Extensive fundal placenta accreta spectrum (PAS) in a nulliparous patient with an unscarred uterus and systemic lupus erythematosus (SLE) necessitating cesarean hysterectomy at delivery.

Placenta accreta spectrum (PAS) defined by various degrees of abnormally invasive placentation is strongly associated with severe maternal morbidity and maternal mortality, mainly reflecting maternal hemorrhage. Predisposing factors for this condition include: multiparity, placenta previa, previous uterine surgeries (previous curettage, cesarean delivery, and myomectomy), smoking and previous PAS. Prenatal sonographic findings associated with PAS include the presence of placenta previa, loss of the hypoechoic retroplacental sonolucency, myometrial thinning, interruption of the irregularity of …

Maternal and neonatal peripartum factors associated with late initiation of breast feeding in Bangladesh: a secondary analysis.

OBJECTIVES: Late initiation of breast feeding (LIBF) is associated with increased neonatal mortality and morbidity. This study aimed to assess the association between intrapartum, early postpartum and neonatal factors, and LIBF in Bangladesh. DESIGN, SETTING AND PARTICIPANTS: In this analysis, we used data from the mothers participating in a cluster-randomised controlled trial (Rang-Din Nutrition Study) conducted in rural northwest Bangladesh. Mothers (n=3594) were interviewed about the time of initiation of breast feeding, and peripartum maternal and neonatal complications within the first 7

Protection against Congenital CMV Infection Conferred by MVA-Vectored Subunit Vaccines Extends to a Second Pregnancy after Maternal Challenge with a Heterologous, Novel Strain Variant.

Maternal reinfection of immune women with novel human cytomegalovirus (HCMV) strains acquired during pregnancy can result in symptomatic congenital CMV (cCMV) infection. Novel animal model strategies are needed to explore vaccine-mediated protections against maternal reinfection. To investigate this in the guinea pig cytomegalovirus (GPCMV) model, a strictly in vivo-passaged workpool of a novel strain, the CIDMTR strain (dose, 1 × 107 pfu) was used to infect dams that had been challenged in a previous pregnancy with the 22122 strain, following either sham-immunization (vector only) or vaccinat

Risk factors for peripartum hysterectomy among women with postpartum haemorrhage: analysis of data from the WOMAN trial.

BACKGROUND: Peripartum hysterectomy can cause significant morbidity and mortality. Most studies of peripartum hysterectomy are from high income countries. This cohort study examined risk factors for peripartum hysterectomy using data from Africa, Asia, Europe and the Americas. METHODS: We used data from the World Maternal Antifibrinolytic (WOMAN) trial carried out in 193 hospitals in 21 countries. Peripartum hysterectomy was defined as hysterectomy within 6 weeks of delivery as a complication of postpartum haemorrhage. Univariable and multivariable random effects logistic regression models we

Regional differences in severe postpartum hemorrhage: a nationwide comparative study of 1.6 million deliveries.

BACKGROUND: The incidence of severe postpartum hemorrhage (PPH) is increasing. Regional variation may be attributed to variation in provision of care, and as such contribute to this increasing incidence. We assessed reasons for regional variation in severe PPH in the Netherlands. METHODS: We used the Netherlands Perinatal Registry and the Dutch Maternal Mortality Committee to study severe PPH incidences (defined as blood loss ≥ 1000 mL) across both regions and neighborhoods of cities among all deliveries between 2000 and 2008. We first calculated crude incidences. We then used logistic multil…

Serum levels of growth arrest specific protein 6 are increased in preeclampsia.

Preeclampsia (PE) contributes to maternal and fetal morbidity and mortality worldwide. Moreover, it is associated with an increased future metabolic and cardiovascular risk for mother and newborn. Recently, growth arrest specific protein (Gas) 6 has been introduced as a novel metabolic risk factor with anti-angiogenic, pro-atherogenic, and pro-adipogenic properties. In the current study, we investigated serum concentrations of Gas6 in patients with PE (n=51) as compared to healthy, age-matched controls (n=51) during and 6 months after pregnancy. Furthermore, association of Gas6 with markers of

Maternal complications and the association with baseline variables in pregnant women with sickle cell disease.

Sickle cell disease is an inherited hemoglobinopathy with multi system complications. It has been associated with multiple maternal complications. A retrospective review of 68 consecutive pregnant women with sickle cell disease, followed in a tertiary center, was conducted over 5 years, to estimate the incidence of different maternal complications and the impact of baseline characteristics. Sixty-eight patients were analyzed (mean age 30 years). Sixty-two patients had a Hb SS genotype. The initial mean hemoglobin (Hb) level was 9.5 g/dL. Twelve patients delivered by Cesarean section. Sixty-fiv

Adrenomedullin, calcitonin gene-related peptide and their receptors: evidence for a decreased placental mRNA content in preeclampsia and HELLP syndrome.

OBJECTIVE: The human placenta expresses a variety of vasoactive substances and neuropeptides, which play an important role in the regulation of placental blood flow in both the maternal and foetal compartment and are therefore of critical importance for foetal growth and development. Our study was planned to examine placental mRNA amounts of vasodilatory adrenomedullin (AM), calcitonin gene-related peptide (CGRP) and their receptors (AM-R and CGRP-R) in preeclampsia and HELLP syndrome (hemolysis, elevated liver enzymes, low platelets). These are severe maternal conditions leading to an altered

Beyond 'Go and bring your husband': a COM-B guided qualitative study on the barriers to male involvement in antenatal care in Bamenda Health District, Cameroon.

Maternal and infant mortality remains a major public health concern in sub-Saharan Africa. While male involvement in pregnancy and child birth has proven to be an effective intervention that can significantly reduce these deaths, low-income settings like Cameroon continue to encounter obstacles in engaging men in maternal and child health services. This study seeks to examine and contextualize barriers to male partner involvement in antenatal care in order to inform the development of an intervention aimed at promoting male participation in maternal and child health in Cameroon. We employed a

The Relationship Between Maternal and Neonatal Microbiota in Spontaneous Preterm Birth: A Pilot Study.

The newborn's microbiota composition at birth seems to be influenced by maternal microbiota. Maternal vaginal microbiota can be a determining factor of spontaneous Preterm Birth (SPPTB), the leading cause of perinatal mortality. The aim of the study is to investigate the likelihood of a causal relationship between the maternal vaginal microbiota composition and neonatal lung and intestinal microbiota profile at birth, in cases of SPPTB. The association between the lung and/or meconium microbiota with the subsequent development of bronchopulmonary dysplasia (BPD) was also investigated. Maternal

Do Maternal Living Arrangements Influence the Vaccination Status of Children Age 12-23 Months? A Data Analysis of Demographic Health Surveys 2010-11 from Zimbabwe.

INTRODUCTION: Although vaccination is an effective intervention to reduce childhood mortality and morbidity, reasons for incomplete vaccination, including maternal living arrangements, have been marginally explored. This study aims at assessing whether maternal living arrangements are associated with vaccination status of children aged 12-23 months in Zimbabwe. It also explores other variables that may be associated with having children not fully vaccinated. MATERIALS AND METHODS: A cross-sectional analysis was performed on the DHS-VI done in Zimbabwe in 2010-2011 (response rate 93%). Incompl

Inclusion of non-viable neonates in the birth record and its impact on infant mortality rates in Shelby County, Tennessee, USA.

Rates of infant death are one of the most common indicators of a population's overall health status. Infant mortality rates (IMRs) are used to make broad inferences about the quality of health care, effects of health policies and even environmental quality. The purpose of our study was threefold: i) to examine the characteristics of births in the area in relation to gestational age and birthweight; ii) to estimate infant mortality using variable gestational age and/or birthweight criteria for live birth, and iii) to calculate proportional mortality ratios for each cause of death using variable

Urban-rural disparities in maternal healthcare knowledge, attitudes, and practices among women with a recent live birth in Abuja and Gombe states, Nigeria.

BACKGROUND: Nigeria bears the highest burden of maternal mortality globally, with marked urban-rural disparities. Rural women have significantly lower uptake of skilled antenatal care (60.7% vs. 91.8%) and facility-based delivery (48.5% vs. 81.3%). However, comparative data on maternal knowledge, attitudes, and practices (KAP) remain limited in northern Nigeria. This study compared maternal healthcare KAP between urban and rural women and identified predictors of good outcomes. METHODS: A community-based cross-sectional study was conducted in Abuja (urban) and Gombe State (rural) from June to…

Effect of delayed cord clamping on maternal and neonatal outcomes during cesarean delivery in twin pregnancies.

INTRODUCTION: Compared with early cord clamping (ECC), delayed cord clamping (DCC) facilitates improved placental transfusion and increases blood volume in singleton pregnancies. However, evidence supporting the use of DCC in twins delivered by cesarean section is limited. The objective of this study was to evaluate the impact of DCC versus ECC on both maternal and neonatal outcomes in twin pregnancies delivered through cesarean section. METHODS: We conducted a retrospective cohort study that included 280 twin pregnancies at ≥28 weeks that were delivered by cesarean section. A comparison of m

Optimizing intermittent preventive treatment for malaria in pregnancy: protocol for phase IV randomized controlled trial of sulphadoxine-pyrimethamine, dihydroartemisinin-piperaquine, and combination therapy in Northern Nigeria.

BACKGROUND: Malaria during pregnancy remains an important cause of maternal and neonatal morbidity and mortality in sub-Saharan Africa. Currently, intermittent preventive therapy with sulphadoxine-pyrimethamine (IPTp-SP) is the standard of care. However, increasing resistance of Plasmodium falciparum to sulphadoxine-pyrimethamine (SP) raises concerns about its effectiveness. Recently, Dihydroartemisinin-piperaquine (DP), an artemisinin-based combination therapy with prolonged post-treatment prophylactic activity, has demonstrated superior parasitological efficacy compared with SP. Despite thes

Role of preconception nutrition supplements in maternal anemia and intrauterine growth: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Impaired intrauterine growth, a significant global health problem, contributes to a higher burden of infant morbidity and mortality, mainly in resource-poor settings. Maternal anemia and undernutrition, two important causes of impaired intrauterine growth, are prioritized by global nutrition targets of 2030. We synthesized the evidence on the role of preconception nutrition supplements in reducing maternal anemia and improving intrauterine growth. METHODS: We undertook a review of the randomized controlled trials (RCTs) assessing the effect of preconception nutrition supplements o
