The role of prenatal exposures on body fat patterns at 7 years: Intrauterine programming or birthweight effects?

BACKGROUND AND AIMS: It remains unknown whether the effects of prenatal exposures on child's adiposity reflect entirely intrauterine programming. We aimed to assess the effects of maternal gestational weight gain, diabetes and smoking on the child's body fat patterns, disentangling the direct (through intrauterine programming) and indirect (through birthweight) effects. METHODS AND RESULTS: We included 4747 singleton 7-year-old children from the Generation XXI birth cohort (Porto, Portugal). At birth, maternal and newborn's characteristics were obtained. Anthropometrics were measured at age 7

Placental transfer of antibiotics administered to the mother: a review.

BACKGROUND: The purpose of antibiotic treatment in pregnant women is to treat the mother and/or the fetus since it is known that antibiotics administered to the mother cross the placenta and reach the fetus. A comparison of the drug concentration in maternal and fetal plasma gives an indication of the exposure of the fetus to the maternally administered antibiotics. AIM: The aim of this study was to review the literature pertaining to the placental transfer of antibiotics in man and to classify the antibiotics according to the type of transfer involved. A table has been developed for use by p

Prenatal Exposure to Delta 9-Tetrahydrocannabinol and Child Neurodevelopment: A Critical Windows Analysis.

OBJECTIVE: To identify critical windows of prenatal exposure to delta 9-tetrahydrocannabinol (Δ9-THC) and child neurodevelopment. STUDY DESIGN: We followed 460 mother-child pairs participating in the Healthy Start study through age 5 years. Δ9-THC and its metabolites were measured at about 17 weeks' gestation (n=460) and about 27 weeks' gestation (n=434) in maternal serum, and at birth via cord blood (n=238). At age 5 years, behavior was assessed via Child Behavior Checklist (CBCL) and cognition was assessed via NIH Toolbox. For each outcome, we fit a generalized estimating equation model wit

Association Between Meconium Acetaminophen and Childhood Neurocognitive Development in GESTE, a Canadian Cohort Study.

Acetaminophen is the only over-the-counter pain reliever that is not contraindicated during pregnancy, but recent studies have questioned whether acetaminophen is safe for the fetus, particularly the developing brain. This prospective birth cohort study probed the previously observed association between in utero exposure to acetaminophen and neurodevelopment by using concentrations of acetaminophen measured in meconium, which more objectively captures exposure of the fetus than maternal report. Exposure, measured by liquid chromatography coupled with tandem mass spectrometry, was categorized i

The emerging spectrum of fetal acetylcholine receptor antibody-related disorders (FARAD).

In utero exposure to maternal antibodies targeting the fetal acetylcholine receptor isoform (fAChR) can impair fetal movement, leading to arthrogryposis multiplex congenita (AMC). Fetal AChR antibodies have also been implicated in apparently rare, milder myopathic presentations termed fetal acetylcholine receptor inactivation syndrome (FARIS). The full spectrum associated with fAChR antibodies is still poorly understood. Moreover, since some mothers have no myasthenic symptoms, the condition is likely underreported, resulting in failure to implement effective preventive strategies. Here we rep

Alcohol Consumption During Pregnancy and Conduct Problems: A Brief Overview of the Literature.

Alcohol is widely consumed globally, with significant risks linked to both mental and physical health conditions. Alcohol exposure during pregnancy is prevalent, with nearly 10% of pregnant women worldwide consuming alcohol. There is no safe limit for alcohol intake during pregnancy, as even minimal exposure can cause severe developmental issues. This narrative review examines the impact of alcohol consumption during pregnancy on the development of specific conduct problems in the offspring, namely Oppositional Defiant Disorder (ODD) and Conduct Disorder (CD). A comprehensive review of existin

Liquid crystal monomers induce placental development and progesterone release dysregulation through transplacental transportation.

Embryonic and fetal development can be affected during gestation by exposure to xenobiotics that cross the placenta. Liquid crystal monomers (LCMs) are emerging contaminants commonly found in indoor environments; however, whether they can cross the placenta and affect placental development remains unexplored. Here, we develop an evaluation system that integrates human biomonitoring, uterine perfusion in pregnant rats, and placental cells. We find fourteen out of the fifty-six LCMs that are detected in maternal and cord serum samples from ninety-three healthy pregnant women, at median levels of

Dynamic overrepresentation of accumbal cues in food- and opioid-seeking rats after prenatal THC exposure.

The increasing prevalence of cannabis use during pregnancy has raised medical concerns, primarily related to Δ9-tetrahydrocannabinol (THC), which readily crosses the placenta and affects fetal brain development. Previous research has identified dopaminergic alterations related to maternal THC consumption. However, the consequences that prenatal cannabis exposure (PCE) has on striatum-based processing during reward pursuit have not been determined. Here, we characterize PCE rats during food or opioid-maintained reward seeking. We find that the supramotivational phenotype of PCE rats is independ

Dioxin male rat reproductive toxicity mode of action and relative potency of 2,3,7,8-tetrachlorodibenzo-p-dioxin and 2,3,7,8-tetrachlorodibenzofuran characterized by fetal pituitary and testis transcriptome profiling.

Fetal rat exposure to 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) reduces epididymal sperm number involving altered pituitary-testicular hormonal signaling as the proposed mode-of-action (MOA). To evaluate this MOA and compare TCDD to 2,3,7,8-tetrachlorodibenzofuran (TCDF), an in utero rat exposure and study was conducted. Endpoints included congener tissue levels and transcriptomes of maternal liver and fetal liver, testis, and pituitary. Decreased gonadotropin subunit mRNAs levels (Lhb and Fshb) and enriched signaling pathways including GNRH Signaling and Calcium Signaling were observed in fe

Effects of dietary exposure to ciguatoxin P-CTX-1 on the reproductive performance in marine medaka (Oryzias melastigma).

Ciguatoxins are natural compounds produced by benthic dinoflagellates Gambierdiscus and Fukuyoa spp., which cause fish intoxication by ciguatera fish poisoning. This study aimed to assess the dietary exposure effects of ciguatoxin P-CTX-1 on the reproductive performance in marine medaka (Oryzias melastigma). Fish which ingested >1.16 pg·day-1 for 21 days exhibited abnormal behaviors including diarrhea, abnormal swimming, loss of appetite and decreased egg production. After 7-day exposure to P-CTX-1 at a dose of 1.93 pg·day-1, significant gender-specific differences in reproductive performance

Prenatal SSRI exposure: Effects on later child development.

The aim of this review is to integrate research on the pharmacological mechanisms of selective serotonin reuptake inhibitors (SSRIs) and the following effects on fetal brain development and child cognitive function seen in children with prenatal exposure to SSRIs. As antidepressants are transferred from the mother to the fetus through the placenta, the fetus is vulnerable to alterations in neurotransmission and possibly altered neural functioning. Because of this risk, pregnant women suffering from depression are often advised to discontinue their use of antidepressants during pregnancy. Thoug

Prenatal alcohol use and offspring size at 10 years of age.

The Maternal Health Practices and Child Development Project is a longitudinal study of the effects of prenatal exposure to alcohol and other substances. Women were selected from a prenatal clinic and interviewed at the 4th and 7th months of pregnancy. Their offspring were examined at delivery, at 8 and 18 months, and at 3, 6, and 10 years. This report examined 610 offspring, at age 10, who were exposed prenatally to alcohol. Most alcohol use in this low-income cohort was light to moderate, although the entire spectrum of alcohol use is represented. The weight, length, head circumference, and s

[New recommendations for maternal-infant care in HIV].

In 2023, BREACH («Belgium Research on AIDS and HIV Consortium») presented new Belgian consensus guidelines for perinatal management of HIV-positive mothers. This article aims to highlight them. We review maternal management, discussing the importance of antiretroviral agents during pregnancy, the recommended follow-up and the preferred mode of delivery. The specificities for the infant will be addressed and include management at birth, post-exposure prophylaxis, new developments regarding breast-feeding and necessary follow-up.

Case Study of Intrapartum Antibiotic Prophylaxis and Subsequent Postpartum Beta-Lactam Anaphylaxis.

Universal screening for maternal group B Streptococcus (GBS) in the prenatal period has led to administration of intrapartum antibiotic prophylaxis (IAP). Although IAP decreased the rate of early neonatal GBS disease, exposure of childbearing women to penicillin and other beta-lactam antibiotics has increased. Beta-lactam-induced anaphylaxis in the breastfeeding woman during the postpartum period illustrates risk factors for beta-lactam allergy and anaphylaxis. Treatment and nursing implications for this adverse reaction are suggested.

Asymmetric dimethylarginine in ELBW newborns exposed to chorioamnionitis.

We measured circulating ADMA concentrations in a group of very premature newborns at birth and during the first week of life. ADMA levels resulted significantly higher in infants born to mothers with histologic chorioamnionitis than in infants delivered for other maternal or fetal indications, both at birth and through the first week of life. We speculate that ADMA might be involved in the complex biological events associated with fetal exposure to chorioamnionitis.

Severe congenital rickets secondary to maternal hypovitaminosis D: a case report.

We report a full-term baby boy who presented soon after birth with severe congenital rickets. Maternal and neonatal vitamin D levels were very low and the infant responded well to oral vitamin D. Transient secondary hyperparathyroidism normalised on treatment. The mother's vitamin D deficiency was attributed to the region's cultural dress code which prevents exposure to sunlight. There has not been a previous report of severe congenital rickets from this region.

Neonatal side effects of maternal labetalol treatment in severe preeclampsia.

OBJECTIVE: Labetalol is often used in severe preeclampsia (PE). Hypotension, bradycardia and hypoglycemia are feared neonatal side effects, but may also occur in (preterm) infants regardless of labetalol exposure. We analyzed the possible association between intrauterine labetalol exposure and such side effects. STUDY DESIGN: From 1 January 2003 through 31 March 2008, all infants from mothers suffering severe PE admitted to one tertiary care center were included. Severe PE was defined according to the International Society for the Study of Hypertension in Pregnancy (ISSHP) criteria. Infants e
