmotic shifts between the plasma and the central nervous system during dialysis. We present the case of a 36-year-old woman with stage V chronic kidney disease secondary to diffuse lupus nephritis who developed suspected DDS during her first HD session. She presented with acute-on-chronic renal failure, severe metabolic acidosis, a creatinine of 12.0

Serotonin syndrome associated with concomitant tramadol and linezolid therapy: a case report and literature review.

Serotonin syndrome (SS) is a potentially life-threatening adverse drug reaction resulting from excessive serotonergic activity in the central and peripheral nervous systems. Tramadol produces analgesia through μ-opioid receptor agonism while also inhibiting the reuptake of serotonin and norepinephrine. Linezolid an oxazolidinone antibacterial agent, possesses reversible, non-selective monoamine oxidase inhibitory activity. Through distinct pharmacological mechanisms, both agents enhance serotonergic neurotransmission and may theoretically increase the risk of SS when used concomitantly. We rep

Intra-arterial chemotherapy for retinoblastoma: a structured narrative review.

Retinoblastoma (Rb) is the most common primary intraocular malignancy in children. Intra-arterial chemotherapy (IAC), often termed ophthalmic artery chemosurgery, has emerged as an important globe-preserving treatment modality for advanced intraocular Rb. We focused on technical delivery, clinical indications, regimen selection, oncological safety, globe salvage, functional preservation and complications. Current evidence indicates that, in appropriately selected patients and experienced centers, IAC can improve local tumor control and globe salvage while reducing systemic toxicity when compar

Diagnosis, Treatment, and Prognosis of Cerebral Venous Thrombosis.

Cerebral venous thrombosis (CVT) is an uncommon but potentially severe cerebrovascular disorder that predominantly affects young adults and women. Advances in neuroimaging have improved diagnostic accuracy and have enabled better characterization of clinically relevant phenotypes, including edema patterns, venous infarction, and hemorrhagic transformation, which may influence prognosis and therapeutic strategies. Anticoagulation remains the cornerstone of treatment across disease severities. Emerging evidence supports the use of direct oral anticoagulants as safe and effective alternatives to

Low-Dose Methotrexate Toxicity Presenting With Inguinal Lesions and Pancytopenia: A Case Report.

Methotrexate (MTX) is a folate antagonist widely used as a disease-modifying antirheumatic drug (DMARD). Despite its efficacy and broad therapeutic range, its narrow safety margin and reliance on renal clearance confer risk for severe multisystem toxicity, particularly in patients with renal impairment, comorbidities, or dosing errors. We describe a 74-year-old male with coronary artery disease, type 2 diabetes, stage 3a chronic kidney disease, obesity, and seronegative rheumatoid arthritis who presented with progressive weakness, painful oral and genital ulcers, and three weeks of watery diar
