Finding the Sweet Spot? Differential Diabetes Risks in Autologous Breast Reconstruction.

BACKGROUND: Autologous breast reconstruction (ABR) offers superior quality of life compared with implant-based reconstruction or mastectomy alone but carries substantial perioperative risk. Diabetes mellitus is a known predictor of postoperative morbidity but is often treated as a binary variable, obscuring differences between patients on oral antihyperglycemic agents (diabetes managed with oral antihyperglycemic agents [DM-ORAL]) and insulin (diabetes requiring insulin therapy [DM-INS]). MATERIALS AND METHODS: Using the National Surgical Quality Improvement Program, we identified female brea…

Is Tumor Bed Boost Necessary in Young Patients With Margin-negative Invasive Ductal Carcinoma in the Modern Radiotherapy Era?

BACKGROUND/AIM: To evaluate whether tumor-bed boost irradiation improves local control outcomes in women aged 50 years or younger with invasive ductal carcinoma (IDC) and widely negative margins after breast-conserving surgery (BCS), and to clarify its clinical value within a modern, risk-adapted radiotherapy strategy. PATIENTS AND METHODS: This retrospective single-center study included 318 female patients aged ≤50 years with IDC and negative surgical margins (>5 mm) who underwent BCS followed by whole-breast irradiation (WBI) at Tohoku University Hospital between 2008 and 2022. Patients tre

An observational study comparing the dosimetry and clinical effects of conventional versus hypofractionated radiotherapy in elderly patients of early breast cancer.

INTRODUCTION: Hypofractionated radiation therapy after Breast Conserving Surgery in patients of early breast carcinoma results in equivalent clinical outcome but better cosmesis as compared to conventional therapy. In this study we have compared the dosimetry and clinical outcome between Hypofractionated and Conventional whole breast radiation followed by boost therapy after patients had undergone breast conserving surgery for early breast cancer. AIMS AND OBJECTIVES: 1. Dosimetric analysis and toxicity assessment between conventional and Hypofractionated radiation therapy.2. Comparative anal

Postmastectomy radiation therapy for implant-based breast reconstruction: a systematic review and meta-analysis for the 2022 Japanese Breast Cancer Society Clinical Practice Guideline.

BACKGROUND: Implant-based breast reconstruction is the most commonly performed reconstructive technique following mastectomy. With an increasing number of patients undergoing implant-based breast reconstruction, concerns have arisen regarding the safety of postmastectomy radiation therapy (PMRT) in reconstructed breasts. This study aimed to investigate the safety of PMRT in implant-based breast reconstruction. METHODS: A comprehensive literature search was conducted for articles published up to March 2021. Eligible studies included clinical trials and observational studies comparing outcomes …

Reduction of subclinical acute cardiac injury through DIBH radiotherapy: a single-institution real-world clinical cohort analysis.

BACKGROUND: Radiotherapy for left-sided breast cancer is associated with additional cardiac risks. Deep inspiration breath hold (DIBH) has emerged as the primary cardiac sparing technique, demonstrating significant reduction in mean heart dose (MHD) and left anterior descending coronary artery (LAD) dose. However, the issue of whether dosimetric advantages can be effectively translated into clinically measurable benefits for patients remains to be further elucidated. The study aims to investigate whether DIBH, in contrast to free breathing (FB), could yield a clinical cardiac advantage, beyond

Enhancing Aesthetics in Bilateral DIEP Flap Breast Reconstruction: the Role of Tissue Pre-Expansion.

BACKGROUND: Achieving symmetrical outcomes in bilateral autologous breast reconstruction is challenging, particularly in cases of asymmetrical recipient sites. Tissue pre-expansion is proposed to improve aesthetics by enlarging the skin envelope for refined breast shaping. This study examines its efficacy in bilateral DIEP flap reconstructions. METHODS: This study systematically evaluated 43 patients that underwent bilateral DIEP flap breast reconstruction between 2004 and 2021. The efficacy of tissue pre-expansion in enhancing aesthetic outcomes, patient demographics, and complications were

Invasive Squamous Cell Carcinoma of the Nipple: Case Report With Literature Review.

In this unusual case, a 73-year-old woman presented with bloody discharge from her right breast and skin thickening was observed on subsequent imaging. Clinically, the findings raised concern for Paget disease. A skin punch biopsy was performed, revealing a high-grade infiltrating carcinoma with squamous features. Metaplastic carcinoma was a differential diagnosis, given that this patient had a history of invasive ductal carcinoma and radiation treatment in the same breast. A subsequent central lumpectomy confirmed the diagnosis of invasive squamous cell carcinoma of the nipple, occurring in t

Does Non-Classic Lobular Carcinoma In Situ at the Lumpectomy Margin Increase Local Recurrence?

BACKGROUND: The clinical significance of nonclassic, lobular carcinoma in situ (NC-LCIS) at the surgical margin of excisions for invasive cancer is unknown. We sought to determine whether NC-LCIS at or near the margin in the setting of a concurrent invasive carcinoma is associated with risk of ipsilateral breast tumor recurrence (IBTR) and locoregional recurrence (LRR). METHODS: Patients with stage 0-III breast cancer and NC-LCIS who underwent lumpectomy between January 2010 and January 2022 at a single institution were retrospectively identified. NC-LCIS margins were stratified as <2 mm, ≥2
